BUREAU OF INDIAN EDUCATION

TONALEA DAY SCHOOL

COUNSELING SERVICES

SCOPE: Tonalea Day School students are in need of Counseling Services. The contractor shall provide personal owned vehicle travel reimbursement, supplies, materials and personnel to provide Counseling Services for Bureau of Indian Education located on the Navajo Nation Indian Reservation at Tonalea Day School in Tonalea, Arizona.

BACKGROUND: The Bureau of Indian Education, Navajo Nation, serves as a branch of the State Education Agency (SEA) for several elementary and secondary schools and dormitories serving the Navajo Reservation students located in the states of Arizona, New Mexico, and Utah. The Statement of work calls for non personal professional related services to assist the targeted Navajo Nation Schools in implementing the special education and related services as authorized under P.L. 108-446 Individualized with Disabilities Education Improvement Act (IDEIA). The purpose of IDEIA program is to improve academic achievement of students with disabilities and introduce strict accountability measures to hold schools, districts and states responsible for the academic results of student with disabilities.

PLACE OF PERFORMANCE: Tonalea Day School, Tonalea, Arizona.

PERIOD OF PERFORMANCE: Base and 4-1 year option

IDENTIFIED STUDENTS: Based upon the need over the last five years at Tonalea Day School, we anticipate 5-10 students per year requiring services. The total hours required (based upon the students IEP) is per evaluation per day.

TASK 1. COUNSELING SERVICES:

1. Make prior arrangements with Tonalea Day School’s Special Education Teacher to develop a schedule to assess referred students as needed for initial or reevaluation and to schedule service delivery days.

2. Contractor shall give direct student counseling services according to IEP.

3. All Student services will be logged and maintained.

4. Weekly services while school is in session will be arranged and maintained on a regular schedule.

5. Contractor shall consult and work with other staff to provide optimum student services.

6. Contractor shall be available for staff and parent training, to give information in the area of expertise.

7. Contractor will provide suggested appropriate goals and objectives for student IEP development.

8. Contractor will make every effort to attend student IEP meetings.

9. Student progress reports will be written and given to parents according to IEP.

KEY PERSONNEL MINIMUM REQUIREMENTS:

The contractor shall provide key personnel that meet or exceed the following minimum requirements.

Evidence of meeting these requirements shall be provided to the Government with the Contractor’s quote and Replacement of key personnel will require prior coordination and approval of the school.

Education: Master's degree in social work, Psychology or Mental Health Counseling from an accredited program # of Years of directly Related professional experience: At least five (5) years experience with Native American students is required.

License(s)/Certification(s) Possess a valid, unrestricted license/certification from a state or U.S. Territory that grants legal authority to provide psychological and/or counseling services as an independent practitioner in the fields of Psychology, Social Work, Marriage and Family Therapy or a similar counseling field.

All Contracting candidates are required to complete background check before on-site services.

Replacement of key personnel will require prior coordination and approval.

Deliverables

Counseling Services:

Estimated 35 visits

Due within 5 working days of request by the COR

Documents shall be written in clear, understandable English that is devoid of grammatical, spelling and cut & paste errors.

Documents shall be delivered on time.

BACKGROUND INVESTIGATION

In accordance with 25 U.S.C. 3201 (P.L. 101-630), Indian Child Protection and Family Violence prevention Act; 42 U.S.C. 13041, Crime Control Act of 1990; 441 Departmental Manual, Personnel Security and Suitability Requirements all positions with duties and responsibilities involving regular contact with or control over Indian children are subject to a background investigation and determination of suitability for employment.

Accordingly, all such positions require a favorable screening (applicant and suitability) determination made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program; and a favorable background investigation determination conducted by the Office of Personnel Management (OPM).

A favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award or made within 60 days of incorporation of this clause into a current/existing contract. Pending completion of a background investigation by OPM, the contractor who has been granted a favorable screening (applicant and suitability) determination must be within the sight and under the supervision of a Government employee who has had a completed background investigation (42 U.S.C. 13041 (3)) during contract performance until a final background investigation determination has been made.

Prior employment with the Federal Government, including BIA, OIEP or the Bureau of Indian Affairs, does not assure that the appropriate screening or investigation was conducted nor does it alleviate or guarantee a favorable screening (applicant and suitability) determination or background investigation.

Prior to contract award or during the 60 day period discussed in paragraph two, it is solely the company, individual or contractor's responsibility to contact the BIA, OIEP Personnel Security and Suitability Program at (505) 563-5287/5288/5289/5294 in order to obtain a complete security package, which includes but is not limited to appropriate investigative forms and/or inquire about their previous investigation; and, obtain a screening (applicant and suitability) determination, and subsequent final background investigation.

If an unfavorable screening (applicant and suitability) determination is made, a contract shall not be awarded to the individual who received an unfavorable screening or to the company who has proposed to employ the individual to perform the work. The individual and or the company who proposed to employ this individual may be determined to be non-responsible if an unfavorable screening (applicant and suitability) determinations is made, which will make them ineligible for contract award.

If an unfavorable final background investigation determination is made, the contract may be terminated and the employee in question shall not be authorized to provide service to the BIA, OIEP, if the service involves contact with or regular control over Indian children.
