STATEMENT OF WORK
BUILDING 362 ENTRANCE FLOORING
GYMNASIUM LAUGHLIN AFB
1.0. DESCRIPTION OF SERVICES: The Contractor shall furnish all materials, labor, tools, equipment, transportation, supervision, and all other incidentals necessary to complete the work outlined in this Statement of Work (SOW) at Laughlin Air Force Base in Building #362
Gymnasium. A total of 236 Square Feet shall be replaced to durable, slip-resistant flooring. The durable, slip-resistant floor will also have 30 sq ft of “12” tile for the entrance. Another 30 sq ft of “9” tile shall be installed by the side walls as well. A total of 60 sq ft of “12” and “9” tile shall be used to border the durable, slip-resistant flooring. The Contractor shall ensure management oversight throughout the accomplishment of this contract. The Period of Performance for this work shall be 60 working days from the Notice to Proceed (NTP) from the time material is delivered.
2.0. DESCRIPTION OF PRODUCT:

3.0. PERIOD OF PERFORMANCE: The contractor shall notify the government 14 working days before they start the project. The total number of days it should take to complete this project after the material has been delivered is 10 working days.
4.0. PLACE OF PERFORMANCE: Installation of all products to occur in Building 362. Contractor will be able to bring materials and stage materials in the designated area agreed upon with project point of contact.
5.0. LOCATION, STORAGE, AND PARKING: Storage for the new durable, slip-resistant flooring aluminum floors shall be available in the back hallway of the fitness center Picture provided.
6.0. PROJECT SCHEDULE: Contractor shall ensure installation is scheduled with the POC. The
Work Shall be performed Monday through Friday 0600 to 1800 and Saturdays are available upon request. Except Federal Holidays and scheduled with Government Representative. Any deviation from the hours of operations shall be approved by the CO in writing. This project is not going to have any phases.
7.0. WORK REQUIREMENTS:
7.1. Demolish all existing carpet from the front entrance. Remove all the permanent adhesive from ground before installing new durable, slip-resistant flooring and tile.
7.2. 236 Sq ft of durable, slip-resistant flooring shall be installed.
7.3 The existing cove base will be replaced with new cove base.
7.4 The durable slip-resistant flooring installed is going to have tile bordering around it, due to manufacturer recommendations. It shall have “12” tile in front of each entrance and “9” on the side walls.
7.5. Repair any damage to the property caused by the contractor at no cost to the Government.
7.6. The contractor is responsible for fully investigating the current condition of the work zone in Building 362. This includes on-site inspections and surveys to gather all necessary information and data required to complete the project”.
7.7. The Contractor is responsible for removing and disposing of all construction debris, including old carpet, tile, and waste. This includes providing a dumpster for on-site collection. Laughlin will designate a location for the dumpster.
8.0. PRE-INSTALLATION INSPECTION: No pre inspection will be needed.
9.0. QUALITY ASSURANCE: The government shall inspect all work performed by the Contractor.
Work performance shall be deemed satisfactory and acceptable to the government when the
Government representative has reviewed the installation for accuracy, declares the project
3 | P a g e satisfactory. The contractor shall repair or replace damaged items at no additional cost to the government.
10.0 BASE FIRE REGULATIONS: All work shall be in strict compliance with NFPA-1 The Fire
Code, DFMAN 91-203 AIR FORCE OCCUPATIONAL SAFETY, FIRE, AND HEALTH
STANDARDS, and all applicable Occupation Safety and Health Administration (OSHA)
Standards. . Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities.
11.0. COMPLIANCE WITH LAWS: The Contractor shall comply and ensure that all subcontractors comply with all applicable federal, state, and local laws, regulations, ordinances, and standards.
12.0. SITE-MAI N T E N A N C E The Contractor shall protect adjacent property, offices and their contents from dust, dirt or other materials. Work areas shall be maintained in a neat, clean, safe condition and shall, at a minimum, be cleaned with all waste and packing materials removed at the end of each shift. Contractor shall maintain clearances through corridors for occupants to pass. Once installation is complete, the contractor shall be responsible for any warranty and service calls throughout the manufacturer’s warranty period. Once warranty and service calls have been requested, the contractor shall follow the manufacturer’s warranty terms and conditions, and work with a government representative until issues are resolved.
13.0. SPECIFIC CONDITIONS/DEFINITIONS:
13.1. Installers shall have a clean environment for each phasing flooring installation. Installers will work around electrical outlets in or around the floors, taking care not to cover any of the outlets.
13.2. Contractor shall provide the full names, birth dates, social security number and driver’s license numbers of each installer associated with this installation as required for sponsorship onto Laughlin AFB Del Rio TX. Work time schedules and the number of installation crews must be provided to the Contracting Officer prior to the start of work.
Such information shall be submitted to the contracting office two weeks prior to the start of work. Base access will be granted to those listed on the above-mentioned document for the duration of the project.
13.3. At no time shall the contractor accept direction or changes in any type of product obtained, configuration or installation without the written approval of the Contracting officer.
13.4. Contractor/subcontractor working on classified, sensitive, and/or high- risk projects will report any suspicious activity or suspicious contacts by personnel to Office of Special
Investigations (OSI) (Eagle Eye at 830-298-5158). Any person finding classified information will immediately secure it (do not open/read/examine) and contact Security
Forces Squadron at 830-298-5100 during non-duty hours and inform them a patrol response is necessary to take control of classified document(s)/material(s). All contracts regardless of cleared or uncleared status will receive INFOSEC Security Education Awareness
Training, (SETA) from the Host Unit Security Manager (USM) under the 47 Flying Training
Wing/Information Protection (47 FTW/IP) office.

13.5 All personnel on Laughlin Air Force Base are required to take environmental
Management System Awareness Training per DAFMAN-32-7001.
Installation Access Requirements:
An Entry Authority List (EAL) of personnel will be provided by the contractor to their sponsor (i.e., CE). EAL’s must be filled out correctly in the original EAL format and submitted a minimum of 72 hours prior to the arrival of contractors. Any modifications to the EAL template will result in an automatic kickback restarting the
72-hour requirement. Department of Defense standards require a complete background vetting of all personnel requesting access onto a military installation.
The EAL must contain at a minimum:
• Full Name
• Full Social Security Number (or Green Card Number)
• Date and Place of Birth
• Driver’s License Number and State Citizenship
• Laughlin AFB Sponsor Name
• Laughlin AFB Sponsor Phone Number
• Arrival (or contract start date) Date
• Departure (or contract completion date) Date
• Expected location of duty
• Reason for access
Escorting: Is not available for contractors entering LAFB. All contractors must be sponsored on either through the EAL or through a day pass obtained in the visitor center.
NOTE: A valid identity proofing document (Official Passport, United States Driver’s
License, Employment Authorization Document with Photograph, a Federal, State, or Local
Government Identification Card) must be submitted with the EAL.
13.6. The contractor is advised IAW DoDI 2000.16_DAFI 31-145 all contractors must complete Level I – AT Awareness Training within 30 days of requiring access, and annually thereafter. For non-Common Access Card holder personnel, training can be accomplished at Joint Knowledge Online (JKO) website at https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf.
All contractor employees shall comply with base Operations Plans/Instructions for
FPCON procedures, Random Antiterrorism Measures (RAMS) and local search/identification requirements. The Contractor shall safeguard all government property, including controlled forms, provided for Contractor use. At the close of each work period, government training equipment, facilities, support equipment, and other valuable materials shall be secured.

All contractors are not authorized entry into controlled areas unless properly escorted or authorized by owner/user. Report any suspicious activity to Security Forces either by dialing 911 or 830-298-5100

14.0. OSHA PUBLIC LAW 91
Comply with the OSHA Public Law 91-596, Occupational Safety and Health Act of 1970 and the resulting standards (e.g., 29 CFR 1910, Occupational Safety and Health Standards, 29 CFR
1926, Safety and Health Regulations for Construction), national consensus standards of the
United States (e.g., National Fire Protection Association and National Electrical Codes), and all applicable federal, state, local, and Department of Defense, environmental, safety, and occupational health statues, policies, and regulations). It is the contractor’s responsibility to make certain that all safety requirements, equipment, and training are met and provided to their employees. Additionally, the contractor is responsible to ensure compliance with safety and health standards by all sub-contractor employees. Air Force Safety, Fire Protection, and
Bioenvironmental Engineering officials may periodically enter a contractor’s workplace to verify working conditions of personnel or to make inspections for protection of Government facilities, personnel, and equipment. If an improper procedure or unsafe condition exists, the contractor shall notify the applicable Wing Representative/CO and immediately correct the unsafe condition within 24 hours after occurrence. Department of Labor OSHA inspectors are authorized right of entry to inspect any place of employment operated by an Air Force contractor.
They are, for the most part, “NO NOTICE” inspections. Notify the COR if an OSHA inspector visits the site unescorted by Air Force Safety Personnel.
