About this file
This is a bid sheet for Request for Bids (RFB) 605 2400000543 issued by Martin County, Kentucky for demolition services. The project encompasses the demolition of Buck Creek Regular Baptist Church, a 2,916 square foot structure located at 12 Big Elk Road, Beauty, Kentucky 41203, along with its septic system. A critical component of the project involves constructing temporary access to the parcel, as the existing bridge at the drive entrance will only support passenger vehicles and cannot be removed. Vendors must bid all parcels to be considered for award and are required to include the cost of constructing necessary equipment access across the parcel within their bid price. Additionally, contractors must design and install a temporary stream crossing consisting of three 24-inch double-walled plastic pipes with an estimated 16-foot length across the Right Fork of Buck Creek to maintain water flow during demolition activities. Following project completion and approval, the temporary stream crossing must be removed, with all disturbed areas seeded and covered with straw according to provided guidelines.
Bidders must submit actual bid prices on this sheet and upload them into the e-procurement system, with separate line items for the amount due from the bidder and amount due from the state. The bid sheet specifies that failure to include an amount for each parcel will result in rejection of the bid. All costs associated with constructing access for equipment and the temporary stream crossing must be included in the bidder's lump sum total, as later claims for these costs may not be approved. The standard drawing for proper installation guidelines of the approved stream crossing has been included with the bid documentation.
