Memorandum
TO: HW556 Contract File FROM: Walsta Jean-Baptiste, Contract Manager SUBJECT: Star Cleaners; Interim Source Removal Implementation; DC029D DATE: November 2, 2020
This Task Assignment authorizes Cardno to implement the approved Interim Source Removal Plan in accordance with the Cardno scope of work, cost estimate form, bid documentation, and schedule received on November 2, 2020.
Subtask 1: Project Management Project management costs are calculated at 12.5 % of the direct labor cost.
Subtask 2: SVE system construction/installation and startup This subtask includes costs for system installation and startup. Based on a review of available FDEP treatment systems, Cardno determined that it was necessary to purchase a system. Because the system is being purchased by the installation subcontractor the cost for SVE equipment has not been broken out separately on the cost estimate form. The subcontractor handling fee is calculated in accordance with the contract at a rate of 10% for the first $50,000 and 7% for the remaining costs. Cardno received three responsive bids and selected the lowest responsive bidder. This subtask also includes $450 in handling fees for utility account setup (electrical and telemetry).
The original property cost was shown as $50,500. The cost has been corrected to exclude carbon and subcontractor costs for SVE equipment assembly. The invoice schedule now shows a cost of $31,800 for property based on the SVE system equipment costs. The adjustment did not result in a cost change.
Subtask 3: Remedial Action Startup Report This subtask includes costs for startup reporting.
The final cost associated with this Task Assignment is $115,023.75 with 274 hours.

FDEP Facility ID: ERIC_4613
Task Assignment: HW556-DC029D
Dear Walsta:
Cardno Inc. (Cardno) is pleased to submit this Task Assignment Request for Soil Vapor Extraction (SVE) construction and installation at the Star Cleaners Depot (ERIC_4613) site. Cardno completed an Interim Source
Removal Plan (ISRP), dated March 27, 2020 (revised September 9, 2020) for the above referenced site, and approved by the Florida Department of Environmental Protection (FDEP) on September 16, 2020.
The implementation of the ISRP requires the following tasks:
• Permitting;
• Schedule coordination with property owner and tenant;
• Modification previously installed Modified Active Gas Sampling (MAGS) wells (M-2 and M-4) into vapor extraction (VE) wells (VE-1 and VE-2, respectively);
• Modification of previously installed MAGS wells (M-3 and M-5) into air injection wells;
• Modification of previously installed MAGS wells (M-1) into vacuum measurement points (VMPs) with protective manhole completion;
• Trenching and installation of underground and above ground piping and appurtenances (including pressure testing) to connect VE wells (VE-1 and VE-2) and air injection wells (M-3 and M-5) to the SVE system compound;
• Construction of SVE system compound and manifold;
• Construction of an enclosed skid mounted SVE system with a 3 horsepower (hp) Ametek Rotron
Regenerative Blower (EN656, or equivalent), a 1 hp Ametek Rotron Regenerative Blower (EN404, or equivalent), and condensate tank with a 0.5 hp Myers CT-05 pump for entrained water, control panel including telemetry system with wireless access;
• Utility account setup and utility hookup to the SVE system (electric and telemetry);
• Baseline groundwater sampling (MW-1 to MW-4, MW-7, MW-8, MW-10, and MW-15, and a duplicate sample), per Table 8 of the ISRP, and laboratory analysis of volatile organic halocarbons (VOHs) by
United States Environmental Protection Agency (USEPA) Method 8260;
