About this file

This is a Past Performance Reference Information Worksheet template designed to collect detailed historical contract performance data from contractors submitting proposals to federal agencies. The document serves as Attachment 4 to solicitation 12444126Q0016 and requires contractors to provide comprehensive information about completed or ongoing contracts to demonstrate their capability and reliability.

The worksheet requires submission of 12 key data elements including: complete customer organization name and address; contract number and dates (contract execution, work start, and work completion); original contract price and final invoiced amount; technical and contracting points of contact with full communication details; work location(s); a detailed description of contract work including nature, scope, and any performance issues, conflicts, or litigation; and current contract status selected from nine predefined options (work continuing on/behind schedule, work completed with various pending actions, terminated for convenience or default, or other). Item 11 specifically requires contractors to explain how the referenced work is similar in nature and scope to the contemplated contract, describe any unusual circumstances or performance problems, address customer conflicts proactively, and document corrective actions taken. The instructions emphasize that contractors should avoid acronyms, provide complete addresses including both post office box and street address where applicable, and ensure contact information identifies individuals most familiar with work quality and contract administration. This worksheet is used by federal agencies during proposal evaluation to assess contractor past performance and relevant experience.
