About this file
This document is an Indian Health Service (IHS) Buy Indian Act Indian Economic Enterprise (IEE) Representation Form, designed to self-certify an offeror's eligibility under the Buy Indian Act for federal contract opportunities. The form requires businesses to represent whether they meet the definition of an "Indian Economic Enterprise" with Indian ownership, which includes meeting specific criteria: at least 51% ownership by Indians or Indian Tribes, receiving at least 51% of contract earnings, and having management and daily operations controlled by one or more Indians.
The form warns that false or misleading information is punishable under federal law, and enterprises must maintain IEE status throughout the solicitation, contract award, and performance periods. Offerors must also be registered in the System of Award Management (SAM). The form provides fields for the business name, federally recognized tribal entity, UEI SAM number, 51% owner's name, and certification signature, serving as a critical documentation requirement for Indian Health Service contract opportunities seeking to provide preference to Indian Economic Enterprises.
