Solicitation W91B4N-20-R-2007 OQAB Guardian Angel Request for Proposal

Attachment 2 Subcontractor/Teaming Member Consent Form for the Release of Past Performance Information to the Offeror

Past performance information concerning subcontractors and teaming members cannot be disclosed to a private party without the subcontractor's or teaming member’s consent. Because an offeror is a private party, the Government will need that consent before disclosing subcontractor/teaming member past and present performance information to the offeror during exchanges. In an effort to assist the Government's Past Performance Evaluators in assessing your past performance information, the following consent form must be completed by the major subcontractors/teaming members identified in your proposal. The completed consent forms shall be submitted in Volume III of your proposal.
