About this file
This is a Responsive Proposal Checklist serving as Attachment E to the RFP for leasing Building 125 at Boston National Historical Park's Charlestown Navy Yard. The checklist outlines all required submission components for offerors interested in leasing the historic 10,188-square-foot building located at 125 Baxter Road in Charlestown, Massachusetts. Offerors must submit a signed transmittal letter from all managing members, owners, partners, or principals, along with a clearly organized proposal addressing six evaluation criteria: Proposed Use and Operations Consistent with National Park Service Values, Improvements/Maintenance/Preservation of Historic Quality, Financial Capability of the Offeror, Demonstrated Experience Operating Similar Properties, Environmental Enhancement, and Financial and Other Terms Offered.
Proposals must include completed GSA Form 10-352 (Business History Information) and appropriate organization forms (10-353 for individuals/sole proprietorships or 10-354 for corporations/LLCs/partnerships/joint ventures), with separate forms required for all parent entities. Financial documentation requirements vary based on business structure: existing entities must provide audited or reviewed income statements for two recent fiscal years, current balance sheets with month-end statements, substantiating earnings and bank statements, and a business credit report with score from the last 30 days; new entities must provide personal financial statements and credit reports for all principals; sole proprietors must provide personal financial statements, substantiating documents, and personal credit reports. All credit reports must include credit scores, as free annual credit reports typically do not contain this required information.
