RE: Task Assignment Request for Six (6) Months Operations and Maintenance
Phenix Supply Company
4763 Distribution Drive, Tampa, Hillsborough County, Florida
FDEP Facility ID: ERIC_4764
Task Assignment: HW556-DC005I
Dear Ms. Jean-Baptiste:
Cardno Inc. (Cardno) is pleased to submit this Task Assignment Request for six (6) months of soil vapor extraction (SVE) system operations and maintenance (O&M) and groundwater sampling activities at the above referenced site. This proposal is for conducting O&M from June 2021 through December 2021.
During each monthly site inspection routine O&M will be conducted and include the following:
• Check system operation during each visit;
• Check/change oil in pump(s), if required;
• Check the water level in the moisture separator; if necessary, accumulated water in the moisture separator will be drained, treated by carbon adsorption and discharged to the pavement. If the groundwater entrainment rate is significant, the wellhead vacuum levels will be reduced. One sample of accumulated water will be collected for laboratory analysis of volatile organic compounds (VOCs) by United States Environmental Protection Agency (USEPA) Method 8260 for generator knowledge (using a work change directive, if required).
• Collect gauge readings from the manifold and remedial equipment during each visit to verify system performance;
• Collect vacuum or pressure readings from the vapor extraction wells/air injection points and vacuum distribution monitoring points (Table 1) and optimize SVE system according to monitoring results; and,
• Collection of depth to water (DTW) measurements from monitoring wells (Table 2).
Florida Department of Environmental Protection - Waste Site Cleanup Section Phenix Supply Company (ERIC 4764) Task Assignment Request DC005I – Six (6) Months Operations and Maintenance Cost Proposal
The SVE System O & M activities include the following subtasks:
Task 1: Project Management
Costs associated with this subtask are based on 12.5% of the direct labor costs of the remaining subtasks.
Task 2: O&M (6 Months) and Semi-Annual Remedial Action Status Report
This subtask is for O&M activities for six (6) months of O&M and a Semi Annual Remedial Action
Status Report. As part of the six (6) months of O&M, the site will be visited monthly. In addition to the routine O&M tasks, the following activities will be conducted:
• Month 15 – Collection of a vapor sample for the influent for laboratory analysis of VOHs by
USEPA Method TO-15 (standard turnaround time) (Table 1);
• Month 18 – Collection of a vapor sample for the influent for laboratory analysis of VOHs by
USEPA Method TO-15 (standard turnaround time) (Table 1), and collection of groundwater samples from TMW-4, MW-7 (and a duplicate), MW-8, MW-10, MW-14, and MW-16 for laboratory analysis of VOHs by USEPA Method 8260 (Table 2).
A Semi-Annual Remedial Action Status Summary (RASS) report will be submitted summarizing the remedial status (both SVE and groundwater amendment), tabulation of current and historic field measured parameters and laboratory analytical results, treatment system radius of influence maps (if applicable), summary of site visits and sampling events, conclusions and recommendations, and Professional
Engineering certification.
Additionally, updated Geographical Information System (GIS) Data Table updates and associated laboratory electronic data deliverables will be submitted.
Subtask 3: Utilities (6 Months)
The estimated costs of utilities is approximately $1,000.00 per month for electricity and $30.00 per month for telemetry. Electrical consumption is based on the historical costs at the site.
A utility bill handling fee of $20.00 per month for electric (fixed fee) is included in subtask 3.
Actual utility costs incurred during the six (6) months O&M time period will be billed to FDEP. Unused estimated utility costs will be returned, or increased utility costs, will be addressed by a change order request.
Subcontractor Selection
Investigative derived waste (IDW), generated from purge water, will be containerized onsite in a 55-gallon drum. Costs for transportation and disposal were solicited from Erwin Remediation, Perma-Fix
Environmental Services, and Petrotech Southeast, Inc. As the lowest bidder, Erwin Remediation is selected to transport and dispose of IDW generated.
Subcontractor costs are included in Attachment A.
