About this file

This is a Past Performance Questionnaire for an Indefinite Delivery/Indefinite Quantity (IDIQ) contract for Gaseous Air Pollution and Meteorological Monitoring for the National Park Service. The document provides a standardized form for offerors to submit past performance information, with specific instructions requiring 3-5 project questionnaires completed within the last five years. The questionnaire is divided into two sections: Section A to be completed by the offeror with project details, and Section B to be completed by the project owner, which includes a detailed performance rating scale ranging from Excellent to Unsatisfactory.

The form requires comprehensive documentation of previous project performance, including project name, value, completion date, and a detailed description of work. The project owner must evaluate the contractor's performance across multiple dimensions such as quality of workmanship, contract requirement conformance, issue resolution, project management, timeliness, responsiveness, and overall cooperation. The document emphasizes that the information is for source selection purposes and is considered Source Selection Sensitive, with strict guidelines prohibiting contractors from using the questionnaire for promotional purposes.
