About this file

The document is a Past Performance Questionnaire (PPQ) from the U.S. Department of the Interior, National Park Service for evaluating a contractor's previous project performance. The form provides a structured method for previous project owners to rate a company's performance across multiple dimensions, including relationship with the contract team, corporate management, quality control, and ability to meet performance schedules. The questionnaire uses a rating scale from "Outstanding" (O) to "Unacceptable" (U) and includes specific questions about customer satisfaction, potential negative actions, and willingness to award future contracts. The evaluator is asked to provide detailed feedback and an overall rating, which will be used in assessing the company's capabilities for a future contract opportunity.
