About this file

This document is a Single Source Justification for Simplified Acquisition Procedures (SAP) under the Simplified Acquisition Threshold (SAT) for the Department of Veterans Affairs (VA) VISN 17, Audie L. Murphy VA Hospital in San Antonio, TX.

The justification is for the purchase of a FujiFilm Sonosite ST Ultrasound System, a medical imaging point-of-care ultrasound system, required to comply with the VA's policy on using a closed catheterization technique. FujiFilm is the sole provider of this proprietary system, which is necessary due to the closed catheter design and advanced imaging features. Market research identified that at least two Service-Disabled Veteran-Owned Small Businesses (SDVOSBs) have previously procured this item for the VA. The anticipated delivery date for the supplies is approximately June 15, 2024. The solicitation is set-aside for SDVOSBs, with a NAICS code of 334510 and a small business size standard of 1,250 employees. The award will be made on a competitive basis evaluating technical, past performance, and price factors.

VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

Last Updated: 02/01/22 Page 1 of 2

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C257-24-AP-2873

JFOC ID: FY24 - NCO 17 - 1653

1. Contracting Activity: Department of Veterans Affairs, VISN 17, Audie L. Murphy VA Hospital, 7400

Merton Minter, San Antonio, TX 78229, NCO 17, Patient Care Team.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

This contract is for the purchase of Sonosite ST Ultrasound System (Brand Name Only).

What is Sonosite ST Ultrasound System (brief description).

- Medical imaging, point of care ultrasound system. Using sound waves to produce images of structures with in body. Imagaes provide information from diagnosing, directing treatment, and assisting with much more.

This product is required to ensure compliance with Policy MCP 002-23-35, with utilizing closed catherizations with, no more than two attempts per clinician with the the guidance of Sonosite ST Ultrasound.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Fujifilm is the sole provider of Sonosite ST due to proprietary purposes. Other Ultrasound devices for Peripherally placed Intra Venous devices use proprietary disposable intra catheterization systems (Medical Supplies) that are not authorized in our infection prevention policy. Our hospital only uses closed disposable intra catheterization systems vs. the Open systems used by the non Sonosite ultrasound systems. Open systems require frequent replacement and have a higher risk of infection.

Catheterisation is achieved by removing the core metal needle to check for arterial blood backflow, advancing the catheter over the needle, and then connecting the catheter to a pressure monitoring system for final confirmation of open intra-arterial placement.

Blood loss during insertion, needle-stick injury, and infection are potential risks with an open catherization. A closed intravascular catheter system was designed to reduce the risks of blood exposure and infections during peripheral venous catheterisation. This technique may have similar advantages during arterial catheterisation. However, the core needle of this closed system cannot be removed to check for blood backflow and the loss of resistance sensation while advancing the catheter over the needle into the arterial lumen cannot be felt because of the high resistance between the catheter and the needle of this closed system.

A radial artery catheterisation technique, with this closed intravascular catheter system combined with monitoring and the ultrasound-guided dynamic needle tip positioning technique meet VHASTX

Policy MCP 002-23-35. Steep Needle Profiling, proprietary imaging algorithm that improves needle reflectance and contributes to needle visibility during a steep angle approach for deep targets.

Sonosite machines prioritize features and applications relevant to immediate decision-making at the point of contact.

4. Description of market research conducted and results or statement why it was not conducted:

After querying the government database FPDS, using the keyword “FujiFilm Sonosite Ultrasound” it was revealed that at least 2 SDVOSBs have previously procured this item. DGA Medical is a SDVOSB and has procured the items for the Dept. of Veterans Affairs in November of 2023 (36C26224P0256). Fidelis Healthcare procured the same items in February of 2024 (36C24824P0809). A search of GSA (Advantage and eLibrary) was conducted using keyword search “FujiFilm Sonosite” and yielded negative results. A search of VA NAC yielded negative results upon search of keyword “FujiFilm Sonosite”. (Please reference Market Research attachments titled “P02 – MARKET RESEARCH” in the purchase request briefcase). An RFI (36C25724Q0561) was placed on FCO April 22, 2024. A total of 4 SDVOSB contractors responded and provided authorized distributor letters confirming they could procure the items requested.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b)(i). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
