About this file
This is a Worker Protection and Investment Certification Form prepared by the Commonwealth of Pennsylvania in accordance with Executive Order 2021-06, applicable to the State Correctional Institution at Phoenix's solicitation for two Muffin Monster Grinders. Bidders must complete and return this certification form with their bid responses submitted electronically through the PA Supplier Portal, with all change notices incorporated as binding modifications to the solicitation.
The certification carries significant legal consequences for non-compliance, including contract termination and three-year debarment from bidding on Commonwealth contracts under 62 Pa.C.S. 531, with false certifications subject to penalties under 18 Pa.C.S. 4904 for unsworn falsification to authorities. Certifying officials must be duly authorized representatives of the bidding contractor or grantee and bear personal responsibility for the accuracy of the certification, with an affirmative obligation to immediately report any subsequent changes in compliance status to the purchasing agency.
