About this file

This is a Conflict of Interest Questionnaire form issued by the Texas Ethics Commission for vendors conducting business with local governmental entities in Texas. The form applies to Collin County's solicitation for Testing, Adjusting and Balancing (TAB) and Commissioning (Cx) professional services on an as-needed basis. The questionnaire must be completed and filed with the local governmental entity's records administrator no later than the seventh business day after the vendor begins discussions or negotiations with the entity, submits an application or response to a request for proposals or bids, or becomes aware of triggering conditions related to conflicts of interest. The form requires vendors to disclose any employment or business relationships with local government officers or their family members, any gifts provided to such officers or family members, and any family relationships with local government officers that could create conflicts of interest under Texas Local Government Code Chapter 176.

The questionnaire establishes disclosure thresholds requiring vendor reporting when a local government officer or family member receives taxable income exceeding $2,500 from the vendor during the preceding 12-month period, or when the vendor has provided gifts with an aggregate value exceeding $100 to an officer or family member within the same timeframe. Vendors must also disclose any business relationships with corporations or entities where a local government officer serves as an officer, director, or holds one percent or more ownership interest. Failure to file the required questionnaire or knowingly violating disclosure requirements constitutes a misdemeanor offense under Texas law. The form applies to any vendor with a business relationship as defined by Section 176.001(1-a) that does not fall within specific exemptions related to regulated transactions, publicly available pricing, or chartered financial institutions.
