All work under this contract shall be in performance with all local, state, and federal laws, regulations, and codes.

Only the awarded contractor shall provide a copy of their insurance certificate required by State and Local Governments that has been acquired for the time period covered under this contract to the Contracting Officer within 10 business days after award. Section C - Descriptions and Specifications

SCOPE OF WORK

Summersville Lake 2981 Summersville Lake Rd, Summersville, WV 26651

Battle Run Utility Improvements Furnish all equipment, material, labor, supplies, and supervision needed to provide and install new potable water valves, pressure reducing valves, flow meters, manhole replacement, culverts, a new water service, and other related appurtenances at the Battle Run Recreation Area and Campground in accordance with the attached scope of work and attached plan drawings.

Expected Start Date: Work shall commence within 30 days of issuance of NTP, but no sooner than 12

October 2021, after the recreation area closes for the season.

Contract Duration: Project shall be complete within 60 calendar days of work commencement.

Project Location: 2981 Summersville Lake Rd, Summersville, WV 26651.

1. SCOPE OF WORK

1.1 General Description: The Contractor shall install new potable waterline valves, flow meters, pressure reducing valves, related appurtenances, and a new water service tap with accessories in the Battle Run Recreation Area at Summersville Lake. The Contractor shall also replace one manhole and install a catch basin and storm sewer culver in Battle Run Campground. The project area is located approximately one mile west of Summersville Dam and 0.3 miles north along Battle Run Road, and is shown on Attachment A, Location and Vicinity Maps. All items shall be installed, tested, and waterlines disinfected for a complete turn-key product. The Recreation Area will be closed to the public during the contract and no provision to maintain existing water or sewer service during construction is required. However, other contractors will be working in the area. All excavation is unclassified;

however, based on previous excavations in the area, solid rock is expected in all areas. Note that this contract replaces or installs new valves on existing waterline and sewerline; therefore, work will be accomplished in the existing trenches. However, if any additional excavation is required to facilitate the installation (particularly the new culvert), it will most likely be in solid rock.

1.2 Real Estate Requirements: The Contractor shall keep work activities confined to a fifty-foot (50’) wide work area (25’ on each side of the proposed feature being replaced). Equipment and Material storage, employee parking, and other incidental work areas shall be at the Bath House Parking Lot in the Campground. Should additional equipment and material storage areas on Government property be required during construction, the Contractor shall submit a request to the Contracting Officer’s Representative (COR) for approval at least 48-hours before the area is needed for use. The contractor shall not completely block or close any site access road. One lane shall be kept open at all times. Government employees and other contractors working in the area must be able to pass on all roads in the work area. Traffic control for WV-129, in accordance with the latest edition of the Manual of Uniform Control Devices and all applicable West Virginia Department of Highways requirements, shall be provided when working on the new tap. The Contractor shall obtain any necessary permits from the WV Department of Highways for work within the highway right-of-way.

1.3 Installation Requirements: Waterline materials and installation shall be in accordance with all applicable standards of the American Water Works Association (AWWA) and the State of West Virginia Bureau of Public Health regulations. The contractor shall clean the work area at the end of each workday and shall maintain a clear and safe means of ingress and egress from the work area at all times. The use of the site for construction staging shall be coordinated with the on-site Government representative. Upon completion of construction, the Contractor shall clean all debris and waste resulting from construction operations and restore all surfaces, structures, ditches, and property to its original condition to the satisfaction of the Contracting Officer. All waste materials shall be disposed at an approved landfill facility. Excess excavation materials (soil and rock) may be placed in low spots in the emergency spillway if graded smooth and seeded and mulched.

All excavation required for this project is unclassified. No additional payment will be made for rock excavation.

The contractor shall thoroughly examine the site and the excavation requirements required to install the work.

Although no rock soundings have been performed at the site, it is anticipated that any additional excavation required will be in solid rock.

The items and location of work for this contract are shown on Attachment B. All waterline and fittings shall be in accordance with its applicable AWWA Standard for material and type. All fittings shall be ductile iron or brass – no plastic. All valves shall be Resilient Wedge Gate Valves per AWWA C509 unless otherwise specified. Trench excavation, bedding, refill, and surface restoration shall be according to the details provided in Attachments C and D. After installation, the contractor shall thoroughly flush all new lines and leak test all newly installed items in accordance with AWWA M23/M55. Before being placed in service, all new lines and appurtenances shall be disinfected in accordance with AWWA C651.

Due to the drawing scale of Attachment B, the location of work items is approximate. Before ordering any materials, the Contractor shall meet with the on-site Government Representative and completely walk the site and inventory all work to be performed. The inventory shall consist of physically locating each item, flagging the location, determining the size, type, material, and what fittings/materials will be necessary to accomplish the work in accordance with these specifications.

Existing valves shall be replaced where shown on Attachment B. Replacement valves shall be resilient wedge gate valves as shown on Attachment C. All valves shall be installed with proper fittings as required and a cast iron valve box with lid marked “WATER”. All replacement valve boxes shall have a concrete apron. All new valves shall match the existing waterline size exactly. New valves shall be installed where shown on Attachment B. New valves shall be cut into the existing line and be resilient wedge gate valves as shown on Attachment C. All valves shall be installed with proper fittings as required and a cast iron valve box with lid marked “WATER”. All new valve boxes shall have a concrete apron. All new valves shall match the existing waterline size exactly.

New flow meters for leak detection shall be installed in existing lines where shown on Attachment B and installed in accordance with Attachment D. Flow meters shall be 3” Diameter and suitable for use in Drinking Water Applications. Meters shall have an analog display with a “spinning gear” that indicates low flow rates, flow rate indication, and flow totalizer. The Unit Totalizer shall Measure in Gallons. Pulse Output shall be 1 pulse per 100 gallons, a dry contact (i.e. no voltage) reed switch with 2 wires, 1.5m. Accuracy shall be ±2% as per AWWA C701 test procedure. Units shall meet AWWA C701 and ISO4064 Certifications. Connections shall be ASME Class 125 Flanges per B16.1. Maximum Water Temperature 104˚F (40˚C). Maximum Working Pressure 232 psi. Meter Body shall be epoxy coated cast iron. Unit size shall be 8.86 inch (length), 7.5 inch (width) and 10.9 inch (height).

Maximum Flow Rate shall be 350 GPM. Unit shall have no more than 1 PSI pressure drop at 240 GPM. Minimum measured Flow Rate: 8 GPM. Unit shall be installed in a horizontal position with dial facing upwards. Meter must be installed with direction of flow as indicated by arrow cast into the meter body. Install meter in a location with at least 10 diameters of straight pipe at the inlet and 5 diameters at the outlet to assure proper flow profile to meter.

Maximum Reading: 999,999,999 gallons. Minimum Reading: 1 gallon. The 1st digit from the right shall have a multiplier of 1000 gallons. Display shall be sealed dry dial for clear readings. The pulse emitter device shall consist of a plastic housing with Reed Switch and 1.5 m cable with 2 cores in red and black. Meter register assembly shall be able to be removed for repair or replacement without disrupting the water flow. Unit shall operate at low pressure loss and offer excellent accuracy. Manufacturer shall provide a 2-year replacement warranty. Contractor shall provide all fittings and reducers as necessary to make the connection to the new flow meters. Flow meters shall be installed in a polymer concrete meter box with a matching lockable or bolted lid marked “WATER”, Box shall have an ANSI T22 load rating. Box dimensions shall be 24” (width), 36” (length), and 36” (depth) and the box shall have an open bottom. Meter box shall have pre-formed “mouseholes” at the bottom of the box for pipe entry/exit through the length dimension of the box (pipe shall be oriented in the center of the longest dimension).

Pressure Reducing Valves shall be installed in the existing vaults at each comfort station in place of the backflow prevention units currently in these vaults. All shut-off valves shall remain in the vault. Ductile iron or brass fittings shall be used as necessary to facilitate installation. All pressure reducing valves shall match the existing line size exactly. The old backflow preventers shall be provided to the Government. New Pressure Reducing Valves shall be installed in existing lines were shown on Attachment B. A Lead-Free Water Pressure Reducing Valve shall be installed as shown on the plans. The water pressure reducing valve shall be constructed using Lead Free materials. Lead Free regulators shall comply with all state codes and standards that require reduced lead content. Provision shall be made to permit the bypass flow of water back through the valve into the main when pressures, due to thermal expansion on the outlet side of the valve, exceed the pressure in the main supply. Pressure reducing valves with built-in bypass check valves and strainer will be acceptable. Approved valves shall comply with ASSE 1003. Maximum working pressure shall be 300 PSI. Adjustable pressure range shall be 40-80 PSI. All pressure reducing valves shall exactly match the size of the line where it is being installed. Where a new pressure reducing valve is to be installed in an existing line, a new meter box (matching that specified for the new water meters above) shall be provided with the installation.

Manhole No. 4 shall be completely replaced as shown on Attachment E. This manhole receives a very large amount of infiltration. The Replacement Manhole shall be watertight and eliminate existing infiltration issues. To ensure this, a groundwater collection drain shall be installed around the manhole after it is replaced. Refer to Attachment E for a detail of the work required. The groundwater collection drain shall discharge to a new WV-DOH Type B Drop Box Inlet. A new 12” culvert shall be installed in the horizontal location of the existing culvert, but installed to the depth and slopes identified in Attachment E. Culvert shall be a 12” diameter high-density polyethylene dual wall culvert with a smooth interior wall and corrugated exterior wall. Pipe joints shall have an integral inline bell and spigot joint meeting ASTM F2648. The joint shall be watertight in accordance with ASTM D3212. Gaskets shall meet the requirements of ASTM F477. Gaskets shall be installed by the pipe manufacturer and covered with a removable, protective wrap to ensure the gasket is free from debris. A joint lubricant available from the manufacturer shall be used on the gasket and bell during assembly. 12- through 60-inch diameters shall have an exterior bell wrap installed by the manufacturer. Refer to Attachment F for Trenching and Bedding details and to Attachment G for street restoration.

A Concrete Manhole Apron shall be installed around the lid of existing manholes were shown on Attachment B.

The apron shall be 24” wide and 6” thick and completely surround the manhole lid. The apron shall be reinforced with two #4 rebar continuously running around the circumference of the apron. One rebar shall be placed 6” from the outside edge and the other placed 6” from the inside edge of the apron.

The work required for the New Water Service and Accessories is shown on Attachment H. This work shall be coordinated with, and accepted by, the Wilderness Public Service District as it ties directly into the PSD’s distribution system. The New Service Water Meter shall be a 2” Sensus OMNI T2 and configured to work with Wilderness PSD’s remote read system. The existing main shall be hot-tapped (live) with a saddle and corporation stop at the location shown on Attachment H. Pipe shall be 2" PVC pressure pipe SDR-13.5 and rated for 315 PSI.

As with previous work items, all excavation and borings are unclassified. However, based on previous excavations in the area, it is anticipated that all excavation and boring will be in solid rock. No extra payment will be made for solid rock excavation or boring. A Bore and Jacked steel casing is required for installing the line under highway WV-129. The roadway in this area is also the sill of the Summersville Lake Emergency Spillway. There are 8" diameter steel reinforced vertical anchors located every 9-feet in the center and on each side of the roadway. if an anchor is encountered during boring, the contractor shall adjust the alignment of the bore to the left or right to miss the anchors. Refer to Attachment H for material specifications for the service tap and road bore.

A new polymer concrete Meter Box shall be installed at the location shown on Attachment H. The Meter Box shall contain the Sensus meter and a matching 2” meter valve. The meter valve shall be located upstream of the meter.

The Meter Box shall have an ANSI T22 load rating. Box dimensions shall be 24” (width), 36” (length), and 36” (depth) and the box shall have an open bottom. Meter box shall have pre-formed “mouseholes” at the bottom of the box for pipe entry/exit through the length dimension of the box (pipe shall be oriented in the center of the longest dimension). The Meter Box shall have a matching lid rated for incidental traffic loading with locking or bolt-down capability. The remote read transponder for the water meter shall be located on top of the lid. The Contractor shall ensure that the meter and transponder are installed per Wilderness PSD’s requirements and that it works with their remote read program.

A concrete vault shall be installed to house the required waterline accessories shall have an inside diameter of 6'-0" minimum and be fitted with a 36" x 36" traffic grade lockable aluminum hatch. The aluminum hatch access frames and covers shall have a ¼-in. thick one-piece, mill finish, extruded aluminum channel frame, incorporating a continuous concrete anchor. The inside of the frame shall have a door-support ledge on two (2) sides. Both frame and ledge must be supported by a full bed of Class A concrete. The door panel(s) shall be ¼- inch 5086-H34 aluminum diamond plate, reinforced to withstand a live load of the H-20 designation. Door shall open to 90 degrees and automatically lock with T-316 stainless steel hold-open arm with aluminum release handles. For ease of operation the hold-open arm shall incorporate enclosed stainless steel compression spring assist. Door(s) shall close flush with the frame. Hinges and all fastening hardware shall be T-316 stainless steel. Unit shall lock with T316 stainless steel slam lock with removable keys. Unit shall carry a Lifetime guarantee against defects in material and/or workmanship. The vault shall have a concrete floor that is sloped to a 4" outlet pipe. A 4" PVC drain line shall be laid on a 2% minimum slope and daylight at the ditch as shown on Attachment H. The end of the drain shall be fitted with an insect/rodent screen.

The concrete vault shall contain the following accessories, in this specific order, from the utility side towards the service side: ball valve, backflow preventer, dewatering tee, pressure reducing valve, and another ball valve. Ball valves shall be lead free bronze 2" ball valves. The Reduced Pressure Principle Backflow Preventer shall be a lead free unit designed to for drinking water applications to protect against both backsiphonage and backpressure of contaminated water into the potable water supply. The Reduced Pressure Backflow Preventer shall be a 2” diameter unit and comply with the following standards: ASSE 1013, AWWA C511, and NSF/ANSI/CAN 61. The Reduced Pressure Backflow Preventer shall be rated to 180°F and supplied with full port ball valves. The main body and access covers shall be low lead bronze (ASTM B 584), the seat ring and all internal polymers shall be noryl and the seat disc elastomers shall be silicone. The first and second check valves shall be accessible for maintenance without removing the relief valve or the entire device from the line. The dewatering tee shall be oriented with the branch pointing downward. The branch shall have two ball valves (redundant 2nd valve) installed and have a 6" drop pipe from the second valve. Valves shall be as described above. The end of the drop pipe shall have a threaded cap. A Lead Free Water Pressure Reducing Valve shall be installed as shown on the plans. The water pressure reducing valve shall be constructed using Lead Free materials. Lead Free regulators shall comply with state codes and standards, where applicable, requiring reduced lead content. Provision shall be made to permit the bypass flow of water back through the PRV into the main when pressures, due to thermal expansion on the outlet side of the valve, exceed the pressure in the main supply. Pressure reducing valves with built-in bypass check valves and strainer will be acceptable. Approved valves shall comply with ASSE 1003. Maximum working pressure shall be 300 PSI. Adjustable pressure range shall be 40-80 PSI. The contractor shall install a pressure gauge on the downstream side of the PRV. The end of the service line shall be fitted with a temporary cap and its location marked with a wooden stake. Refer to Attachment F for trenching and bedding requirements.

1.4 Restoration requirements: Any areas disturbed by construction shall be restored to their undisturbed (pre-project) condition before the end of the contract as shown in the attached plan drawings. Complete restoration shall consist of proper trench refill and surface treatment. Proper trench refill shall include adequate compaction. In forested and lawn areas, mound soil approximately two-inches higher than the surrounding grade to provide for possible settlement UNLESS this mound will interfere with surface drainage, then omit any mounding and grade to drain. Seed mixture shall contain at least 40% Creeping Red fescue, 40% Perennial Rye grass, and 20% Kentucky Bluegrass. Seed and mulch application rates shall be per the manufacturer’s instructions. All seeded areas shall be thoroughly mulched (straw) and watered. If installed between November and March, an equal amount of Annual Rye grass seed shall be added to the permanent seed mix to establish cover as soon as possible. Paved areas, including gravel, asphalt, or concrete surfaces shall be restored to their original grade and condition. All work, staging, and stockpile areas shall be thoroughly cleaned before the Contractor completes work at the site.

2.0 SUBMITTALS & SUBMITTAL PROCEDURES

2.1 General. The Contractor shall provide the submittals listed herein to the Government for approval. Units of weights and measures used on all submittals are to be the same as those used in the contract drawings and scope of work. Each submittal is to be complete and in sufficient detail to allow ready determination of compliance with contract requirements.

Submittals shall be submitted to the COR within 30 days of Contract award. Proposed deviations from the contract requirements are to be clearly identified. Include within submittals items such as: Contractor's, manufacturer's, or fabricator's drawings; descriptive literature including (but not limited to) catalog cuts, diagrams, operating charts or curves;

test reports; samples; O&M manuals (including parts list); certifications; warranties; and other such required submittals.

2.2 Required Submittals

Preconstruction Submittals (1-5) are required following Notice of Award and prior to the issuance of the Notice to Proceed. Material Submittals (6-17) are required prior to beginning the phase of work where they will be used.

Required submittals are:

1. List of proposed Subcontractors (if used)

2. Health and Safety Plan (Accident Prevention Plan)

3. Activity Hazard Analysis

4. Work Plan

5. Quality Control Plan

6. Waterline Pipe

7. All Fittings and Adapters

8. Gate Valves

9. Valve Boxes

10. Water Meters

12. Pressure Reducing Valves

13. Manhole Shop Drawings

14. Drop Inlet Shop Drawings

15. Culvert

16. All Service Tap Accessories (Backflow Preventer, PRV, valves, fittings, gauges, casing spacers, etc.)

17. Pipe Bedding and Aggregates

2.3 Disapproved or Rejected Submittals: Contractor shall make corrections required by the Contracting Officer.

If the Contractor considers any correction or notation on the return submittals to constitute a change to the contract drawings or specifications; notice as required under the clause entitled, "Changes," is to be given to the Contracting Officer. Contractor is responsible for the dimensions and design of connection details and construction of work.

Failure to point out deviations may result in the Government requiring rejection and removal of such work at the Contractor's expense. If changes are necessary to submittals, the Contractor shall make such revisions and submission of the submittals in accordance with the procedures above. No item of work requiring a submittal change is to be accomplished until the changed submittals are approved.

2.4 Approved/Accepted Submittals: The Contracting Officer's approval or acceptance of submittals is not to be construed as a complete check, and indicates only that design, general method of construction, materials, detailing, and other information appear to meet the Solicitation and Accepted Proposal. Approval or acceptance will not relieve the Contractor of the responsibility for any error which may exist, as the Contractor under the Contractor Quality Control (CQC) requirements of this contract is responsible for design, dimensions, all design extensions, such as the design of adequate connections and details, etc., and the satisfactory construction of all work. After submittals have been approved or accepted by the Contracting Officer, no resubmittal for the purpose of substituting materials or equipment will be considered unless accompanied by an explanation of why a substitution is necessary.

2.5 Withholding of Payment: No payment for materials incorporated in the work will be made if all required Government approvals have not been obtained. No payment will be made for any materials incorporated into the work for any conformance review submittals or information only submittals found to contain errors or deviations from the Solicitation or Accepted Proposal.

3.0 CONTRACTOR QUALITY CONTROL

The Contractor shall establish a quality control program to assure the requirements of the contract are provided as specified. The Contractor shall furnish for review by the Contracting Officer, not later than ten (10) days after receipt of notice of award, the Contractor Quality Control (CQC) Plan proposed to implement the requirements of Contract Clause 52.246.12 INSPECTION OF CONSTRUCTION. The CQC plan must be accepted prior to Notice to Proceed. The CQC plan shall identify all personnel, procedures, control, instructions, test, records, and forms to be used. Construction will be permitted to begin only after acceptance of the CQC Plan. If the Contractor fails to submit an acceptable CQC plan within the time herein prescribed, the Contracting Officer may refuse to allow construction operations to begin.

3.1 Acceptance of Plan: Acceptance of the Contractor's plan is required prior to Notice to Proceed. Acceptance is conditional and shall be predicated on satisfactory performance during the construction. The Government reserves the right to require the Contractor to make changes in his CQC plan and operations including removal of personnel, as necessary, to obtain the quality specified.

3.2 Control: Contractor Quality Control is the means by which the Contractor ensures that the construction, to include that of subcontractors and suppliers, complies with the requirements of the contract. The controls shall be adequate to cover all construction operations, including both on-site and off-site fabrication, and will be keyed to the proposed construction sequence. The controls shall include at least three (3) phases of control to be conducted by the CQC Manager for all definable features of work, as follows:

3.3 Documentation: The Contractor shall maintain current records of quality control operations, activities, and tests performed, including the work of subcontractors and suppliers. These records shall be on an acceptable form and shall include factual evidence that required quality control activities and tests have been performed.

3.4 Quality Assurance: The Government will monitor the Contractor's performance in this contract and reserves the rights to use whatever additional surveillance procedures are deemed appropriate. A Government employee shall not be considered an Inspector unless he identifies himself as such to the Contractor, the Contractor Representative, or Quality Control Inspector. No Inspector is authorized to change any provision of the specifications without written authorization of the Contracting Officer, nor shall the presence or absence of an inspector relieve the Contractor from any requirements of the Contract.

4.0 ENVIRONMENTAL PROTECTION

4.1 General. For the purpose of this specification, environmental protection is defined as the retention of the environment in its natural state to the greatest possible extent during project construction and to enhance the natural appearance in its final condition. Environmental protection requires consideration of air, water, and land and involves noise and solid waste management, as well as other pollutants. In order to prevent and to provide for abatement and control of, any environmental pollution arising from the construction activities in the performance of this contract, the Contractor and its subcontractors shall comply with all applicable Federal, State, and local laws and regulations concerning environmental pollution control and abatement. The Contractor's attention is invited to Drawing Numbers GI002 and EP001 concerning environmental protection measures. The Contractor shall comply with all applicable local, state, and federal environmental laws and regulations.

5.0 SUBMISSION OF OFFERS

5.1 Offer Format: The contract shall be bid as a Unit Price contract. The price for all work required shall be included in one of the bid items provided below. All incidental work shall be included under the bid item to which it most closely pertains. No other payments will be made. The Government may add or reduce quantities of bid items 2, 3, 4, and 6 at its discretion upon identification of additional needs. A Bid Schedule is included as Attachment I and includes the following bid items:

5.2 Bid Item Description:

Item 1 shall include the cost of all Performance and Payment Bonds and overhead associated with performance the work. Item 1 shall also include all mobilization, demobilization, and incidental costs not included in any other bid item. Mobilization costs shall include all costs associated with any activity required to completely establish materials, labor, and equipment on the project site to begin construction. Demobilization costs shall include all costs associated with any activity required to completely remove all materials, labor, and equipment from the project site after construction is complete. Incidental costs shall be defined as any other cost required to complete the work that is not included in any other bid item and shall include, but not be limited to: Security submittals, Accident Prevention Plans, Activity Hazard Analysis, Quality Control Plans, Work Plans, Environmental Protection Plans, Material Submittals, Overhead costs, Insurance costs, Project Meetings, Temporary Facilities, Closeout Submittals, Environmental Protection Measures, or any other item necessary to complete the project that is not included in another bid item.

Item 2 shall include all materials, labor, and equipment necessary to completely to REPLACE EXISTING VALVE IN EXISTING WATERLINE with new resilient wedge gate valves as described herein and shown on Attachments B and C; and shall specifically include, but not be limited to, excavation, rock excavation, removal and disposal of old valve, bedding material, pipe spools, fittings, gaskets, thrust blocking, joint restraints, valve box with lid marked “water”, valve box aprons, trench refill, compaction, grading, final grading, seeding, mulching (straw), flushing, leak testing, disinfection, final bacteriological lab testing, and any other incidental item required to completely replace existing waterline valves. No separate payment shall be made for different size valves. Valve size shall match the valve that is being replaced. Valves may range from 3/4” to 4”. Item 2 shall be bid, measured, and paid per EACH waterline valved replaced, regardless of size.

Item 3 shall include all materials, labor, and equipment necessary to completely INSTALL NEW VALVE IN EXISTING WATERLINE using resilient wedge gate valves as described herein and shown on Attachments B and C; and shall specifically include, but not be limited to, excavation, rock excavation, bedding material, pipe spools, fittings, gaskets, thrust blocking, joint restraints, valve box with lid marked “water”, valve box aprons, trench refill, compaction, grading, final grading, seeding, mulching (straw), flushing, leak testing, disinfection, final bacteriological lab testing, and any other incidental item required to completely install new waterline valves. No separate payment shall be made for different size valves. Valve size shall match the valve that is being replaced.

Valves may range from 3/4” to 4”. Item 3 shall be bid, measured, and paid per EACH waterline valved installed, regardless of size.

Item 4 shall include all materials, labor, and equipment necessary to completely INSTALL NEW FLOW METER IN EXISTING WATERLINE as described herein and shown on the Plan Drawings; and shall specifically include, but not be limited to, excavation, rock excavation, bedding material, water meters, water meter boxes with lid marked “water”, gaskets, thrust blocking, joint restraints, trench refill, compaction, grading, final grading, seeding, mulching (straw), watering, traffic control, flushing, leak testing, disinfection, final bacteriological lab testing, and any other incidental item required to completely install a new water in an existing line and the meter box to grade.

Item 4 shall be bid, measured, and paid per EACH Water Meter and Meter Box installed.

Item 5 shall include all materials, labor, and equipment necessary to completely INSTALL NEW PRESSURE REDUCING VALVE IN EXISTING VAULT at comfort stations as described herein and shown on the Plan Drawings; and shall specifically include, but not be limited to, removal of existing backflow preventers, installation of new pressure reducing valve, pipe spools, gaskets, joint restraints, trench refill, leak testing, disinfection, final bacteriological lab testing, and any other incidental item required to completely install New Pressure Reducing Valves in Existing Vaults. Item 5 shall be bid, measured, and paid per EACH Pressure Reducing Valves in Existing Vaults.

Item 6 shall include all materials, labor, and equipment necessary to completely INSTALL NEW PRESSURE REDUCING VALVE IN NEW VAULT as described herein and shown on the Plan Drawings; and shall specifically include, but not be limited to, excavation, rock excavation, bedding material, pressure reducing valve, ball valve, meter box with lid marked “water”, gaskets, spool pipe, gaskets, thrust blocking, joint restraints, trench refill, compaction, grading, final grading, seeding, mulching (straw), flushing, leak testing, disinfection, final bacteriological lab testing, and any other incidental item required to completely install the pressure reducing valve assembly. Item 6 shall be bid, measured, and paid per EACH New Pressure Reducing Valve in New Vault installed.

Item 7 shall include all materials, labor, and equipment necessary to completely INSTALL REPLACEMENT MANHOLE WITH DRAIN as described herein and shown on Attachments B and D; and shall specifically include, but not be limited to, excavation, rock excavation, bedding material, manhole base, forming flow channels, placing concrete, mastic, grout, joint sealer, sealing around existing pipes, manhole riser section, manhole flat top slab, manhole lid marked “sewer”, perforated drain pipe, graded filter stone, non-perforated drain pipe on grade to drop inlet, pipe bedding, refill, compaction, grading, final grading, seeding, mulching (straw), leak testing, and any other incidental item required to completely install the Replacement Manhole with Drain. Item 7 shall be bid, measured, and paid as a LUMP SUM for the Replacement Manhole with Drain installed.

Item 8 shall include all materials, labor, and equipment necessary to completely INSTALL DROP BOX INLET AND 12” CULVERT as described herein and shown on Attachments B and D; and shall specifically include, but not be limited to, excavation, rock excavation, bedding material, concrete drop box inlet, cast iron grating, mastic, connection to 12” Culvert, connection to 6” Drain, grout, joint sealer, pipe bedding, 12” CPP culvert, refill, compaction, grading, final grading, seeding, mulching (straw), and any other incidental item required to completely install the Drop Box Inlet . Item 8 shall be bid, measured, and paid as a LUMP SUM for the Drop Box Inlet and 12” Culvert installed.

Item 9 shall include all materials, labor, and equipment necessary to completely INSTALL CONCRETE MANHOLE APRON as described herein and shown on Attachment B; and shall specifically include, but not be limited to, excavation, concrete, steel reinforcement, final grading, and any other incidental item required to completely install the Concrete Manhole Apron. Item 9 shall be bid, measured, and paid per EACH Concrete Manhole Apron installed.

Item 10 shall include all materials, labor, and equipment necessary to completely INSTALL NEW WATER SERVICE AND ACCESSORIES as described herein and shown on Attachment H; and shall specifically include, but not be limited to, excavation, rock excavation, bore and jack casing, bedding material, hot tapping the existing water main, water meter, meter box, valves, concrete vault, aluminum hatch, vault drain, backflow preventer, tees, pressure reducing valve, refill, compaction, grading, final grading, seeding, mulching (straw), leak testing, traffic control, and any other incidental item required to completely install the New Water Service and Accessories. Item 10 shall be bid, measured, and paid as a LUMP SUM for the New Water Service and Accessories installed.

6. SPECIAL CONTRACT REQUIREMENTS

6.1 Site Visit/Project Points of Contact: Bidders are highly encouraged to make a site visit prior to bidding to ascertain the complete requirements of the contract. Bidder shall contact the following before coming on-site:

6.2 Hours of Operation

a. The contractor shall perform all work during the following hours: Monday through Friday, 0700-1600 except Federal holidays. Contractor shall submit work schedule to Resource Manager two (2) weeks prior to work activities for review and approval. The existing water system shall remain operational throughout construction. Facilities shall be out of service only when transferring to the new system.

b. Federal Holidays: Normally, all work will be performed on the basis of one (1) 8-hour shift per day between the hours of 0700 and 1600 Eastern Time Zone, five (5) calendar days a week. Unless other times and days are approved by the Contracting Officer. The normal work week will run from Monday through Friday, excluding holidays. Work that interferes with other ongoing functions at the project shall be scheduled around normal working hours to be approved by Project Manager. Work shall be scheduled around the Federal holidays listed below unless approved in advance. Eleven Federal holidays observed are as follows:

• New Year’s Day (January 1)

• Martin Luther King Day (3rd Monday in January)

• President’s Day (3rd Monday in February)

• Memorial Day (Last Monday in May)

• Juneteenth (June 19)

• Independence Day (July 4)

• Labor Day (1st Monday in September)

• Columbus Day (2nd Monday in October)

• Veterans Day (November 11)

• Thanksgiving Day (4th Thursday in November)

• Christmas Day (December 25)

When one of the above-designated holidays falls on a Sunday, the following Monday is observed as a legal holiday.

When a legal holiday falls on a Saturday, the preceding Friday is observed as a holiday. If the contractor plans to work outside the normal working hours or on a Federal Holiday, the contractor must notify the Contracting Officer or his representative in writing one (1) week prior to beginning an alternate work schedule.”

6.3 Contractor's Responsibilities: The Contractor shall be held responsible for any and all damages attributable to the actions of the Contractor or its subcontractors outside the limits of construction, shown on the drawings or designated by the Contracting Officer. This shall include damage to existing roads, drainage structures, electrical or telephone facilities, pavement, and other structures and facilities. All such damage shall be repaired to the satisfaction of the Government or proper municipal or state authorities by the Contractor and at his expense.

6.4 Pre-Construction Conference: The Contractor shall attend a pre-construction conference prior to the initiation of any work at the site. The Contractor’s Accident Prevention Plan (APP) and Quality Control Plan (QCP) must be accepted prior to this meeting. The location of this meeting (construction site or office) will be determined after contract award.

7. SECURITY REQUIREMENTS

All Contractors and subcontractor employee(s) working on any contract who require access in and/or around any Huntington District facilities, projects, owned or leased facility/space shall receive authorization to be on site at these areas based upon a favorable background investigation in accordance with “Homeland Security Presidential

Directive 12: Policy for a Common Identification Standard for Federal Employees and Contractors” (HSPD 12).

The Huntington District Security Management Office shall submit contractor(s) personal information to complete the basic background investigation in order to make a recommendation to the Contracting Office of an employee’s suitability for work on Government property. The Procuring Contracting Officer (PCO) has the right to deny any contractor or subcontractor employee from working on any contract based upon the results of the background investigation provided by the Huntington District Security Management Office. The Government, Contractor, and subcontractors shall not permit any employee(s) access to drawings, blueprints, records, photos, sites, or projects until the Huntington District Security Management Office has completed an Operations Security (OPSEC) review of such items and determined they can be released.

FOR ALL CONTRACTS 1-YEAR AND SUBSEQUENT OPTION YEARS:

Contractors shall submit a complete investigation packet for each contractor and subcontractor employee to the Huntington District Security Management office within 10 working days after award of any contract, or prior to the individual being permitted unescorted access in or on District facilities.

The Contractor must also provide the following: (1) Name of the Contract Specialist with whom he/she is working,

(2) Project Name, (3) Contract Number (4) Task Order Number when applicable, and (5) Security Information Sheet on each employee working on site must be submitted directly to the Huntington District Security Management Office.

The Contractor must provide a full legal name, complete SSN#, Date of Birth, Place of Birth (City and State) and Driver’s license number if available. It is the Contractor’s responsibility to ensure the above information is accurate and legible, otherwise delays may occur.

Contractors shall have background checks completed for their employees annually in the case of Contracts with option periods. For instance, if the employee was cleared in May of 2018, they shall be cleared again in May of 2019. It is the Contractor’s responsibility to ensure that each employee’s security is up to date. Failure to comply may lead to possible removal from the project/jobsite.

The Contractor must ensure if new employees are brought in to work on an existing Contract with the U.S. Army Corps of Engineers (USACE), they must process the employee through the Huntington District USACE Security Management Office for background purposes.
