About this file

This statement of work outlines the requirements for a five-year blanket purchase agreement to provide laboratory gases and related services to the Uniformed Services University of the Health Sciences and the Armed Forces Radiobiology Research Institute. The contractor shall deliver liquid nitrogen, liquid argon, and other gases listed in the document on a weekly or as-needed basis. Additional requirements include refilling gas cylinders, transporting cylinders between the two facilities, cylinder maintenance such as valve replacements and pressure testing, and providing monthly usage reports. The contractor must meet all safety and regulatory standards and hold insurance for workers compensation, general liability, and automotive liability. The quality assurance surveillance plan establishes performance metrics for timely deliveries, coordinating services, and complying with contractual obligations.

STATEMENT OF WORK

LABORATORY GASES

I. Introduction

The Uniformed Services University of the Health Sciences (USUHS) is the Nation's federal health sciences university and is committed to excellence in military medicine and public health during peace and war. As a medical school, which includes several multi-disciplinary research laboratories, USUHS requires the delivery and related services of liquid nitrogen, liquid argon, and other laboratory gases, refer to the table below in Section VIII.

II. Scope of Services

The contractor shall perform all necessary services, qualified personnel, materials, facilities, and equipment not otherwise provided by the Government as needed to perform the work described below:

1. The contractor shall pick up various gas cylinders that are owned by USU and AFRRI.

2. Provide refill of gas cylinders and deliver gases to USUHS and AFRRI, refer to addresses below.

3. Painting services are provided by the vendor upon request.

4. Cylinder static testing and valve replacements are to be performed in accordance with industry standards for all cylinders on an as needed basis.

5. Turnaround time for cylinders shall be no more than one week for all gases. Any deviations from this schedule must be approved by the USU Point of Contact (POC).

6. Contractor delivery representatives will meet directly with USU receiving personnel when exchanging cylinders to verify cylinders are being picked up, delivered and recorded. This is performed to maintain accountability and ensure turnaround times are being met.

7. The contractor shall notify the USU Point of Contact, the receiving supervisor, when items are identified as out of standards.

8. A level reading must be taken before and after filling the tanks at both USU and AFFRI. The readings must be provided with the invoice. A USU or AFFRI representative may be present.

9. The contractor shall provide a monthly summary of the USU and AFRRI Delivery Log which will identify the quantity of items processed, item charges, and total charges for each call. This summary will be provided to the Receiving Point of Contact (or Authorized Caller as back-up), who will verify accuracy and provide return confirmation to the vendor at the time of delivery. The contractor must then issue a formal monthly invoice to the BPA Caller. Once the invoice is received, processing for payment will be initiated by the authorized caller.

III. Contractor Qualification Requirements

The contractor shall be solely responsible for obtaining all necessary licenses and permits, and for complying with all applicable Federal, State and local laws, codes, and regulations in connection with the execution of the work to be performed under this Blanket Purchase Agreement (BPA).
