SECTION B – SERVICES AND PRICES

B.1 GENERAL

The contractor shall provide the services specified in Section C, Performance Work Statement, of this contract for a one-year base period and four one-year option periods at the rates specified in each Contract Line Item Number (CLIN). These prices shall be in effect through the base period of the contract and through all option periods, if exercised.

The contractor shall underwrite and administer the Office of the Comptroller of the Currency’s (OCC’s) 24-Hour Personal Accident Insurance (24-Hour PAI) Program for OCC employees and their eligible family members and Business Travel Accident (BTA) Insurance Program for OCC and the Office of Financial Research (OFR) employees.

For the 24-Hour Personal Accident Insurance Program, the OCC employees pay 100 percent of the premium via payroll deductions on a bi-weekly basis. The OCC will remit 24 Hour premium payments electronically via its payroll vendor, the National Finance Center (NFC) on a biweekly basis. For the Business Travel Accident Insurance Program, the OCC pays 100 percent of the premium for its employees. The OCC will pay the Contractor the full yearly premium in a lump sum payment in January of each year after being invoiced by the Contractor.

B.2 CONTRACT LINE ITEM NUMBER (CLIN) DESCRIPTION AND

ARRANGEMENT

This contract will be a firm fixed price contract. The number of eligible employees enrolled in the 24-Hour Personal Accident and Business Travel Accident insurance plans is estimated and used for the purposes of establishing a funding amount for each performance period. The OCC employee population may fluctuate within each performance period resulting in an increase or decrease throughout the year (i.e., there may be a variation in quantity). The OCC will remit an annual premium payment for Business Travel Accident insurance based on the number of active employees at the performance period.

B.2.1 24-HOUR PERSONAL ACCIDENT AND BUSINESS TRAVEL ACCIDENT

SECTION C – PERFORMANCE WORK STATEMENT (PWS)

C.1. INTRODUCTION

The Office of the Comptroller of the Currency (OCC), headquartered at 400 7th Street, SW, Washington, DC 20219, is a non-appropriated federal financial regulatory bureau of the Department of the Treasury. The OCC charters, regulates, and supervises national banks, including supervision of federal branches and agencies of foreign banks. The OCC employs approximately 4,000 and there are approximately 1,000 employees at its headquarters location in Washington, DC. There are other employees in primary locations in Chicago, Dallas, Denver, Houston and New York. There are also employees who work in approximately 70 cities nationwide, including less than 10 employees in London.

The OCC is unique from most other federal government agencies because it has independent authority to offer its employees compensation and benefit programs that are not offered by other agencies. The OCC sponsored benefit programs include, but are not limited to, dental, vision, group life insurance, accidental, death & dismemberment (AD&D) insurance, flexible spending accounts, short and long-term disability and 401(k) programs.

OCC, as a governmental plan, is specifically exempted from Title I of ERISA.

C.2. SCOPE OF WORK

C.2.1. General

The OCC follows the procurement rules of the Federal Acquisition Regulations (FAR) and due to the upcoming expiration of the current contract with the Federal Insurance Company, referred to as Chubb on June 30, 2015, the OCC is required to undergo a competitive procurement process to award a new contract commencing July 1, 2015 for AD&D insurance for employees.

OCC is seeking an experienced and qualified Contractor to underwrite AD&D insurance for its 24-Hour Personal Accident (24-HR PAI) and Business Travel Accident (BTA) insurance programs. Also, under this contract, the Contractor is expected to insure approximately 250 employees who work for the Office of Financial Research (OFR). OFR was established as a result of Dodd-Frank as a small office within the Department of the Treasury and was given the same independent authority as the OCC to offer its employees other benefits that are not normally offered by the federal government. OFR determined that it would be best served by being a participating group under the OCC’s AD&D contract. In 2011, OFR chose to become a participating group only in the OCC’s BTA program (not the 24-HR PAI).

OCC will not utilize the services of an outside broker or consultant to assist with this procurement process. The OCC is seeking to contract with a single Contractor for services required under this contract.

In support of OCC’s efforts to educate its employees on the benefits of AD&D insurance coverage and wellness, the Contractor is expected to do the following:

Attend an onsite, one-day, benefits fair that is generally held in November of every year at OCC’s headquarters location at 400 7th Street, SW, Washington, DC. The Contractor shall supply program materials and giveaways for approximately 400 employees and be prepared to answer employee questions concerning the 24-HR PAI and BTA programs.

Provide in electronic form all documents and program brochures in support of the administration of the 24-HR PAI and BTA programs that can be posted on OCC’s intranet site.

Support OCC’s wellness initiatives by arranging for the attendance of a health or safety vendor at an OCC sponsored wellness event to be held annually starting in 2016 at OCC’s headquarters location. The Contractor shall be notified at least 120 days prior to the scheduling of the event.

The Contractor is expected to provide services in accordance with this Performance Work Statement (PWS) regardless of the actual amount of services or fluctuation in services. There is no guaranteed minimum amount of services to be covered. Any information in this PWS regarding estimated or historical services is intended to aid the Contractor in understanding the requirements associated with the services and in submitting an appropriate bid/proposal.

C.2.2. Eligibility

Employees who are hired on a full-time, part-time, or temporary appointment that is expected to last for more than one year can be participants. Employees can elect to provide coverage for their eligible dependents under the 24-HR PAI program. Eligible dependents are defined as follows:

• A lawful spouse or domestic partner (same or opposite sex);

• Children (defined as a natural, step, foster, legally adopted or child of the domestic partner) up to age 22 or up to age 25 if the child is a full-time student; and

• A child age 22 or older who is incapable of self-supported due to a physical or mental disability that existed before the child reached age 22.

For employee census data as of February 1, 2015, refer to Attachment 1 for the OCC population and Attachment 2 for the OFR population. The headcount for OCC and OFR is not expected to increase above 4,000 and 250 respectively over the next 10 years. The Contractor shall insure all employees, regardless of their work location (both U.S. and international work locations).

C.2.3. Enrollment Opportunities and Effective Dates

Employees must submit a completed enrollment form in order to participate in the 24-HR PAI program. An employee’s participation in the BTA is automatic (no enrollment form required).

Below is a chart that identifies instances when an employee’s enrollment starts under the 24- HR PAI and BTA programs, as well as the effective dates of those enrollments.

Enrollment Opportunity Description Effective Date New Hire or Change in Employment

Employee hired by the OCC or OFR on an appointment type that makes them eligible to enroll to participate in the 24-HR PAI and BTA programs.

Election of 24-HR PAI coverage will become effective on the first day of the pay period following receipt of the completed form by OCC Human Resources providing the form is received within 31 days of the effective date of the enrollment opportunity.

BTA coverage is effective on the date of the enrollment opportunity.

Life Event Only applicable to an employee who is currently enrolled in the 24-HR PAI program under the Employee Only option can switch to the Employee & Family option only at the same benefit amount due to a life event of marriage, establishment of a domestic partner relationship or gaining a new dependent child.

Election will become effective on the first day of the pay period following receipt of the completed form by OCC Human Resources providing the form is received within 31 days of the effective date of the enrollment opportunity.

Open Season The annual 4-week period during November and December that is announced by the OCC that allows employees to enroll in or make changes to their current 24-HR PAI election.

January 1 of the year following the end of the enrollment opportunity.

Voluntary Election to Decrease 24-HR PAI Coverage

A reduction in the benefit amount or a change from Employee & Family to Employee Only that does not increase the benefit amount.

Election will become effective on the first day of the pay period following receipt of the completed form by OCC Human Resources.

The Contractor shall agree to the enrollment opportunities and effective dates stated in the chart above.

OCC will retain all copies of the 24-HR PAI enrollment forms and will provide a copy of the actual enrollment form in the event of an AD&D claim for benefits. The

Contractor shall accept, as proof of enrollment, the most recent enrollment form on file by the OCC, or in lieu of an actual enrollment form, a copy of payroll records that confirms payroll deductions for the benefit election. OCC will not require employee who are currently enrolled in the 24-HR PAI program to complete a new enrollment due to the executive of this contract.

C.2.4. Termination of Employment When an employee’s employment ends, their coverage terminates on the date of they are last employed by OCC or OFR. The Contractor shall make available the opportunity for separated employees the option to continue or convert their coverage under the 24-HR PAI program to an individual policy within 31 days from the date they separated from OCC or OFR. The Contractor shall provide OCC with a coverage continuation or coverage conversion document that provides an applicant with all the information that need in order to make an informed decision.

C.2.5. Survivor Benefits

None applicable.

C.2.6. Funding

OCC pays 100 percent of the premium for the BTA insurance and will make a lump sum payment annually after receipt of an invoice from the Contractor. Employees pay 100 percent of the premium for the 24-HR PAI coverage. The current biweekly rates for the predetermined benefit amounts are as follows:

Employee’s Benefit Amount* Cost for Employee Only Cost for Employee &

Family $ 50,000 $0.27 $0.48 $ 75,000 $0.41 $0.71 $100,000 $0.54 $0.95 $125,000 $0.68 $1.19 $150,000 $0.81 $1.43 $175,000 $0.95 $1.66 $200,000 $1.08 $1.90 $225,000 $1.22 $2.14 $250,000 $1.35 $2.38 $300,000 $1.62 $2.85 $350,000 $1.89 $3.33 $400,000 $2.16 $3.80 $450,000 $2.43 $4.28 $500,000 $2.70 $4.75

* OCC has no desire to make any deviations from the current benefit amounts above.

OCC will remit to the Contractor automatically (without invoicing from the Contractor) premiums that have been deducted from employees’ pay on a biweekly basis. Upon request after contract award, the Contractor shall provide the OCC with required banking information so that the OCC, through its payroll vendor, the National Finance Center (NFC), can remit biweekly premium deductions electronically directly to the Contractor’s bank account. In January, of each year, the OCC will send the Contractor census data on employee participation in the 24-HR PAI program. The Contractor shall accept the annual census data from the OCC (for both OCC and OFR employee elections) in lieu of actual enrollment forms to reduce the transmission of Personally Identifiable Information (PII).

C.2.7. Pricing

The Contractor is to submit its offer of pricing for BTA as an annual premium amount for the OCC and OFR groups separately. The Contractor is to submit its offer of pricing for 24-HR PAI as a biweekly amount (Contractor needs to convert its monthly rate to a biweekly rate) based on the coverage increments as reflected in C.2.7. To assist the Contractor in providing its offer of pricing for BTA and 24-HR PAI insurance, refer to Attachment 3 for Business Travel for OCC Employees from September 2011 thru December 2014 (no data available for OFR at this time) and Attachment 4 for Premium Rate History from January 2010 thru June 2015 for both the BTA and 24-HR PAI insurance programs.

C.2.8. Plan Design

Refer to Attachments 5 and 6 for a copy the current AD&D insurance policies for BTA and 24- HR PAI coverage, respectively. There are a few areas within the existing policies that require modification to the language as followings:

1) under the age of twenty-two (22);

2) under the age of twenty-five (25) if enrolled as a full-time student at an Institution of

3) has a close and committed personal relationship with the Primary Insured Person and is each other’s sole domestic partner not married to or partnered with any other spouse, spouse equivalent or domestic partner;

4) is in a financially interdependent relationship with the Primary Insured Person in which both parties mutually agree to be responsible for each other’s common welfare and share financial obligations; and

5) has exclusively lived with the Primary Insured Person for at least six months prior to the effective date of enrollment.

The incapacity must have occurred prior to the child attaining age 22.

The Contractor should note that for the BTA insurance, OCC does not own, lease or operate any aircrafts. Also, for this same coverage, OCC approves travel for approximately 200 prospective employees and approximately 30 authorized guests per year. OCC authorizes business travel for less than 50 employees for company approved relocation trips.

The Contractor shall insure the following class types:

C.3. PERSONNEL REQUIREMENTS

The Contractor shall provide a single point of contact that will serve as a liaison to the OCC to address day-to-day matters. The person is expected to ensure that all issues are addressed and responded to in a timely manner. If an issue (other than a claim issue) cannot be resolved by the next business day, then the person must provide the OCC with a reasonable date that the issue will be resolved. The individual must have a minimum of five (5) years of account management experience in the life insurance industry.

C.4. CLAIMS EXPERIENCE

The Contractor is expected to process all claims accurately and timely. There have been no claims filed against the Business Travel Accident Insurance program in the past 15 years.

Male Child Death 16 May 4, 2011 $500,000.00 $50,000.00 $75.00* *Other expenses amount was due to the insurance company’s need of an interpreter to translate the accident report documents and death certificate since the accident occurred in Costa Rica.

C.5. PERIOD OF PERFORMANCE

OCC’s contract with the current Contractor commenced on January 1, 2010 and is scheduled to expire on June 30, 2015. OCC is seeking to award a single contract to one Contractor that covers the services for its BTA and 24-HR PAI programs. The contract award period under this solicitation will include a 6-month Base Period, which will commence on July 1, 2015, and four (4) one-year Option Year Periods for a total period of performance of 4½ years.

C.6. PLACE OF PERFORMANCE

INVESTIGATIONS (JAN 2014)

All contractor personnel assigned to this contract who would have access to OCC facilities or systems, shall submit to a background investigation before beginning work under this contract

The number and type of investigations will depend on factors including, but not limited to, the length of time the individual will work on a contract, the individual’s role, the sensitivity of information the individual will have access to, and the results of past investigations on that individual. Investigations typically conducted by the OCC include an FBI criminal check and/or a suitability investigation.

Contractor personnel must submit information for background investigations electronically through the e-QIP system (http://www.opm.gov/e-QIP/). The Office of Security (OS) will notify contractor personnel when e-QIP is ready to receive their information. After submitting their information, contractor personnel must print required security forms from e-QIP. Printed forms must be signed by the employee and provided to OS.

For background investigation purposes, contractor personnel are required to provide a set of fingerprints. Contractor personnel must appear in-person to provide fingerprints. See OCC Clause 1052.245-8007 for requirements regarding in-person appearances for security purposes.

The results of all background investigations will be reviewed and adjudicated by OS.

Based on investigation results, the OCC has the unilateral right to bar an individual from working under a contract or otherwise restrict the individual’s role under a contract.

The costs for all investigations required under this contract will be borne by the OCC.
