Quote Due Date and Time: 3:00 P.M. EST, 18 August 2021

This is a Non-Personal Service requirement for Catholic Religious Education Coordinator Services. Contractor shall provide services for the 11th Wing Chapel in accordance with the attached statement of work. The Government has a current requirement for the Contractor to perform work 16-20 hours per week with weekly meetings with the Chaplain/Priest. The Contractor will serve as a trainer and organizer to communicate and make provisions for all religious Sacraments provided in the statement of work. The Contractor must maintain and follow all aspects of the Religious Educational program that are required by the Catholic Church.

The Contractor will be responsible for maintaining records and publishing yearly guidance and Religious Education calendar. The Contractor will provide assistance with budget plans for appropriated and Chapel Tithes and Offerings Funds (CTOF), as directed by the Catholic Chaplain/Priest.

All other specific details for this position, please see attached Statement of Work.

Period of Performance is estimated for a base period of 01 September 2021 to 30 August 2022, plus four one-year option periods.

Provisions 52.212-1, Instructions to Offerors:

Proposal submission. The Contractor shall submit the above Cost Sheet. The Contractor’s Quote shall include: date, quote number, quote expiration date, POC name and contact information, CAGE Code, DUNS Number, and a written statement confirming they can fully comply with the requirement as presented. Submission of a similar Cost Sheet document will be accepted in .word or .xls format only, providing all information as outlined on the Cost Sheet is included. If the Offeror has not completed the annual representations and certifications electronically via the SAM Portal at http://www.sam.gov/portal in the last 12 months, the Offeror shall complete paragraphs (c) through (u) of FAR provision 52.212-3 and include a completed copy with its quote. Submit quote via email to jacqueline.wilborne@us.af.mil and joseph.trevino.3@us.af.mil NLT 3:00 PM EST, 18 August 2021.

52.212-2, Evaluation:

The following factors shall be used to evaluate your quote: 1) Price. Price will be evaluated in accordance with FAR 13.106-3. The Lowest Price Technically Acceptable (LPTA) shall be determined fair and reasonable prior to award. The LPTA will be determined by adding all prices for all CLINs for technical evaluation purposes only. Options. The Government may determine that the offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the options.

52.212-3, Offeror Representations and Certifications:

The offeror verifies by submission of their quote that the representation and certifications posted at FAR 52.212-3 have been entered or updated electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov/portal in the last 12 months, and are current, accurate, complete, and applicable to this solicitation. Alternatively, if the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision and include a completed copy with its offer.
