U.S. BUREAU OF MEDICINE AND SURGERY
Capability, Development and Integration Office (CD&I) RFP for Hard-Wall Containerized Medical System
1.0 Overall Objective: The overarching objective of the Hard-Wall Containerized Medical System is to provide a theater medical capability that delivers a responsive, rapidly deployable, modular and scalable expeditionary medical capability that is clinically effective, incorporating material alternatives into completely integrated hard-wall shelters. This Request for Proposal (RFP) is to engage industry regarding the use of medical and non-medical hard-wall shelters capable of being assembled and employed to provide primary surgery, ancillary capability and humanitarian assistance. This capability shall be self-sustainable if required to function in hostile and/or austere environments.
1.1 Expectation: The contractors prepare a written proposal for review by the Government based on criteria and performance objectives stated in sections 3 and 4.
2.0 Background: The US Fleet Forces approved a change to the way operational forces will operate, introducing the concept of adaptive force packages (AFP) that are manned, trained and equipped to address contingency operations and major combat across the full range of military operations (ROMO). The Capabilities-Based Medical Treatment Facility (CB-MTF) shall provide a rapidly deployable, independently operable, scalable, and modular Medical Force Package (MFP), both Standard (MSFP) and Adaptive (MAFP) to support the Joint Force Commander (JFC) in the maritime domain. Five medical standard force packages comprise CB-MTF: Adaptive Trauma Care System (ACTS), Role 2 Light Maneuver (R2LM), Role 2 Enhanced (Role 2E), Role 3 CB-MTF and Temporary Holding MFSP.
This RFP is focused specifically for the Role 2E portion of the CB-MTF.
2.1 Vision: Navy Medicine is the pinnacle of excellence - answering the call across any dynamic - from kinetic operations to global engagement. Our healthcare is patient-centered and provides best value, preserves health, and maintains readiness. Agility, professionalism, an ethos of care, and the ability to deploy to any environment or sea state are our hallmarks.
2.2 CB-MTF Core Attributes: CB-MTF core attributes center on an expeditionary medical proficiency that provides a flexible, rapidly deployable and highly responsive capability. The four attributes elaborated on below are those that support the Role 2E CB-MTF mission.
2.2.1 Possess Scalable Capability and Capacity: Enables transition from light to heavy casualty throughput, with “snap together” modules to increase capability and capacity; (i.e. being able to transition from smaller to larger configurations to facilitate patient throughput)
2.2.2 Possess modular capability: Ability for Medical components to operate independently and “snap together” for increased capacity. While related to scalability, it refers to the capability to operate independently.
2.2.3 Possess Clinical Effectiveness: Conform to clinical practice guidelines for surgical and other health services support. To include stabilization, triage, treatment, evacuation and conformance to clinical practice.
2.2.4 Possess Deployable Capability: Reflects a small footprint, low weight, small “cube” easy to store easy to transport and compatible with rapid intermodal handling platforms (air, sea, truck and rail).
3.0 Criteria: The Role 2 Enhanced hard-wall containerized medical system shall conform to following criteria.
3.1 The hard-wall shelter containerized capability shall provide basic health care, built around primary surgery, intensive care unit, ward beds, provide berthing/hotel services for 79 medical professionals and be self-sustainable with the exception of fuel and water for 30 days.
3.1.2 The hard-wall shelter shall meet the International Standard Organization (ISO) standards of the 20 foot standard length container intermodal freight transport (rail, ship, truck, air).
3.1.3 The hard-wall Shelter shall meet the International Convention for Safe Containers; certified stackable 9 high.
3.1.4 The hard-wall shelter (packed out) maximum weight shall not exceed 20K pounds in its closed position ready for transportation.
3.1.5 The hard-wall shelter shall have the capacity to store medical equipment, medical / surgical, pharmaceutical consumables and administrative supplies. See appendix A
3.1.6 The hard-wall shelter container shall be capable of being cooled or heated independently or as part of the integrated system.
3.1.7 The hard-wall shelter to the greatest extent possible shall be outfitted with prefabricated lighting, 60 Hz/120V outlets, and shelving and in the case of berthing prefabricated containers with beds.
3.1.8 The hard-wall containerized operating room shall have high-efficiency particulate air (HEPA) filtration, light emitting diode (LED) operating room lighting as part of its prefabricated design and meet the applicable national standards for safe Operating Room practice.
3.1.9 All medical equipment that receives, transmits, creates, edits, and stores data regarding patient information shall conform to the Risk Management Framework (RMF) for DoD Information Technology (IT).
3.1.10 The hard-wall shelter containerized system shall have a power distribution system to support the entire complex to include generators, electrical cabling, plugs, adapters, power panels, central digital monitoring and the necessary redundant equipment for independent operations or scaled to increase capacity.
3.1.11 The Hard-wall Container shall have interlocking capability to connect or disconnect allowing for multiply configurations within the design.
3.1.12 The Hard-wall berthing container design shall be able to house/berth 79 medical professionals.
3.1.13 The Hard-wall containerize system shall provide laundry (capable of washing soiled medical linen (CDC: min 160 degrees Fahrenheit-25 mins), bathroom and shower service for a minimum of 10 patients and 79 medical professionals.
3.1.14 The Hard-wall containerized system shall provide food service capability: store, refrigerate, cook, distribute, feed, wash and disinfect for a minimum of 10 patients and 79 medical professionals.
3.1.15 The Hard-wall containerized system shall be capable to receive, store and disburse fuel and water for 3 days of distribution throughout the system.
3.1.16 The Hard-wall containerized system shall have the means to collect and/or dispose of hazardous/medical waste, grey/black water.
3.1.16 The Hard-wall containerized system shall have in its design additional spaces for administration, medical logistics and Command and Control.
3.1.17 The Hard-wall containerized system shall have medical oxygen generation capability.
4.0 Performance Objectives: note that the performance objectives below are only put in place for offerors to ascertain the approximate or estimate level of effort for this contract. This is not to be construed as neither mandatory nor necessarily the best technical approach. It is only in place as a reference to allow offerors to understand the general scope of this effort from the government's perspective.
4.1 The Hard-wall containerized medical system design shall support a 2 Operating Room table, 4 Intensive Care bed and 6 ward bed capability sustainable for 30days before resupply (except for fuel and water: see 3.1.15).
4.1.2 The Hard-wall containerized medical system shall provide the necessary medical equipment and supplies to be able to stabilize post-surgical cases for evacuation to the next higher level of medical care i.e. Medical treatment facility. See appendix A
4.1.3 The integrated internal design of the hard-wall shelter containerized system shall allow for unencumbered reception of patients, casualties, free-flow movement for staff and patient transport via standard military patient carrying systems such as field litter, gurney, wheelchair or stretcher throughout the structure.
4.1.4 Shall have the capability to receive, transmit, create, edit, and store data regarding patient location patient status telemedicine diagnosis and therapy, medical regulation and medical logistics.
4.1.5 Shall provide sufficient power to operate the entire Hard-wall containerized medical system, it shall have sufficient back up power in the event of main power interruption and shall have the means to either scale up or scale down the power distribution to support the modularity attribute: see 2.2.2.
