About this file

The document is a Worker Protection and Investment Certification Form related to a state and local contract opportunity in Pennsylvania. The Commonwealth of Pennsylvania is seeking fiber modules and panels needed at SCI Somerset, and the contract is being administered through the Department of General Services (DGS). The contract is an ELECTRONIC bid only, and registered vendors can find the bid on the PA Supplier Portal website. All bidders are responsible for monitoring the DGS eMarketplace website for any changes to the Invitation for Bid (IFB). To submit a bid response, interested vendors must be registered with the PA Supplier Portal.

The certification form requires contractors and grantees to certify compliance with various Pennsylvania state labor and workplace safety laws, including unemployment compensation, workers' compensation, and prevailing wage requirements. Providing false certifications can result in contract termination and three-year ineligibility to bid on Commonwealth contracts. Pricing terms, set-asides, and funding sources are not specified in the provided documents.
