About this file

This document is an ITQ/RFLI/RFI Submittal Questionnaire for the City of Miami Beach, Florida, designed for potential bidders to provide comprehensive information when responding to a solicitation. The multi-section form requires bidders to complete detailed information about their business, including years in operation, number of employees, contact details, and business history. The questionnaire covers various compliance certifications such as acknowledging addenda, conflict of interest disclosures, anti-lobbying requirements, veteran and disadvantaged business enterprise status, and commitments to ethical business practices.

The document mandates bidders to provide at least three professional references for similar work, disclose any potential conflicts with city officials, certify compliance with local ordinances regarding human trafficking, fair chance hiring, and contracting restrictions, and affirm adherence to the city's code of business ethics. Notable sections include requirements related to the city's moratorium on travel to and purchasing from Mississippi, veteran business enterprise preferences, and restrictions on contracting with entities that have provided compensated services to political candidates. The form serves as a comprehensive vetting tool to ensure potential contractors meet the City of Miami Beach's legal, ethical, and operational standards before being considered for contract award.
