About this file

This document is a Certification Statements form for state and local contract opportunities in Ohio, primarily focused on bidder preferences and business designations. The form requires bidders to provide detailed information about their business origins, locations, employee base, and eligibility for various state preferences, including Domestic Source (Buy American), Ohio Preference (Buy Ohio), Veterans Preference (Buy Veteran), and E.D.G.E. (Encouraging Diversity, Growth, and Equity) business designations.

The certification form allows bidders to claim preferences by demonstrating their business characteristics, such as where products are manufactured, the percentage of components produced in the United States, economic presence in Ohio, number of employees in Ohio or border states, veteran-friendly status, and E.D.G.E. certification. Bidders must carefully complete the form, as intentionally submitting false or misleading information can result in immediate disqualification and potential legal action, including potential debarment. The form provides specific instructions for claiming preferences across multiple categories and requires documentation of business registration, tax compliance, and geographic economic contributions.
