About this file

This is a Vendor Response Form template designed for use with a Sources Sought Notice issued by the Food and Drug Administration (FDA). The form is a Microsoft Excel-based template that vendors must complete and return in native format to express interest in a federal contract opportunity for custom analytical reference standard solutions.

The response form requires vendors to provide comprehensive company information including name, address, Unique Entity Identifier (UEI)/CAGE code, points of contact, and confirmation of active SAM.gov registration. Vendors must identify their business size under the stated NAICS code and indicate any applicable socioeconomic categories (8(a), HUBZone, SDVOSB, VOSB, WOSB, EDWOSB, SDB). The form requires vendors to detail their role in fulfilling the requirement, provide evidence of capability and capacity to meet the full scope of work, and describe relevant recent experience and applicable certifications. For small businesses acting as distributors or resellers, the form mandates identification of manufacturers and evidence of authorized distribution. Additionally, vendors must address domestic manufacture and availability by specifying where end items are manufactured, certifying domestic content percentages if applicable, identifying country of origin for critical components, and describing production capacity against stated quantities. The form includes sections for vendors to detail any teaming, joint venture, or subcontracting arrangements and to identify existing contract vehicles such as FSS/MAS, GWAC, or IDIQ/BPA agreements they may leverage.
