About this file

This is a Corporate Certificate form required for lease verification when doing business with the government. The form must be completed by someone other than the individual who signed the lease document and serves to certify that the person who signed the lease document had proper authority to do so on behalf of the corporation named as Lessor.

The form requires documentation of the certifying official's position within the corporation, verification of the lease signatory's position, and confirmation that the lease document was properly authorized by the corporation's governing body. It includes a Privacy Act statement citing authority under 40 USCA Section 49(h)(1) and noting that while disclosure is voluntary, failure to provide the information will prevent the lease from being finalized.
