About this file

This is a Bidder Declaration form (GSPD-05-105) from the California Department of General Services, Procurement Division. The form is designed for prime bidders to declare their certification status and subcontractor information when submitting bids for state contracts. The document requires bidders to identify their current California certifications (Microbusiness, Small Business, Nonprofit Veteran Service Agency, or Disabled Veteran Business Enterprise), declare whether subcontractors will be used, and provide details about the distinct elements of work to be performed.

The form includes specific requirements for DVBE-certified businesses, including disclosure of broker/agent status and equipment ownership requirements for rental equipment (51% ownership threshold). For subcontractors, the form requires detailed information including contact information, certification status, work to be performed, percentage of bid price, good standing status, and rental equipment ownership status if applicable. The document emphasizes the importance of commercially useful functions as defined in Military and Veterans Code Section 999 for DVBEs and Government Code Section 14837(d)(4)(A) for small/microbusinesses.
