Method of Surveillance

PRS # 1

The Contractor is required to notify the COR and Contracting Officer of the inconsistency within three days of identification as related to section 3.1

The Contractor performs the required testing in accordance with HHS and DOI guidelines.

The Contractor performs HHS panel testing with no more than 1 significant error

Monthly review of test results for verification of correct panel

PRS # 2

Collections in public establishments or personal residences are not authorized. An exception to this rule shall be approved by the appropriate Agency before the collection is made as related to section 3.1.1

The Contractor performs the required testing in accordance with HHS and DOI guidelines.

The Contractor provides negative test results to the Medical Review Officer within 24 hours and not more than 48 hours after obtaining results.

Monthly review of test results average time to be sent the Medical Review Officer

PRS # 3

Contractor shall ensure the availability of collection services 24 hours a day/7 days a week at testing locations in the event post-accident or reasonable suspicion testing is required at off-site and/or on-site locations as related to section 3.1.1

The Contractor performs the required testing in accordance with HHS and DOI guidelines.

The Contractor reports all positive tests for a specific drug to the Medical Review Officer within an average of 48-72 hours after receipt of specimen

Review monthly of test results average time to be sent the Medical Review Officer

PRS # 4

Alcohol testing shall be completed within two hours of notification to the Contractor for post-accident testing, and within two hours of notification to the Contractor for reasonable suspicion testing as related to section 3.1.1

Litigation package information includes a laboratory summary report; affidavit; custody and control documents;

screening and confirmation test printouts; laboratory certifying documents;

certifying scientist and technical staff qualifications.

The litigation package is accurate and does not contain technical or grammatical errors.

Quarterly review of response to litigation responses

PRS # 5

The Contractor shall not make notifications to applicants when they don’t have a reasonable amount of time (48 hours) to get to the collection site (i.e. on Fridays or when the collection site is not open on the weekends) as related to section 3.1.2

The Contractor prepares and ships split specimens to a second certified laboratory in accordance with PWS Section 2.4.

Zero deviation from the performance standard.

The COR perform a quarterly review of the Contractor’s actions taken on splits sent to a second lab

PRS # 6

The Contractor shall submit a monthly Financial Progress Report (FPR) for each task order issued to the Contracting Officer and COR

The contents of the FPR shall include all, cumulative, relevant financial data from the effective date of each task order through the month preceding the FPR submission IAW PWS section 3.1.12.1 Financial Progress Report

10th of each month COR reviews monthly
