STATEMENT OF WORK

Windom VC & Reception Carpet Installation & Replacement

Windom Wetland Management District U.S. Fish & Wildlife Service

1. General:

1.1 Describes the project in general terms. (Who (Windom Wetland Management District, 49663 County Road 17, Windom, MN 56101 )), What (Replacement and installation of carpet in our Visitor Center, Reception and Administration office), and Where ( to our office headquarters located at 49663 County Road 17, Windom, MN 56101).

1.2 Purpose/End Application: Contractor shall provide all labor, tools, equipment, materials, transportation, and supervision for the removal and installation of carpet. The carpet at Windom WMD was new in 2000 and is/has been popping up in several spots causing potential tripping hazards for staff and visitors. Windom has repaired the carpet several times but continues to only be a temporary fix.

1.3 Contractor Qualifications: Contractor will provide all labor, equipment and materials to prep the area (move existing structures, equipment, furniture), remove existing carpet and installation of new carpet.

1.4 Other: Include, a relevant statutory authority, regulations, and/or references, pertinent background materials, or other relevant info. For extended information, reference an attachment or digital reference is recommended.

2. The carpet will be delivered to Windom Wetland Management District, Windom MN during the estimated period of December 1, 2021 through April 1, 2022, until the project is completed. Period of Performance will be finalized at the time of award.

2.1 The address the carpet will be removed and installed is 49663 County Road 17, Windom, MN 56101.

2.2 Annotate the Performance start and end date. Estimated period of December 1, 2021 through April 1, 2022, until the project is completed. Period of Performance will be finalized at the time of award. Annotate any conditions, limitations, or windows of opportunity impacted by the habitat, seasons or weather, or other projects.

2.3 Normal duty hours Monday through Friday 7:30 a.m. - 4:00 p.m. In the event of emergencies, Contractor shall provide contact information.

2.4 Non-duty hours include week nights, weekends, and Federally Recognized Holidays.

3. Scope

3.1 Contractor shall provide all labor, tools, equipment, materials, transportation, and supervision for the removal and installation of carpet. Contractor shall provide assistance in moving existing office structures, equipment and office furniture for the removal and installation of carpet in Visitor Center, Reception area and Administrative Office.

4. Deliverables and/or Delivery Schedule: Contractor will work with employees of Windom Wetland Management to schedule the removal and installation of carpet.

4.1 Clearly state the deliverables required, the schedule for delivery, the quantities, schedule (for multiple items). Marterials needed for this project:

1,700 square feet of Get Moving Tile/238/Sandstone, Item: getmoving238 (24”x24” carpet squares) 2 units of 4 gallons each (8 gallons total) Optiseal/sealant for carpet tile, Item Optiseal 2 units of 4 gallons each (8 gallons total) Carpet tile adhesive, Item: Enpress PSA 1,700 square feet of tear up and disposal of existing carpet.

1,700 square feet of labor/install carpet.

15’ vinyl trim-reducer, joiner, itc, Item: Vinyl Trim Labor to move existing structures, office equipment and office furniture.

4.2 Identify the type of documentation (printed and electronic-such as manuals) to be provided, as well as those quality indicators desired by the Government. Vendor will provide all manuals for the care of the carpet.

5. Data Management Plan (DMP) for Science Data Agency Reference. The general criteria is if the contract has a data collection component. Ideally, these requirements are flagged by the different program data stewards. Use the following link to establish if a DMP exists and for template for developing a DMP. Before submitting the SOW delete the link.

5.1 Annotate if a DMP is being used for data collection, when data is collected for a decision document, when new data is collected in the field, or with continued data collection such as in annual surveys. If a DMP is not relevant for ADHOC or when downloading existing data from the internet so state. NOTE: The Project Lead or technical cognizant has used discretion as to whether or not a DMP is relevant or not relevant and has determined it to be ___relevant or ___ not relevant . N/A

5.2 If the Plan is to be a deliverable as a prerequisite to data collection, so state and note as a deliverable in the Deliverable Element (above). OR N/A

5.3 If the Plan exists and will be available as Government Furnished Information, so state and note as GFI in the GFI Element (below) and attach or reference the Plan. N/A

6. Government-Furnished Equipment (GFE) / Government-Furnished Information

6.1 Identify any Government-furnished equipment (GPE) and Government- furnished information (GFI) or note N/A

6.2 Identify any limitations.

7. Evaluation and Acceptance The work shall be accepted when all the work is completed as described here within and in accordance with any contract specifications. Work will be inspected to assure compliance with the contract documents.

8. Quality Control/Quality Assurance:

8.1 For commercial supplies reference clauses for inspection and acceptance. Where appropriate, QC will be maintained according to industry standards and made available to the agency with the contractor’s proposal.

8.2 Identify and detail measures for assessing quality control and acceptance; each requirement is unique and a table format is visually friendly and may be desirable.

9. Technical Coordinator:

9.1 Identify the primary individual responsible for the final product or service. This individual is ordinarily the author of this document, the Program Manager, or other. Include the Name, title, address, phone number and email for each individual.
