Additional Program Requirements – (If Applicable) Prior to writing the 2016 funding application, applicants should carefully review the required recipient objectives in the Funding Opportunity Announcement (CDC-RFA-IP13-1301 amended). The application and budget request should reflect the applicant’s ability to implement required objectives to achieve the goals described in the “Purpose” section of the announcement. To that end, this Program Guidance links the five-year performance goals and the Program Requirements in the “Program Implementation” section of the Funding Announcement with required activities referenced in the Immunization Program Operations Manual (IPOM).
PPHF funding will be focused on the following programmatic areas:
· Program stewardship and accountability
· Assess program performance for program improvement
· Assuring access to vaccines
· Assure that the immunization information technology infrastructure supports program goals and objectives
· Improving and maintaining preparedness
Immunization Program Operations Manual (IPOM) Applicants should read the foreword to the IPOM. The Foreword provides an explanation of the 2016 IPOM layout as well as key information that will help in developing work plans and activities. The IPOM Unit A Program stewardship and accountability, Unit B Assess program performance for program improvement, Unit C Assuring access to vaccines, Unit D Assure that the immunization information technology infrastructure supports program goals and objectives and Unit E Improve and maintain preparedness each convey the required and suggested activities and performance measure to address the performance goals of this funding.
Performance Measure Reporting CDC programs must require awardees to submit performance measures annually and may require reporting more frequently. Performance measure reporting must be limited to data collection. When funding is awarded initially, CDC programs must specify required reporting frequency, data fields, and format.
Awardee work plan objectives, activities and performance measures for the next funding year should be documented in the eGrATIS application module and submitted as a PDF file attachment in Grants.gov.
PPHF Grantees Semi-annual Reporting CDC Office of Appropriations (OA) will distribute the pre-populated reporting templates to PPHF Grantees through the OA mailbox (PPHF@cdc.gov). All recipients will receive their pre-populated template prior to the start of the reporting period with additional guidance. CDC will validate grantee email addresses through OGS and PPHF programs. The prime grantee will receive the report and will be responsible for populating the sub-award information.
