About this file
This document is a Certification Statements form for state procurement in Ohio, specifically designed for bidders seeking preferences for Domestic Source End Products, Ohio preference, and/or Veteran Friendly Business Enterprise (VBE) status. The form requires detailed declarations about the origin of products/services, economic presence within Ohio, and veteran business certification. Bidders must accurately complete sections related to domestic manufacturing (Buy American), Ohio-based economic activity (Buy Ohio), and veteran enterprise status to potentially qualify for preferential treatment in the state procurement process.
The form outlines multiple certification paths, including demonstrating that products are manufactured in the United States, have significant components sourced domestically, or are raised/produced in Ohio. Additional considerations include having a substantial economic presence in Ohio (such as paying state taxes, being registered with the Ohio Secretary of State, having employees based in Ohio or border states), and being certified as a Veteran Friendly Business Enterprise or E.D.G.E. (Economic Development/Generating Employment) business. The document warns that intentionally submitting false or misleading information could result in immediate disqualification and potential legal action, including potential debarment from state contracts.
