SCOPE OF WORK

PHYSICAL SERVICES FOR JOHN F. KENNEDY DAY (JFK) SCHOOL

John F. Kennedy Day School are in need of an On-Site/Direct Physical Therapy Service. John F.

Kennedy Day School is located on the White Mountain Apache Indian Reservation near Cedar Creek, AZ which is about 35 minutes from Show Low, AZ.

Background John F. Kennedy Day School serves students on the White Mountain Apache Indian Reservation. The scope of work calls for professional related services to assist the school in implementing special education and related services as authorized under P.L. 108-446 Individuals with Disabilities Education Improvement Act (IDEIA) The purpose of the IDEIA program is to provide a free appropriate public education to students who have an identified disability and by reason thereof require specialized instruction, including related services, to meet their unique educational needs.

There are approximately 2 students who presently require physical therapy service. The hours and reports are estimated and based on student’s IEPs and student enrollment.

The Contractor shall provide the following:

1. Provide Physical Therapy Services John F. Kennedy Day School.

2. Services will be provided in accordance with P.L. 108-446, the Individuals with disabilities

Education Improvement Act of 2004, as specified in individual student IEPs

3. Services will include direct therapy to students, consultations with staff/parents, evaluations/assessments, providing documentation/service logs of therapy sessions and offer compensatory time when and if needed.

4. Provide direct service log for each student and each visit; and will be asked to meet with the Special Education Department at the beginning of the contract. Further, the contractor will be involved in a school-wide Child Find and Screening for students as needed.

5. Provide Physical Therapy Services and evaluations (both initial and re-evaluations) to the students John F. Kennedy Day School and complete services as outlined in Individualized Education Program’s (IEP) when/if determined eligible.

6. The contractor will service students between 8 am to 4 pm, excluding holidays, one to two days per month.

7. The contractor will be available to answer any questions whenever necessary by email, phone calls, virtual meetings, and when on-site to parents/guardians and/or JFK staff.

8. Observes, screens, evaluates students and provide documentation of student referrals.

9. Attends and participates in multidisciplinary team meetings.

10. Develops and/or implements therapy plans including a narrative description of students exact needs and specific, measurable individual goal statements, and completes quarterly progress reports.

11. Maintains service/directions to teaching assistant who provides direct services to students as outlined in Individualized Education Program’s (IEP’s).

12. Provides supervision direction to teaching assistant who provides direct services to students in IEP’s.

13. Identifies equipment needs and makes recommendations of needs to the Special Education Department.

14. Completes student reevaluations and initial evaluations in accordance to BIE timelines.

15. Provides written evaluation reports to the multidisciplinary team prior to the team meetings.

16. Completes and submits to the Special Education Department written reports of student progress at the end of each school quarter.

17. Provides training and support as needed to staff and parents.

18. Provide Equipment, Tools, Materials and Supplies.

19. Contractor shall provide all equipment, tools, materials and/or supplies that are needed by

Provider to accomplish the services agreed to be performed.

20. Submit all documentation of services and reports to the Special Education Department and local business office.

21. Collaborate with the Special Education Coordinator to establish weekly/monthly schedule.

22. Meet with Special Education Department on the first day of work to organize structure for service provision and procedures delineated for providing specified services.

KEY PERSONNEL MINIMUM REQUIREMENTS:

The contractor shall provide key personnel that meet or exceed the following minimum requirements.

Evidence of meeting these requirements shall be provided to the Government with the Contractor’s quote and Replacement of key personnel will require prior coordination and approval of the school.

Education: Master's Degree in Physical Therapy- Graduate of a professional physical therapist program accredited by a national accreditation agency approved by the board.

# of Years of directly Related professional experience: At least two (2) years of experience and preferred experience with Native American students but not required.

Certification(s) National Board Certification is recommended but not required.

License(s) Arizona State License (License in state where services are to be provided.)

ALL Contracting candidates are required to complete background check before on-site service.

Virtual Service:

Strictly at the discretion of school, not an option to circumvent on-site/direct services.

Scheduling will be determined by the Business Technician based on weather conditions, school testing periods, school-based activities, as well as other unforeseen circumstances.

Documents shall be written in clear, understandable English that is devoid of grammatical, spelling and cut & paste errors.

Documents shall be delivered on time.

Service Fee is an All-inclusive mileage, meetings, etc. conducted on the same day.
