M - Evaluation Factors for Award

The Bureau Of Land Management (BLM) will award to the responsible firm whose quote conforms to the RFQ and will be the most advantageous to the Government after an evaluation of Certifications/Qualifications, Past Performance and Price in order to select the overall “best value” offer. Therefore, the award may be made to other than the lowest price offered. The evaluation criteria are listed below. Technical factors other than cost/price when combined, are considered significantly more important than cost/price.

Past performance. The Government will evaluate the offeror's past performance deemed relevant to the requirements of this RFQ. The Government will use information submitted by the offeror and any other sources of information available to assess past performance. The Government will evaluate past performance on an Acceptable/Unacceptable basis as follows:

■ Acceptable: Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown.

■ Unacceptable: Based on the offeror’s performance record, the Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.

NOTE: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror shall be determined to have unknown (or “neutral”) past performance. ○

Past Performance Sample Submission Instructions: The offeror shall provide past performance information for no more than three (3) of the most relevant veterinarian service contracts performed for Federal agencies, state/local governments and/or commercial customers within the last three (3) years from the issue date of this RFQ or currently being performed. Relevant contracts include contracts similar or greater in scope, magnitude, and complexity than the effort described in this RFQ.

Please list the following information: Year, title, award amount, point of contact and validate the point of contact’s telephone number is current. If evaluation panel can not contact the reference because of a invalid phone number, it may be reflected in your evaluation score.

Personnel qualifications. List key supervisors and personnel qualifications with area-specific experience of performing veterinary service similar or greater than the requirements in this RFQ statement of work. Submit each Veterinarian’s licenses who will perform services on horses. Veterinarian shall be licensed in the State of Kansas with USDA accredited for issuing interstate health certificates and skilled in handling and treating ungentle large animals specifically horses and burros.

(Relevant experience can also be included in previous projects.)

Insurance. Submit copy of insurance that covers your veterinary practice for unprofessional conduct described under Kansas Board of Veterinary Medical Examiners.

Price: Price will be evaluated by the firm fixed price provided by the contractor to Include all services to complete the Statement of Work.
