About this file

This is a certification statement document for bidders responding to a quote request from the Ohio Department of Veterans Services-Georgetown for a Hobart Legacy+ HL200 20QT Commercial planetary stand mixer with Guard and Accessories. The document outlines mandatory certification requirements that bidders must complete to qualify for applicable bid preferences, including Domestic Source End Products preference, Ohio preference, and Veteran Friendly Business Enterprise (VBE) preference. Bidders are required to certify the country of origin for products and services being offered, with specific documentation needed if products are manufactured outside the United States but contain at least 50 percent domestic components. False or misleading certifications result in immediate disqualification and potential legal action including debarment.

The certification statement requires bidders to complete applicable sections based on their business status and product sourcing. Bidders claiming Ohio preference must demonstrate that products are raised, grown, produced, mined, or manufactured in Ohio, or alternatively establish significant economic presence within Ohio by meeting criteria such as paid state taxes, Ohio Secretary of State registration, and specified employee thresholds in Ohio or border states (Kentucky, Michigan, New York, Pennsylvania, or Indiana). Additionally, bidders must certify whether they hold E.D.G.E business designation and confirm status as a "business entity" as defined by Ohio Revised Code Section 145.037. MBE suppliers must be certified for specific UNSPSC codes: 31160000, 48100000, 48101600, or 48101608.
