About this file

The document is a Department of Interior Past Performance Questionnaire (PPQ) designed for evaluating a contractor's previous project performance. The form is part of a solicitation (No. 140P6025Q0036) by the Department of the Interior National Park Service Midwest Region for a "LIHO - Replace Heat Pump" project. The PPQ requires contractors to submit 3-5 past performance projects completed within the last five years, with detailed ratings across ten performance categories including corporate management, quality control, schedule adherence, and overall relationship with the client.

The questionnaire instructs offerors to complete Section A with project details and then forward the form to a project owner/evaluator to complete Section B, which includes comprehensive performance ratings. Ratings range from "Outstanding" (O) to "Unacceptable" (U) and cover aspects such as contract team relationship, management integrity, quality control, and performance schedule. The form also requires evaluators to indicate whether they would award another contract to the company and if the customer was satisfied with the final product and services. Completed questionnaires must be submitted by the specified date and time in the solicitation, with electronic submission delays taken into account.
