About this file
This file is Amendment No. 1 to the Bells Gap Project bid specification, issued by the Colorado Division of Reclamation, Mining & Safety on August 16, 2016, following a pre-bid meeting held on August 9, 2016. The project involves safeguarding hazardous mine openings through subsidence feature backfilling and revegetation work in Huerfano County. The amendment provides clarifications and revisions to feature descriptions for nineteen subsidence features (BG01 through BG19), with work tasks including topsoil stripping and replacement, installation of imported rockfill materials, fence removal and replacement, removal of branches and wood from subsidence features, and revegetation with hay or straw mulch. Key revisions address feature dimensions, excavation specifications, and work descriptions, with depths ranging from six feet to thirty-five feet and imported rockfill quantities ranging from ten to thirty cubic yards depending on the feature.
The revised bid schedule contains seven line items including mobilization/demobilization, development of a Safety, Health, and Environmental Action Plan (SHEAP), installation of mine closures, safeguard of additional hazardous openings, and three non-guaranteed contingency items for additional imported common fill (50 cubic yards), machine time (10 hours), and additional imported rockfill (10 cubic yards). All features are funded through Huerfano County OSM (Office of Surface Mining) funding. The amendment addresses fourteen contractor questions regarding temporary road construction, material specifications, waste pile reuse, landowner contact information, revegetation mulch requirements, gate guard provisions, SHEAP training requirements, backfill procedures, compaction testing, and MSHA/OSHA certification requirements. No compaction testing is required; backfill shall be compacted in one-foot lifts using multiple passes with heavy equipment per Section 2.5.1 of the General Bid Specifications, 2009.

CONTRACTOR QUESTIONS
1. Would it be possible to construct a temporary road from the waste pile to Features BG-07, BG-08, BG-09, BG-10, BG-11, BG-12, BG-18 and BG-19?
Page 3 of 5 Pages
A road between the waste pile and the features listed above can be constructed. Any roads constructed by the Contractor will need to be reclaimed.
2. The bid spec book calls for “hard, durable and well graded 12” material and sufficient finer particles to minimize the presence of voids when placed”. Do we need to order the material half 12” and half finer, say pit run?
Yes.
3. Could the mine waste piles by BG16 and BG17 be used as fill material at the north holes, assuming a deal could be negotiated with the owner?
Yes.
4. Do you have a phone number for the owner, I believe his name is Raymond Pfaffenhauser?
I have talked with Ray Pfaffenhauser and he said that he does not mind receiving calls.
His number is (719) 859-1770.
5. Are straw mulch and hand crimping required for the revegetation portion of this project?
General Bid Specifications, 2009, Section 20 Revegetation, 20.4.3 Mulches Mulch shall consist of hay or straw mulch, pest-free or weed-free to the extent possible.
Approximately 30 percent by weight of the mulch material shall be ten inches (10”) in length or longer. Rotted, caked or moldy material will not be accepted. Hay or straw mulch shall be applied uniformly at the rate of two (2) tons per acre. Hydromulch shall be applied at the rate of one and a half (1 ½) tons per acre.
General Bid Specifications, 2009, Section 20 Revegetation, 20.5.5 Mulching Hay or straw mulch shall be applied uniformly at the rate of two (2) tons per acre over seeded areas. Hydromulch shall be applied at the rate of one and a half (1 ½) tons per acre. Mulch shall be applied to the designated areas immediately after seeding and not later than 24 hours after seeding has been performed. Mulching shall not be done when the wind velocity exceeds fifteen (15) miles per hour.
General Bid Specifications, 2009, Section 20 Revegetation, 20.5.6 Stabilizing Mulch On areas larger than one (1) acre a mulch crimper shall be used to stabilize or anchor the mulch into the soil after hay or straw mulch has been spread. The crimper shall be equipped with scrapers to keep the blades clean. Spacing of crimper blades shall not exceed nine inches (9”). On areas less than one (1) acre crimping shall be achieved by hand with a suitable tool, such as a spade, on 18” centers.

6. Do we need a gate guard when hauling in material at the entrance to BG1 or can we leave it open and close at the end of the day?
We will plan on using a gate guard at Feature BG01. I will discuss the feasibility of keeping the livestock away from the project site during construction with the landowner.
7. Can you please make it known what training, if any, is required with SHEAP plan.
SHEAP Section B. PART 48 TRAINING REQUIREMENTS Each CONTRACTOR employee shall receive orientation training, provided by the CONTRACTOR, comprised of all aspects of the contract’s Safety, Health and Environmental Action Plan (SHEAP).
The contractor should review the SHEAP specifications with each employee prior to beginning the project. Submit the signed signature page (page 6) to the Project Manager within five (5) working days of the Notice to Proceed date.
8. If there is a visible tunnel/shaft leaving an existing hole that needs to be filled, do we do anything different to try and fill the shaft? Or do we simply fill the hole and not worry about the side tunnel/shaft?
Please see Article 36, Differing Site Conditions, of the General Conditions of the Contract.
9. If we use a remote controlled piece of equipment and do not put a person in the hole being filled, do we still need to lay the side slopes back?
All of the Features in the Bells Gap project are to be complete backfills to the ground surface.
10. For the holes that we do need to lay the side slopes back, do we have to lay all 4 sides back at a 3:1 or can we lay 1 side back and leave 3 sides vertical?
All of the Features in the Bells Gap project are to be complete backfills to the ground surface.
11. What type of compaction testing is required? If there is testing, is that something that will be provided or do we included geotechnical testing in our bid?
No compaction testing will be required. Backfill shall be compacted in lifts of one foot (1’) using multiple passes with available heavy equipment. Please see Section 2.5.1 Shaft, Stope, and Subsidence Feature Backfilling of the General Bid Specifications, 2009.

12. What type of MSHA/OSHA mine certifications/training is required for the workers on this project?
MSHA certifications/training is not required.
Please see Article 9, Accident Prevention and Safety Measures, of the General Conditions of the Contract regarding OSHA/MSHA requirements.
13. Is this governed by MSHA or OSHA?
Please see Article 9, Accident Prevention and Safety Measures, of the General Conditions of the Contract regarding OSHA/MSHA requirements.
14. For the large holes that the specs are requiring 1’-3’ sized boulder as fill, can we use material with fines in it?
The rockfill needs to be the specified volume of 1’-3’ sized rock. If a source of material contains rock and fines the material will need to be sorted and only the rockfill as specified installed.
All terms and conditions of the Invitation For Bid other than as changed by this amendment remain the same.
Receipt and acceptance of this amendment shall be acknowledged by submitting the Revised Bid Schedule.
