Big Lagoon State Park Amphitheater Inspection CN825, Task Assignment

No.

Subtask 1: Perform Ceiling Inspection and Provide Report

Description:

JGP will perform on-site field inspection of the existing ceiling system conditions for the Amphitheater structure located at Big Lagoon State Park. The information obtained during our visits will be utilized to formulate an inspection report. This work will be based upon the current observed conditions of the existing ceiling system at the time of our visit. Field observations may be impacted and limited by existing site conditions. Items to be reviewed will include the following:

Attachment to framing Condition of panel framing secured to joists Condition and spacing of panel fasteners Condition of steel bar joists Condition of tectum panels Review of amount of corrosion on various elements and fasteners

When determining areas of concern, due to the observation techniques used, only visible conditions can be noted. It must be understood that our work does not attempt to identify hidden conditions, damage and defects relative to the structure. The extents of some conditions may not be known until removal of the overlying finishes is performed.

Existing architectural documents and available shop drawings are to be provided prior to our inspection.

No inspection work beyond what has been described will be performed.

The work will be performed with the degree of skill and the standard of care common to the profession. No warranty is expressed or implied performing the noted scope of work.

It is anticipated that various panels will require removal to allow observation of the supporting framing and steel bar joists. JGP will not be responsible for elements that cannot be simply reinstalled as they were removed. If the panels cannot be reinstalled, a temporary sheet of polyethylene sheeting will be placed over the panel opening.

The information from this investigation will be utilized to formulate repair methods and construction documents.

Deliverable:

JGP will submit an inspection report regarding the observed ceiling system conditions for the Amphitheater. The inspection will be performed within 21 days of receiving the executed task assignment and the report will be provided within 7 days after performing the inspection work. The report will be provided in pdf format and may contain plans, photographs, tables, etc.

Performance Measure:

Deliverables will be reviewed by the Department to confirm they are complete, accurate, and timely.

Fee:

Fees for the noted work will be performed on a lump sum basis. Our hourly rates are provided and will be multiplied by the number of hours expended for the noted tasks. The cost of the equipment rental will be charged as a reimbursable cost item. It is possible that the equipment cost could be more or less than the noted amount. If this cost is different, a change order request will be submitted. Additionally, based on unknown conditions, it is possible that the expended hours may be more or less than the noted amount, whereby a change order request will be provided.
