Statement of Work (SOW)

Special Education Services for Speech/Language Service Seba Dalkai Boarding School

1. BACKGROUND

Non-personnel services shall ensure that Seba Dalkai Boarding School is meeting requirements of the Individuals with Disabilities Education Act (IDEA, 34 Code of Federal Regulations 300.323. IDEA requires a free appropriate public education to eligible children with disabilities throughout the nation and ensures special education and related service to those children. Specifically, these non-personnel services are intended to help Seba Dalkai Boarding School meet all requirements for conducting and documenting the evaluation described in 34 CFR, Subpart D, as well a preparation and maintenance of each individual Education Program (IEP) for qualifying students.

2. SCOPE

Contractor shall provide qualified personnel, equipment, necessary materials, supplies and testing materials to perform speech pathology services. The Contractor will be required to perform speech/language service for all student in grades Kindergarten through eighth grade (approximately 14 student and future students that may qualify services) referred for evaluation during the course of the contract period. The Contractor will be expected to:

2.a. Be present during initial placement or multidisciplinary team committee meetings for student evaluated by the contractor.

2.b. Meet with and advise the special education staff, parents, students, and classroom teacher(s) and other service providers regarding the continuous assessment, evaluation and implementation of speech therapy programs for individual students and will meet with individual students on a regular basis to provide suggestions for carryover of therapy strategies into the home when necessary.

2.c. Upon the receipt of the parental permission form for assessment/evaluation by the school, the contractor shall:

I. Assesses students’ communication skills (e.g., articulation, language fluency, voice, etc.) for the purpose of determining their speck and language deficits and developing recommendations.

II. Consults with teachers, parents, other personnel and/or out professionals for the purpose of providing requested information, developing plans for services and/or making recommendations.

III. Participate in meetings: IEP meetings and multidisciplinary team meetings or the purpose of developing plans and/or providing information regarding student’s functional goals.

IV. Prepare documentation (e.g. evaluations, service logs, observation, progress contacts with parents, teachers, and outside professionals, etc.) for the purpose of providing written support, developing recommendations and/or conveying information.

V. Present information on various topics related to area of profession expertise in psychology or the purpose of communicating information and gaining feedback on treatment issues.

VI. Research resources and methods (i.e., intervention and treatment techniques, assessment tools and methods. community resources, etc.) for the purpose of determining appropriate approach for students’ speech and language services.

VII. Screen students for the purpose of determining and assessment according to laws regarding minority non-discriminatory practices.

Direct (in-person) student services shall be provided twice a week (6-8 sessions a day) for 30-40 minutes a day for 36 weeks throughout the school year and coordinated with the school. An additional 20 hours for evaluation and reporting preparation. During the annual student reviews parents will be notified by Special Education staff of any Compensatory education hours needed for their students. Parents may choose to make up the Compensatory hours during the regular school year or during the extended school year (ESY).

3. KEY PERSONNEL MINIMUM REQUIREMENTS

The contractor shall provide key personnel that meet or exceed the following minimum requirements. Evidence of meeting these requirements shall be provided to the Government with the Contractor’s quote and Replacement of key personnel will require prior coordination and approval of the school.

Education: Masters Degree in Speech Language Pathology

# of Years of directly Related professional experience: At least five (5) years experience with Native American students is required.

Certification(s) Certificate of Clinical Competence Speech-Language Pathology (CCC-SLP) preferred.

License(s) Arizona State License (License in state where services are to be provided.

Security Requirements New consultants shall comply with Background Investigation as required prior to commencement of any work to be performed. 25 – Chapter 34- Section 3201 (P.L. 101- 630); contact with or control over Indian children are subject to a background investigation and determination of suitability for employment. Contractor personnel shall present Government PIV issued identification card to the front desk and sign-in with the School Administrative Office prior to entering the school classroom. Contractor’s personnel are subject to background investigations and must successfully pass the background prior to performance under the contract.

5. PLACE OF PERFORMANCE

Contractor shall provide services at Seba Dalkai Boarding School which is located 35 miles north of Winslow, Arizona. Cost for travel and per diem are the responsibility of the contractor, the firm-fixed-price shall include these amounts for scheduled trips.

6. PAST PERFORMANCE ASSESSMENT

The School Administrator along with certified Special Education teacher will evaluate the Contractor’s performance under this contract, which will be shared with BIA contracting agencies and used as past performance references.

7. INVOICING REQUIREMENTS

All invoices shall follow the Three Way Match Process is method of matching the vendor or supplier invoice to the purchase order and getting confirmation of this match with goods/services received note before issuing payment.
