COMBINED SYNOPSIS AND SOLICITATION:

This is a Combined Synopsis/Solicitation for commercial items prepared under the authority of and in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6-

Streamlined Procedures for Evaluation and Solicitation for Commercial Product and Commercial

Services, as supplemented with additional information included in this notice. This notice constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is also issued under the authority of and in accordance with FAR Part 13- Simplified Acquisitions Procedures.

This solicitation is a Request for Quotation (RFQ). This RFQ is a Small-Business set-aside

Competitive Solicitation. The solicitation and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2024-07 effective 08/29/2024.

The North American Industrial Classification System Code (NAICS)- 339113 Surgical

Appliance and Supplies Manufacturing with a small business size standard of 800 personnel is applicable to the current requirements. The Product Service Code (PSC) 6515- Medical and

Surgical Instruments, Equipment, and Supplies, is applicable to the current requirements.

This RFQ is being issued by the United States Department of the Interior (DOI), Bureau of

Safety and Environmental Enforcement (BSEE). The purpose of this RFQ is to obtain competitive quotations in response to the requirements: Hearing Conservation-custom hearing protection. This is a new requirement and no previous contract was issued.

SECTION 1

PRICING

1.1 GENERAL

The United States Department of the Interior (DOI), Bureau of Safety and Environmental

Enforcement (BSEE), anticipates awarding a firm fixed price Order for the purchase of on-site audiometric testing, training, hearing protection device fitting, and employee notification services. This acquisition is a commercial purchase in accordance with Federal Acquisition

Regulation (FAR) Part 13. The North American Industry Classification System (NAICS) code for this acquisition is 339113 Surgical Appliance and Supplies Manufacturing with a Product

Service Code (PSC) of 6515 Medical- Supplies , and a small business size standard of 800

Employees is applicable to the current requirements.

SECTION 2

STATEMENT OF WORK

On-Site Custom molded Hearing Protectors

2.1 Background:

The Bureau of Safety and Environmental Enforcement (BSEE) has a need for custom molded hearing protectors. The Bureau has employees who are exposed to noises that exceed an 8-hour time-weighted average of 85 dBA. This exceedance requires BSEE to have a hearing protection program designed to protect employees from this hazardous noise. The preferred method for protecting employees is through engineering controls, however this is not possible with the work that the BSEE inspectors and engineers perform therefore we must provide them with personal protective equipment (PPE) to reduce their noise exposure. Custom fit hearing protection is a form of PPE that has several benefits, the main ones being a superior and comfortable fit for consistent, effective noise reduction, and better sound quality due to precision filters.

2.2 Objective:

This statement of work is for the performance of on-site earmold impressions and custom hearing protection for BSEE employees exposed to occupational noise levels above 85 decibels

(dB) or more over an eight-hour time weighted average as required by the Occupational Safety and Health Administration (OSHA) regulation, 29 CFR 1910.95 Hearing Conservation.

Employee earmold impressions shall be performed by a licensed or certified audiologist, otolaryngologist, or other physician, hearing instrument specialists, or other certified hearing health professionals. A technician who performs the earmold impressions must be under the direction of an audiologist, otolaryngologist, or physician.

2.3 Scope:

Perform earmold impressions and provide custom hearing protectors with a minimum Noise

Reduction Rating (NRR) of 15 or an average attenuation of 16dB for up to 120 Bureau employees. These employees will be located at the CAJUNDOME & Convention Center in

Lafayette, LA between January 26-30, 2026.

2.4 Deliverables:

1. Vendor must be able to provide on-site earmold impression services.

2. Vendor shall provide custom hearing protectors with a minimum NRR of 15 or an average attenuation of 16dB that includes an earplug lanyard and carrying case. Hearing protectors with acoustic filters are preferred.

3. Vendor shall have the capability to accommodate multiple days onsite between January 26-30, 2026 to perform the earmold impression services.

5. Vendor must include an earset adjustment process for any fit issues.

6. Services will be provided at specified and mutually agreed upon date(s)/time(s) between

January 26-30, 2026 between the hours of 0700 and 1800 hours.

SECTION 3

TERMS AND CONDITIONS

3.1 TYPE OF ORDER

A FFP Purchase Order shall be issued under the authority of the Department of the Interior

(DOI), Bureau of Safety and Environmental Enforcement (BSEE) for the effort identified in

Section 2, Statement of Work.

3.2 DELIVERY DATE/PERIOD OF PERFORMANCE

The period of performance shall be (4) four months.

3.4 INVOICING FREQUENCY AND PAYMENTS

One (1) invoice, after acceptance of the deliverables by the Government, shall be submitted via the Internet Payment Platform (IPP) as delineated in Section 3.4.1 below.

3.4.1 Invoice Submission Instructions

Payment requests must be submitted electronically through the U. S. Department of the

Treasury's Invoice Processing Platform System (IPP).

"Payment request" means any request for contract financing payment or invoice payment by the

Contractor. To constitute a proper invoice, the payment request must comply with the requirements identified in the applicable Prompt Payment clause included in the contract, or the clause 52.212-4 Contract Terms and Conditions - Commercial Items included in commercial item contracts. The IPP website address is: https://www.ipp.gov.

Under this contract, the following documents are required to be submitted as an attachment to the

IPP invoice: Documentation required by FAR 32.905(b). One invoice shall be submitted after all deliverable have been received.

The Contractor must use the IPP website to register access and use IPP for submitting requests for payment. The Contractor Government Business Point of Contact (as listed in SAM) will receive enrollment instructions via email from the Federal Reserve Bank of St. Louis (FRBSTL) within 3 - 5 business days of the contract award date. Contractor assistance with enrollment can be obtained by contacting the IPP Production Helpdesk via email

IPPCustomerSupport@fiscal.treasury.gov or phone (866) 973-3131.

If the Contractor is unable to comply with the requirement to use IPP for submitting invoices for payment, the Contractor must submit a waiver request in writing to the Contracting Officer with its proposal or quotation.

3.5 DIAR 1452.201-70 AUTHORITIES AND DELEGATIONS (SEP 2011)

(a) The Contracting Officer is the only individual authorized to enter into or terminate this contract, modify any term or condition of this contract, waive any requirement of this contract, or accept nonconforming work.

(b) The Contracting Officer will designate a Contracting Officer's Representative (COR) at time of award. The COR will be responsible for technical monitoring of the contractor's performance and deliveries. The COR will be appointed in writing, and a copy of the appointment will be furnished to the Contractor. Changes to this delegation will be made by written changes to the existing appointment or by issuance of a new appointment.

(c) The COR is not authorized to perform, formally or informally, any of the following actions:

(1) Promise, award, agree to award, or execute any contract, contract modification, or notice of intent that changes or may change this contract;

(2) Waive or agree to modification of the delivery schedule;

(3) Make any final decision on any contract matter subject to the Disputes Clause;

(4) Terminate, for any reason, the Contractor's right to proceed;

(5) Obligate in any way, the payment of money by the Government.

(d) The Contractor shall comply with the written or oral direction of the Contracting Officer or authorized representative(s) acting within the scope and authority of the appointment memorandum. The Contractor need not proceed with direction that it considers to have been issued without proper authority. (written or oral) that the Contractor considers to exceed the COR's appointment, within 3 days of the occurrence. Unless otherwise provided in this contract, the Contractor assumes all costs, risks, liabilities, and consequences of performing any work it is directed to perform that falls within any of the categories defined in paragraph (c) prior to receipt of the Contracting Officer's response issued under paragraph (e) of this clause.

(e) The Contracting Officer shall respond in writing within 30 days to any notice made under paragraph (d) of this clause. A failure of the parties to agree upon the nature of a direction, or upon the contract action to be taken with respect thereto, shall be subject to the provisions of the

Disputes clause of this contract.

(f) The Contractor shall provide copies of all correspondence to the Contracting Officer and the

COR.

(g) Any action(s) taken by the Contractor, in response to any direction given by any person acting on behalf of the Government or any Government official other than the Contracting

Officer or the COR acting within his or her appointment, shall be at the Contractor's risk.

3.6 CLAUSES INCORPORATED BY REFERENCE

In accordance with FAR 52.252-2, this order incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. The Contractor shall comply with these clauses unless the circumstances do not apply. Upon request, the Contracting Officer will make their full text available. (d) Comptroller General Examination of Record. 3.8 FAR 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The

Contracting Officer may exercise the option by written notice to the

Contractor within 5 days of contract expiration.

3.9 CONTRACTOR’S QUOTE

The Contractor’s quote may be incorporated by reference into the contract. Any inconsistencies or in event of a conflict between terms and conditions of the contract and the contractor’s quote shall be resolved by giving precedence to the terms and conditions of the contract.

QUOTATION SUBMISSION INSTRUCTIONS

NOTICE: The failure to submit any of the information requested in this RFQ may lead to the rejection of your quotation without further consideration.

4.1 FAR 52.252-1, SOLICITATION PROVISIONS INCORPORATED BY

This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. 4.2.1 Quotation Submission Instructions

Quotations shall be submitted in two (2) volumes (Technical Submission and Price Submission) as detailed below.

This RFQ in no way obligates the Government to issue or award orders. The Government will not be responsible for any costs incurred by responding to this RFQ or subsequent orders.

4.2.1 Technical Volume

(a) The Technical Submission shall not contain price information.

(b) The Technical Submission shall be as succinct as possible, but no more than 5 pages, maximum. A cover, cover letter, table of contents, and tab dividers are allowed and are excluded from the page limitations. Any pages in excess of these limitations will not be evaluated. Your

Custom Hearing Protection Page 15 of 23 submission may use any font, and will use pitch size 12, single spaced, double sided, 8.5” x11” paper. A single sheet of paper printed on both sides shall be considered two pages.

(c) The Technical Submission must state in detail how the offeror intends to comply with the work requirements in the Statement of Work (Section 2) of this RFQ, not merely that the offeror intends to do so. The Technical Submission shall, at a minimum, contain a work plan for how the offeror will successfully accomplish the requirements in Section 2.3 and provide the deliverables in Section 2.4.

4.2.2 Price Volume

The Price Submission shall include a pricing structure in accordance with the proposed technical solution and Section 1.2 of this RFQ. The Price Submission shall include:

(a) Detailed pricing that provides a basis of estimate to support your proposed pricing including, at a minimum, the following data:

1. Labor rates by personnel name, labor mix, estimated direct labor hours, a description of the skills and experience per category, and any other proposed associated costs, for calculating the proposed cost for this effort.

2. If consultants and/or subcontractors are proposed, detailed data concerning proposed consultant costs should include the following:

a. Names of consultant(s) and/or subcontractors to be engaged.

b. Daily fees or hourly rates to be paid to each consultant and or subcontractors.

c. Estimated number of hours of consulting services.

3. Materials, Equipment, and Other Direct Costs – If proposed, must be itemized with costs.

4. A statement that your quote is valid for a minimum of 120 calendar days after the date specified for the receipt of quote.

5. A statement confirming that your organization is registered and active with the

System for Award Management (SAM) (see http://www.sam.gov for more details).

Additionally, your Representations and Certifications must be updated in SAM in accordance with FAR 4.12. Failure to adhere to this requirement may result in your quote being determined to be ineligible for award.

4.2.3 QUESTIONS DUE DATE
