About this file
This document contains a sole source justification for a commercial item acquisition. The justification is for the Department of the Air Force to lease a CT trailer with a pre-installed Toshiba Cannon Aquilion 64/128 VeloCT system on a brand name only basis. The rental period is from May 20, 2022 through September 19, 2022. A sole source award is being made because the radiology technicians are only trained on Toshiba equipment and using a different brand would require additional training. Market research found one potential vendor but they could not provide assurances of meeting the delivery timeline. The justification cites needs related to patient safety and care.

I. Contracting Activity.
35th Contracting Squadron, Acquisitions Flight, Misawa AB, Japan
II. Nature and/or description of the action being processed.
Requiring Activity: 35 MDG Radiology Section
This acquisition is being conducted under the authority of Federal Acquisition Regulation (FAR) 13.501(a) allowing the Contracting Officer (CO) to solicit from one source (Including Brand-Name). The justification for the required Brand- Name product is based on compatibility with the existing Computerized Tomography (CT) scanner which will undergo renovation. To lease a CT trailer with a Toshiba Cannon Aquilion 64/128 VeloCT system installed is immediately required as a temporary CT scanner while the current equipment undergoes renovation.
The proposed contract will be a Firm Fixed Price Purchase Order for commercial supply with the basic period and one
(1) one-month option.
Impact If Not Leased: First, the CT scanner is a critical piece of machinery to the daily operations of the facility. The Urgent Care Services (UCS) flight uses this equipment during regular business hours and after hours. The current guidance from the American Heart Association recommends that if a patient is suspected of having a stroke, the CT scan needs to be completed within 20 minutes of presentation to the UCS. Treatment for a stroke cannot begin without this scan. In the last 2 weeks, there have been 3 patients seen within the UCS with new seizure activity that also requires an immediate CT Scan to rule out bleeding or tumors, etc. In the last 2 years there have been several "Code Strokes". Second, trauma patients require timely scans as well to diagnose internal bleeding without doing exploratory surgery. There have been 2 significant (life threatening) traumas on Misawa AB in the last 15 months. Even a seemingly small trauma that leaves a patient stable temporarily with a slow bleed can decompensate quickly. If that bleed is found with a CT scan, it can be repaired before the patient deteriorates or codes. The CT scan is also critical for any emergent abdominal illness that could require surgery. Without the equipment, patients could either be delayed in definitive surgical care or could get an unnecessary surgery because of their clinical presentation that made surgery appear necessary. There are the highlights of patient safety issues without CT scanner capability. Finally, referral to the local Japanese facilities is not a guarantee especially in the COVID environment. We are required to provide a negative COVID test for referrals to the local community hospital. This will further delay diagnosis and risk poor patient outcomes, disability or death.
III. Description of supplies/services required to meet agency needs.
The Contractor shall provide one (1) CT trailer with a Toshiba Cannon Aquilion 64/128 VeloCT system installed in accordance with the Statement of Work (SOW). A generator, Equipment setup and verification must be provided to the CT trailer and completed by the Contractor no later than 19 May 2022. During the initial implementation, an English-speaking technical representative must be available by telephone 24 hours 7 days a week. All equipment displays and supporting documents must be in the English language. The period of rental will be 20 May 2022 through 19 Sep 2022 for 4 months.
IV. Statutory authority permitting other than full and open competition.
This action is under the procedures of FAR 13.5 under the authority of 41 U.S.C. 1901, Simplified Acquisition Procedures. In accordance with FAR13.501, this J&A will be posted to the Government Point of Entry, SAM.gov with the solicitation.
V. Demonstration that the contractor's unique qualifications or nature of the acquisition requires the use of the authority cited above (applicability of authority).
CT trailer with a Toshiba Cannon Aquilion 64/128 VeloCT system is the only brand of CT scanner that the radiology technicians in the requiring activity are trained for and getting different brand will require additional trainings to be able to use it. This brand name acquisition is integral to generate significant estimated cost savings in training

Three (3) vendors provided a response to the announcement; however, they couldn't provide detailed information to show their capability of delivering the required equipment within the required time frame as required in the announcement.
