About this file
The document is a Past Performance Questionnaire (PPQ) from the U.S. Department of the Interior National Park Service, used to evaluate a contractor's previous project performance. The form provides a structured method for project owners/evaluators to rate a company's performance across multiple dimensions, including relationship with the client, corporate management, quality control, and ability to meet performance schedules. The questionnaire uses a rating scale from Outstanding (O) to Unacceptable (U) and includes specific questions about customer satisfaction, potential negative notices, and willingness to award future contracts. The form is designed to be completed in two sections: Section A by the offeror with project details, and Section B by the project evaluator with performance ratings and comments.
