TO: HW556 Contract File
FROM: Walsta Jean-Baptiste SUBJECT: Interim Source Removal Implementation; DC026D
Plaza Dry Cleaners & Laundry (ERIC_5294) DATE: June 7, 2021
This Task Assignment authorizes Cardno to oversee interim source removal implementation in accordance with the approved work plan, Cardno scope of work, bid documentation, cost estimate, and schedule received on June 3, 2021.
Cardno requested bids from three subcontractors but only received two responses. Based on overall cost as well as estimated time to complete the implementation work, Petrotech SE is the proposed subcontractor. A revised bid to include utility hookup and SVE transportation to the site was only requested from Petrotech, as the low bidder. A suitable FDEP system was located after bids were initially requested from implementation subcontractors. The number of days onsite estimated by the subcontractors even with the additional work (SVE transportation and electrical hookup) for Petrotech remains lower than Equipment with Experience resulting in a lower cost for Cardno oversight. Using Petrotech is also preferred because in addition to lower costs for Cardno oversight, the facility is currently a pharmacy so a pharmacist is required to be onsite for all interior work.
The subcontractor handling fee for Petrotech was calculated correctly at a rate of 10% for the first fifty thousand and 7% for the second fifty thousand. A utility handling fee of $450 is also included for account setup related to electric ($225) and telemetry ($225).
Changes include several corrections to equipment rates and usage to match the days onsite and allowable contract amounts. The PM also requested that Cardno adjust the Petrotech bid to show unit rate costs where appropriate. There was no change in the Petrotech cost related to the clarification.
The cost related to this Task Assignment is $114,513.39 with 270 hours.

RE: Task Assignment Request for Soil Vapor Extraction System Installation and Construction Activities
Plaza Dry Cleaners
934 Patricia Ave., Dunedin, Pinellas County, Florida
FDEP Facility ID: ERIC_5294
Task Assignment: HW556-DC026D
Dear Walsta:
Cardno Inc. (Cardno) is pleased to submit this Task Assignment Request for Soil Vapor Extraction (SVE) modification and installation at the Plaza Dry Cleaners (ERIC_5294) site. Cardno completed an Interim Source
Removal Plan (ISRP), dated March 4, 2020 (revised August 5, 2020) for the above referenced site, and approved by the Florida Department of Environmental Protection (FDEP) on August 7, 2020.
The implementation of the ISRP requires the following tasks:
Permitting;
Schedule coordination with property owner and tenant;
Modification previously installed Modified Active Gas Sampling (MAGS) wells (M-1, M-6, and M-7) into vapor extraction (VE) wells (VE001 through VE003, respectively);
Installation of one (1) VE well (VE004) to a depth of approximately 24 feet below land surface (ft bls)
(adjustments to the total depth and screen interval may be required based on depth to water measured at
MW-Unk during the time of installation so the VE well is approximately 1 foot above the water table) and three (3) monitoring wells (MW-9 through MW-11) to a total depth of 70 ft bls terminating in 10 feet of
0.01-inch factory slotted screen;
Modification of previously installed MAGS wells (M-2 through M-5, M-8, and M-9) into vacuum measurement points (VMPs) with protective manhole completion;
Trenching and installation of underground and above ground piping and appurtenances (including pressure testing) to connect VE wells (VE001 through VE004) to the SVE system compound;
Mobilization of the McSherry’s SVE system (Property ID number: 138631) located at 11612 N Dale
Mabry Highway, Tampa, Hillsborough, to the Plaza Dry Cleaners site;
Construction of SVE system compound and manifold;
Utility account setup and utility hookup to the SVE system (electric and telemetry);
Baseline groundwater sampling (MW-2 to MW-4, MW-6, MW-9, MW-10, and MW-11, and a duplicate sample), per Table 1, and laboratory analysis of volatile organic halocarbons (VOHs) by United States
Environmental Protection Agency (USEPA) Method 8260;

Submittal of property reporting form, chain of custody (if applicable), and Attachment M Replacement of
Major Equipment (if applicable) and associated documentation; and, Prepare a Remedial Action System Startup Report to include updated tables, figures, laboratory data, summary of remedial actions, conclusions, and Construction Drawings.
Subcontractor Selection
For the onsite construction activities and the construction of the SVE system, bids were solicited from Preferred
Drilling Service, Equipment with Experience, and Petrotech SE. Preferred Drilling Service was deemed nonresponsive. As the lowest bidder and the lowest number of field days, Petrotech SE is selected to perform the onsite construction activities. At the time of bidding, an FDEP owned SVE system was not available to transport. As the system at McSherry’s Dry Cleaner Inc. (ERIC_4787) became available, Petrotech SE was requested to provide updated costs to transport and install electrical and electrical connections to the system. A copy of each bid, and the proposed subcontractor scope of work is provided in Appendix A.
Schedule to Complete
The proposed deliverables and due dates are summarized below and are based on the assumption the SVE system will take approximately 6 weeks to identify, mobilize, and install:
Subtask 1: Project Management
Subtask 2: SVE Construction and Installation
Deliverable: Submittal of field documentation, subcontractor submittals (permits, etc.), groundwater and vapor laboratory data, property reporting forms, and photodocumentation
Deliverable Due Date: August 31, 2021
