About this file
This is a Documented Quote issued by the Colorado Bureau of Investigation Forensic Services (CBIFS), a division of the Colorado Department of Public Safety (CDPS), soliciting customized sexual assault evidence collection kits for distribution to Colorado medical providers and law enforcement agencies. The solicitation requests bids for two kit types: victim kits (approximately 1,900–2,200 annually) and suspect kits (approximately 250–500 annually). Each victim kit contains 13 sample envelopes, cotton-tipped applicators, microscope slides, sterile implements, paper forms, evidence tape seals, consent and incident forms, instruction lists, and an outermost container (either paper envelope or box). Each suspect kit contains 10 sample envelopes, cotton-tipped applicators, sterile implements, paper forms, collection instruction lists, and an outermost container. Bids were due Monday, July 25, 2016, at 2:00 p.m. MT and were to be submitted via email to Tammy Lichvar at tammy.lichvar@state.co.us. The award period runs from the effective date of the purchase order through June 30, 2017, with an option to extend for four additional years through June 30, 2021, contingent upon fund appropriation and budget availability.
All pricing must be binding and valid for the length of the award, include FOB Destination shipping and handling, and shall not increase more than 3% per year unless modifications to kit contents requested by CBIFS exceed 3%, which would require justification and approval. Bidders must provide documentation of sterile and DNA-free material supply capabilities, with preference given to those with ISO 18385 or similar accreditation. Bidders must also submit three customer references for similar customized evidence kits and a quality control protocol. CBIFS reserves the right to review vendor performance annually before renewal and may request sample kits from unknown vendors. The successful bidder must deliver customized kits within 60 days of award and agree to fulfill orders on an as-needed basis directly to CBIFS customers without CBIFS maintaining stock inventory. Invoices are due 10 business days after receipt of order and should be sent to Sheri Murphy at the CBIFS office in Arvada, Colorado. A completed W-9 form must accompany the bid submission.
