PERFORMANCE WORK STATEMENT
JOHN MARTIN DAM ELEVATOR MAINTENANCE
CONTRACT
PART 1 GENERAL INFORMATION
1.0 General: This is a non-personal services contract to provide maintenance and repair services for the John Martin Elevator. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contractservice providers shall be accountable solely to the primary contractor who, in turn is responsible to the Government.
1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, other items, non-personal services necessary to inspect, test, maintain and repair the John Martin Dam Elevator as defined in this Performance Work Statement (PWS), in strict accordance with all terms, conditions, general and special provisions and specifications except for those items specified as Government furnished property and services. Equipment covered by this contract includes all mechanical, control, electrical, hoisting, testing weights and all other elevator equipment and elevator components up to the point of disconnect with the building power. The contractor shall perform to thestandards in this contract in accordance with current versions of ASME A17.1-2019/CSA B44:19 and ASME A17.3-2017.
The contractor shall be licensed by the State of (New Mexico / Colorado) and shallprovidedocumentation of this license.
The Contractor shall be responsible for the planning and estimating; material acquisition; and scheduling and accomplishment of all work (maintenance and repair) described in this PWS except for instances where the Government has agreed to furnish resources. It is highly encouraged that the Contractor visit the facility to determine the extent of work requirements and formulate an operational sequence necessary for completion of the contract. The Contractor shall thoroughly familiarize themselves with the electric traction elevator and associated equipment located at the John Martin Dam to ensure against misunderstanding between skills and material needed duringthe performance period of the PWS. Before work begins, a Maintenance Control Program (MCP) schedule shall be prepared by the Contractor and reviewed/approved by the Contracting Officer andthe John Martin Project Manager (POC).
1.2 Background: The John Martin Dam elevator wasreplaced by Kone Elevator in 2010. The Minnesota Elevator Inc. (MEI) elevator will providebetter, more reliable service. Replacing parts will now be easier due to this upgrade. However, the elevator is still in need of a comprehensive maintenance program that will maximize performance, safety, and life span. The elevator is an overhead traction control elevator with 480 volts, 3 phase, and 60 hertz power supply. The elevator operates at a speed of 125, capacity is 1,500lbswith three (3) openings(operating gallery, air venting gallery, roadway).
1.3 Objectives: The objective is to provide quarterly and annual inspections, make routine repairs and maintenance / adjustments and complete the 5-year load test asrequired by code to keep elevator in operable condition.
1.4 Scope: The Contractor shall service the elevator and all elevator equipment described in section 5 of this document. During these quarterly inspections, the contractor shall lubricate and grease all applicable elevator components, inspect and notify the John Martin Dam project POC of any repairs or component replacement that, in the contractor’s judgment, should be accomplished. Repair and/or replace components as described in section 5 of this document. The contractor’s contract manager, or alternate, shall coordinate services as detailed in section 1.6.3.1.
1.5 Period of Performance: The period of performance shallbe for one (1) Base Year of 12 months and four(4) 12-month option years. The Period of Performance reads as follows:
Base Year: Date of Award – December 31, 2022 (5-Yr Load Test/Quarterly Inspection/Annual Inspection)
Option Year I: January 1, 2023 - December 31, 2023 (Quarterly Inspection/Annual Inspection) Option Year II: January 1, 2024 - December 31, 2024(Quarterly Inspection/Annual Inspection) Option Year III: January 1, 2025 - December 31, 2025 (Quarterly Inspection/Annual Inspection) Option Year IV: January 1, 2026 – December 31, 2026(Quarterly Inspection/Annual Inspection)
1.6 General Information:
1.6.1 Place of Performance: The work to be performed under this contract willbe performedatthe North Tower, at John Martin Dam, Bent County, Colorado.
1.6.2 Type of Contract: The Government will award a Firm Fixed-Price, Performance-based contract for the services describedherein.
1.6.3 Hours of Operation: The contractor is responsible for conducting business between thehours of 7:00 AM to 4:00 PM Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. For other than firm fixed price contracts, the contractor will not be reimbursed when the Government facility is closed for the above reasons. The contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the contractor shallkeep in mindthatthestability and continuityof theworkforceare essential.
1.6.3.1 Coordination
Normal Service Requests (Routine testing, inspection, maintenance and repair) shall be accomplished during the hours of 0730-1600, Monday through Friday, excluding Federal Holidays. The contractor’s contract manager, or alternate, shall return calls from the John Martin Dam project POC within 1-day.
After Hours Emergency Service Requests are always to be provided on a 24 hour/day basis to reduce potential entrapment time of personnel in the elevator. Response time for After Hours Emergency Service Requests is detailed in section 5. The Contractor shall be responsible for coordinating site access with the John Martin Project Office POC if it is necessary to work outsideof these hours.
1.6.4 Recognized Holidays: The following holidays are recognized as Federal holidays and Government offices will not be open.

1.6.5 Quality Control: The Contractor shall develop and maintain an effective Quality Control Program (QCP) to ensure services are performed in accordance with this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services.
The Contractor’s QCP is the means by which they assure themselves that their work complies with the requirement of the contract. The QCP will be submitted to the Dam Project Staff and to the District Office for acceptance within 1 week of award of contract.
1.6.6 Security Requirements:
1.6.6.1 Physical Security: The contractor shall be responsible for safeguarding all Government equipment, information and property provided for contractor use. At the close of each work period, Government facilities, equipment, and materials shall be secured.
1.6.6.2 Per the guidelines mandated with the implementation of OPORD 2013-74: Integrating Antiterrorism and Operations Security(OPSEC) into the USACE Acquisition Process, thefollowing OPSEC provisions apply to this requirement:
Access and General Protection/Security Policy and Procedures: All contractor and all associated sub- contractor’s employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks or background investigation and to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the Government may require changes in contractor security matters or processes.
For contractors who do not require CAC but require access to a DoD facility or installation.
Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC- III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05 / AR 190-13), applicable installation, facility andarea commander installation/facility access andlocalsecuritypolicies andprocedures(provided by government representative, as NCIC and TSDB are available), or, at OCONUS locations, in accordance with status of forces agreements andother theater regulations.
For Contracts that Require an OPSEC Standing Operating Procedure/Plan.
The Contractor shall develop an OPSEC SOP/Plan within 90 days of contract award. The OPSEC SOP/Plan must be reviewed and approved by the RA OPSEC Officer. The SOP/Plan will include the government's critical information, why it needs to be protected, where it is located, who is responsible for it and how to protect it. In addition, the contractor shall identify an individual who will be an OPSEC Coordinator.
Pre-screen candidates using E-Verify Program: The Contractor must pre-screen Candidates using the E- verify Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the Contracting Officer Representative (COR) if appointed, or the POC no later than 3 business days after the initial contracta ward.
1.6.7 Special Qualifications: The Contractor shall ensure that all trades personnel employed for work under this contract are at least on the experience level of a journeyman, except for helpers and apprentices, who shall be under the direct supervision of a journeyman or above. All journeymen must have current credentials and have completed an appropriate accredited apprenticeship program or have equivalent skills and knowledge acquired through training and experience and possess any industry standard licensing specific to their field of work. Journeyman on site need to possess a current (New Mexico /Colorado) Conveyance Inspector/Mechanics License.
1.6.8 Post Award Conference/Periodic Progress Meetings: The contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The Contracting Officer, Contracting Officer’s Representative (COR) or POC, and other Governmentpersonnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the Contracting officer will apprise the contractor of how the Government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additionalcostto the Government.
1.6.9 Contracting Officer’s Representative (COR): The COR, if appointed, will be identified by separate letter. The COR monitors all technical aspects of the contract and assists in contract administration.
The COR is authorized toperform the followingfunctions: assurethat the contractor performsthe technicalrequirements of the contract: perform inspections necessary in connection with contract performance; maintain written and oral communications with the contractor concerning technicalaspects of the contract; issue written interpretations of technical requirements, including Government drawings, designs, specifications; monitor contractor's performance and notifies both the Contracting Officer and contractor of any deficiencies; coordinate availability of Government furnished property, and provide site entry of contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting contract.
1.6.10 Contractor Key Personnel
The followingpersonnelareconsideredkeycontractor personnelbythe Government and willbe submittedbyname to the John Martin Point of Contact(POC):
Contract Manager: The Contractor shall provide a Contract Manager who shall be responsible for the performance of the work. The Contract Manager or alternate shall have full authority to act for the Contractor on all contract matters relating to daily operation of this contract. This individual is responsible overalladministration and coordination of the Contractor’s work.
Alternate Contract Manager: An alternate with full project manager authority shall be designated to actfor the projectmanager during absences such as vacation, illness, etc.
Maintenance / Repair Personnel: As part of the Maintenance Control Program (MCP), the contractor shall submit the names and qualifications of proposed maintenance and repair personnel, including any resumes, certifications, licenses, etc. required to document qualifications. At a minimum, the contractor shall submit licenses and proof of qualifications for all specialized personnel that will be performing work on this contract.
Elevator Inspector: Any work or testing requiring inspection by an Elevator Inspector, as indicated in ASME A17.1-2019/CSA B44:19 and ASME A17.3-2017 and according to the Coloradoconveyance regulations and industry code shall require an Colorado State Licensed Elevator Inspector who is currently certified andqualified.
As part of the proposal, the contractor shall submit the names and qualifications of proposed maintenance and repair personnel, including any resumes, certifications, licenses, etc. required to document qualifications. The contractor will associate the personnel with appropriate tasks as defined in the MCP. In addition, the contractor shall provide the phone numbers for their emergency call service, and programthe emergency cab telephone to call this number, within 15 calendar days after contracta ward.
1.6.11 Government Key Personnel
The followingare key Government personnelassociated with this PWS:
The USACE Project Manager / John Martin Project Manager (POC) is the Government's representative responsible for the management of the project. Questions regarding technicalissues under this contract should be directed to this individual. The USACE Project Manager does not have the authority to change the terms or conditions of this contract including time and cost. The Contractor will be notified, in writing, of any changes in the USACE Project Managers.

1.6.12 Identification of Contractor Employees: All contract personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor productsor that contractor participation is appropriately disclosed
1.6.13 Safety:
1.6.13.1 U.S. Army Corps of Engineers’ Safety and Health Requirements Manual, EM 385-1-1 and OSHA Requirements: The contractor willbe required to comply with allpertinent provisions of thelatestversionof the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385 -1-1, in effect on the date of the contract award. EM 385-1-1 and its changes are available at http://publications.usace.army.mil/publications by selecting Engineer Manuals, EM 385-1-1 “Safety and Occupational Health”. The Contractor must also comply with Occupational Safety and Health Act (OSHA) standards. OSHA standards are subject to change. It is the Contractor's responsibility to maintainfamiliaritywith current OSHA standards.
1.6.13.2 Zero Accident Program: The Albuquerque District, in its continual pursuit of excellence in safety has established a goal of zero contractor injuries. To attain this goal, it will be necessary for the contractor management personnel to communicate to the workers an expectation of zero injuries; that it is no longer acceptable to take chances; that shortcuts taken because of laziness or even while trying to do an efficient job are no longer welcomed; that praise for shortcuts or chance-takingwill not exist. It is imperative that contractor management clearly sets forth the expectation for zero injuries so that the workers can begin to believe that the company is truly serious about safety. From the standpoint of worker psychology, zero is the only supportable goal. In attaining zero-injury performance, there is no substitute for the concept of setting and communicating safety performance expectations to the workers.
PART 2 DEFINITIONS &
ACRONYMS
2.0 Definitions and Acronyms:
2.1 Definitions:
2.1.1 Contractor: A supplier or vendor a warded a contract to provide specific supplies or service to the Government. The term usedin this contract refers to the prime.
2.1.2 Contracting Officer (KO): A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the Government. Note: The only individual who can legally bind the Government.
2.1.3 Contracting Officer’s Representative (COR): An employee of the U.S. Government appointed by the Contracting Officer to administer the contract. Such appointment shall be in writing and shall state the scope of authorityand limitations. This individual has authority to providetechnicaldirectionto the contractor as longas that direction is within the scope of the contract, does not constitute a change, and has no funding implications.
This individualdoes NOT have authority to change the terms andconditions of the contract.
2.1.4 Defective Service: A service output that does not meet the standard of performance associated with the Performance Work Statement.
2.1.5 Deliverable: Anything that can be physically delivered, but may include non-manufactured things such as meetingminutes or reports.
2.1.6 Key Personnel: Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitmentfrom the persons that are listed in the proposal.
2.1.7 Physical Security: Actions thatprevent theloss or damage of Government property.
2.1.8 Quality Assurance (QA): The Government procedures to verify that services being performed by the contractor are performed according to acceptable standards.
2.1.9 Quality Assurance Surveillance Plan (QASP): An organized written document specifying the surveillancemethodology to be used for surveillance of contractor performance.
2.1.10 Quality Control (QC): All necessary measures taken by the contractor to assure that the quality of an end productor service shallmeetcontract requirements.
2.1.11 Subcontractor: One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.
2.1.12 Work Day: The number of hours per daythe contractor provides services in accordancewith the contract.
2.1.13 Work Week: Mondaythrough Friday, unless specifiedotherwise.

PART 3
GOVERNMENT FURNISHED PROPERTY, EQUIPMENT AND
SERVICES
3.0 Government Furnished Items and Services:
3.1 Utilities: The Government will provide water and electricity to the contractor. The Contractor sha l instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the wasteof utilities, which include turningoff thewater faucets or valvesafter use.
3.2 Equipment: The Government willnot provide any equipment for this contract.
3.3 Materials: The Governmentwill provide the followingmaterialfor this contract.
3.3.1 Manuals / Parts Catalogs / As-Builts
The Government will assist the contractor with copies of Operation / Maintenance Manuals and Parts Catalogs if they exist and are available in the Governmentshistoricalrecords.
PART 4
CONTRACTOR FURNISHED ITEMS AND SERVICES
4.1 General: The Contractor shall furnish all labor, supplies, equipment, and services required to perform work under this contract that are not listed under Section 3 of this PWS.
4.2 Materials
The Contractor shall provide all supplies, parts and materials required to perform inspection, maintenance, repair, and component replacement as required to maintain the elevators in accordance with the manufacturer's specifications to accomplish Normal Routine Service Requests (Quarterly, Annual, 5-Year) as stated in Part 5.
4.3 Repair Parts
The Contractor shall provide, at no additional cost, all the necessary repair parts, materials and supplies to maintain, service, inspect and test the elevators as required by this contract to conduct Normal Service Requests.
Necessary expendable supplies common to the trade will not be reimbursed and shall be included in the price of the service request. The Contractor is required to use replacement parts, which are equal to or greater than the quality of the part being replaced. In addition, ASME A17.1 details additional requirements for repairs to specific parts and equipment. See section 3.3 for Government Materials and section 4.2.2. for Material Exclusions.
4.4 Exclusions
The Contractor shallnot assume responsibility for the followingitems, which are not included in this contract:
Other items necessitated by vandalism, negligence, or misuse of the equipment by persons other than the contractor, his representative and employees, excluding wear and tear. Contractor shall obtain District’s written approval to repair vandal related problems. Payment will not be made for any unauthorizedwork.
New attachments as may be recommended or directed by insurance companies or by federal, state, municipal, or other governmentauthorities.
Car enclosures and related items, including, but not limited to, fixed or removable panels, door panels, car gates, plenum chambers, hung ceilings, light diffusers, fluorescent tubes, dry cell batteries, handrails, mirrors, floor coverings, andother architectural features a nd accessories;
Power supply feeder circuits to the machineroom circuit breakers.
Computer and microprocessor devices not exclusively dedicated to the elevator equipment such as terminalkeyboards anddisplayunits.
Communications equipment, such as telephones, intercoms, heat detectors, and smoke sensors, which were not installed by the Contractor or theoriginalelevator installer.
Major Repairs: Any individual unit or incident of repair with a total estimated cost (labor and direct material costs) exceeding $3,000.00 which is not covered under Normal Service Requests (Part 5), is not covered by this contract but will be negotiated on an as need basis.
Such repairs will normally be accomplished by separate purchase order or contract. This exclusion does not apply if the repair is to correct damage causedby Contractor negligence.
4.3 Equipment: The Contractor shallprovide allequipment, and/or tools neededfor this contract.
PART 5
SPECIFIC
TASKS
5.0 Specific Tasks:
5.1 Basic Services
Each year of the term of this contract (base year and/or optional years) the contractor shall provide maintenance, routine/minor repairs and services, quarterly and annual inspections for the John Martin Dam elevator located at the North Tower of the Dam. The contractor shall complete the 5-year load test when the deadline for this test falls within the contractor’s base year and/or optionalyears for the John Martin Dam elevator located at the North Tower of the Dam. The contractor will also respond to normal out of cycle service calls to address minor problems and entrapments during normalbusiness hours. These calls shall be limited to 4 calls per year. Additionally, the contractor will also respond to Emergency After Hours service calls primarily to address cases of entrapment and fire. These calls shall be limited to 4 calls per year. As well as the aforementioned service requests, the contractor will provide servicerequests as described in section 5.5 suchas Callback Service Requests.
5.2 Limits of Basic Services
The Contractor shall provide trained elevator personnel with the appropriate tools and testing equipment for scheduled maintenance, unscheduled repairs, emergency repairs/assistance, safety inspection, and safety testing as required by this contract. The Contractor shallprovideatno additionalcost all the necessaryrepair parts (see section
4.2.2 for exclusions), materials and supplies to maintain, service, inspect and test the elevators as required by this contract. As part of the basic services thecontractor is responsible for thefollowing:
All parts, equipment, labor and overhead associatedwith routine repair work of a minor nature.
This work, identified byinspectionor service request, is required toreturnelectric tractionelevator and associated equipment located at John Martin Project Dam to a state of repair and efficiency suitable for the elevator and associated to effectively perform its intended function, except as noted in section 4.2.2.
All parts, equipment, labor and overhead associated with 4-service requests, per Calendar Year, is provided by the contractor after being notified of a problem by the John Martin Project Office POC. Includes all costs incurred by the contractor, except as noted in section 4.2.2.
All parts, equipment, labor and overhead associated with callback service requests provided by the contractor after being notified of a problem by the John Martin Project Office POC. Work is associated with prior work, within a 72-hour period, completed by the contractor that has been determined to be defective, incomplete and/or reoccurring malfunction. Work is identified as routine repair work which includes all costs incurred by the contractor, except as noted in section 4.2.2.
All parts, equipment, labor and overhead associated with quarterly inspections and 5-year loadtest during normal working hours. Includes all costs incurred by the contractor, except as noted in section 4.2.2.
5.3 Summary of Basic Services
The Contractor shall provide full service to meet routine maintenance requirements as discovered or required during or because of the quarterly / annual inspections. The Contractor shall maintain elevators so that the elevators operate in a safe andefficient condition. In the eventof a break down, the Contractor shall make every effort to immediately return the elevator to an operating condition. During the quarterly inspections, any required or necessary maintenance or repair shall be brought to the attention of the John Martin Project Office POC and annotated in the Work Completion Report. If repairs exceed the contractor’s limits of responsibility and are considereda Non-Routine Major Service Request, the contractor shalladhere to the requirements as set in section
5.5.6 (Non-Routine Major Service Request). The contractor shall examine all elevator equipment for optimum operation. The contractor shall repair and adjust elevator parts and components, clean and lubricate the system for smooth and efficient performance, maximizing performance and safe operation. The examination, cleaning, lubrication andadjustment, androutine repairs will cover, at a minimum, thefollowingcomponents:

• Controlandland positioning systems
• Signalfixtures
• Machines, drives, governors, sheaves (re-groove) andwire ropes
• Hoist Motors
• Power units, pumps, valves and jacks
• Car andhoistwaydoor operatingdevices and door protection equipment

• Dismantle, clean, examine, replace worn parts, lubricate, reassemble, and adjust brake plunger assemblies
• Examine, lubricate, adjust, repair andreplacecar & corridor operatingstations, car & corridor hangers & tracks, door operating devices, door gibs and car fans
The objective of scheduled routine maintenance is to eliminate or minimize elevator malfunction, breakdown and deterioration. Contract maintenance of the elevator must assure continuous, safe, and satisfactory operation of all elevators, their parts and components. The Contractor shall schedule routine maintenance to include all tasks herein described, in addition to routine lubrication and adjustments. The repair of mechanical items / components typically involves disassembling components; repair/replacement of worn or damaged parts; adjusting, cleaning and lubricating movingparts; andreassembling/reinstalling the original item/component or replacing with a new component in the original position. All parts and components (including kits, assemblies and subassemblies) required for maintenance and repair of all facilities and equipment except as noted in section 4.2.2.
5.3 Summary of Inspection and Testing Services
Each Calendar Year of the term of this contract (base year and/or optional years) the contractor shall provide quarterly inspections for the John Martin Dam elevator located at the North Tower of the Dam. In addition, once per Calendar Year, the contractor will conduct a Category 1 annual test and inspection and once per every 5 calendar years from the date of the last Category 5 load test / inspection, the contractor will conduct a Category 5 annual test and inspection of the electric traction elevator located at John Martin Project Dam. The contractor shallcomplete these tests and inspections in accordance with current versions of ASME A17.1-2019/CSA B44:19 and ASME A17.3-2017. Also, the contractor shall provide periodic safety testing as outlined and in accordance with current versions of ASME A17.1 and ASME A17.3 or the governing statecode of jurisdiction.
5.4 Maintenance Control Program
The contractor shall perform service in accordance with current versions of ASME A17.1-2019/CSA B44:19 and ASME A17.3-2017. In particular, the sections of the code requiring a Maintenance Control Program (MCP). As a part of the bid package, the contractor shall submit their proposal, including their Maintenance Control Program (MCP). The MCP shall display a schedule of type of Normal Service Requests with associated estimated hours per task, and the total number of hours per Quarter that the contractor will be performing the required maintenance, excluding travel time. In addition, the MCP shall specify examinations, tests, cleaning, lubrication, and adjustments to applicable components at regular intervals, and any other details specific to the proposed maintenance schedule. Upon award, the successful bidder’s proposal, including their MCP, shall become a part of the contract, and any proposed changes to the plan will require negotiation and issuance of a contract modification prior to implementation of the proposed change. The MCP shall only include maintenance and inspection procedures applicable to the electric traction elevator located at John Martin Project Dam. As part of the MCP, the contractor shall submit to the POC a description of the scheduled routine maintenance and inspection tasks which the Contractor plans to provide. This schedule of tasks shall be supplied to the John Martin POC seven business days prior to the contractor performing any Quarterly, Annual, or 5-Year Normal Service Request. The Contractor shall prepare this schedule and task description in a checklist format similar to Technical Attachment 3. The John Martin Project Office POC must approve the proposed “Scheduled Routine Maintenance Task Checklist” prior to contract work commencement. The contractor shall meet or exceed these requirements set within the approved MCP. The MCP shall include documentation of tasks and records to include maintenance, repair, replacements and tests performed and as required by ASME A17.1-2019/CSA B44:19 and ASME A17.3-2017. The successful bidder’s MCP shall become theproperty of the Governmentafter the MCP is approved and implemented.
5.4 Full Coverage Parts Repairand Replacement
As part of the basic services of this PWS, the contractor will provide full coverage parts repair and/or replacement for all components worn due to normal wear and tear. The contractor shall have an inventory of parts to support this effort. All replacement parts shall be new or refurbished to meet typical industry standards. The contractor shall re-lamp all signals as needed during the regularly scheduled visits. The contractor’s representative shall be a mechanic that has all the training and skills necessary to make “on the spot” repairs and replacements and shall not be, just a lubricating technician.
5.5 Service Requests
During scheduled Quarterly, Annual and 5-Year Normal Service Requests, the contractor shall perform inspections and repairs as called for in the approved MCP and as identified by findings the contractor discovers during the execution of work of the approved MCP. The contractor is expected to work until the task is completed. Extended work outside the normal hours of operation is not authorized for Normal Service Requests. If multiple days are needed to accomplish tasks associated with the Normal Service Requests, the contractor shall plan accordingly. All work associated with a Service Request shall be finished before the Government approves any payment for which it is invoiced. The Contractor shall provide appropriate laborer / mechanic / journeyman who is appropriately trained, certified and possesses the correct licensure for the tasks assigned by the Service Request and MCP. All necessary, expendable parts supplies andequipmentcommonto thetrade andrequired toexecute Normal Service Requests will not be reimbursed but shall be included in bid price of the Normal Service Request as identified in the MCP. During the Service Request, the Contractor shall inspect the site, determine necessary repairs, and notify the John Martin Project Office POC immediately if a repair exceeds the contractor’s limits of responsibility as defined by this PWS. If the repair or service exceeds the contractor’s limits of responsibility, the service call will be classified as a Non- Routine Major Service Request and follow the appropriate associated action.
5.5.1 Quarterly Normal Service Requests
Once per quarter per year, the contractor shall conducta quarterly periodic maintenance, repairs andinspections of the electric traction elevator located at John Martin Project Dam. The contractor shall perform all recommended maintenance, inspections and associated repairs that is pertinent to the John Martin Project Dam elevator referenced in A17.1-2019/CSA B44:19 and ASME A17.3-2017. The Contractor shallperform and documentallsystem testing, inspection findings and maintenance in their Work Completion Report and submit a copy to the John Martin Project Office POC prior to submitting a Payment Invoice. Work shall be accomplished and performed during normal working hours as coordinated with John Martin Project Office POC. This scheduled service request will be annotated in the contractors proposed MCP. Results of the quarterly inspection and associated maintenance actions will be annotated in a detailed Work Completion Report. Quarterly Service Request shall be referred to as a Normal or Routine Service Call.
5.5.2 Annual Normal Service Request (Category 1)
Once per year, the contractor shall conduct a Category 1 Annual test and inspection of the electric traction elevator located at John Martin Project Dam. The contractor shall perform all recommended testing / inspection that is pertinent to the John Martin Project Dam elevator as shown in ASME A17.1-2019/CSA B44:19, Table N-1, Category 1 elevator. The Contractor shall perform and document all system testing / inspection findings in their Work Completion Report and submit a copy to the John Martin Project Office POC prior to submitting a Payment Invoice. Work shall be accomplished and performed during normal working hours as coordinated with John Martin
Project Office POC. This scheduled service request will be annotated in the contractors proposed MCP. Results of the annual test and inspection will be annotated in a detailed Work Completion Report and specific memo detailing inspection and results from the contractor. Work for the Annual Service Request (Category 1) shall be referred to as a Normalor Routine Service Call.
5.5.3 5-Year Normal Service Request (Category 5)
Once per every 5 years from the date of the last Category 5 inspection, the contractor shall conduct a Category 5 Annual test and inspection of the electric traction elevator located at John Martin Project Dam. The contractor shall perform all recommended testing / inspection that is pertinent to the John Martin Project Dam elevator as shown in ASME A17.1-2019/CSA B44:19, Table N-1, Category 1 elevator. The Contractor shall perform and document all system testing / inspection findings in their Work Completion Report and submit a copy to the John Martin Project Office POC prior to submitting a Payment Invoice. Work shall be accomplished and performed during normal working hours as coordinated with John Martin Project Office POC. This scheduled service request will be annotatedin the contractors proposed MCP. Results of the Category 5 test and inspection will be annotated in a detailed Work Completion Report and specific memo detailing inspection and results from the contractor. Work f or the Category 5 shallbe referred to as a Normal or Routine Service Call.
5.5.4 Out of Cycle Service Request (4– Times Annually)
A maximum of 4-times annually, the contractor shall provide, at no extra cost, a non-emergency response service.
The contractor shall ensure a trained elevator mechanic will be on site, within one working day, to trouble shoot and repair an elevator malfunction identified by John Martin Dam personnel for the electric traction elevator located at John Martin Project Dam. The Contractor shall perform anddocument all repairs, system testing / inspection findings in their Work Completion Report and submit a copy to the John Martin Project Office POC prior to submitting a Payment Invoice. Work shall be accomplished and performed during normal working hours as coordinated with John Martin Project Office POC. This scheduled service request will be annotated in the contractorsproposed MCP. Work and associated maintenance actions will be annotated in a detailed Work Completion Report. Work for the Out of Cycle Service Requestshallbe referred to as a Normalor Routine Service Call.
5.5.5 After Hours Emergency Service Request (1– Times Annually) A maximum of 1-times annually, the contractor shall provide an emergency response service. Work to be accomplished and performed at other than normal work hours as directed by the John Martin Project Office POC (or designated alternate), also referred to as an Emergency Service Call. The Contractor shall provide, a 24 hours/day, 7 days/week, 365-day/year coverage for emergency trouble calls not to exceed 1. A trained mechanic, with appropriate certification, licensure, shall be “on call” and shall be on site within a two-hour time period of the placement of an emergency trouble call by the POC (or designated alternate). Emergency situations include people trapped in an elevator car, the suspicion/confirmation of a fire in or around elevator equipment, or an inoperative elevator with no suitable backup. Any work or maintenance actions completed as a After Hours Emergency Service Request will be annotated in a detailed Work Completion Report Work.
5.5.6 Non-Routine Major Service Request
Work exceeding the contractor’s limits of responsibility is determined to be a Non-Routine Major Service Request.
Any finding discovered during a Normal Service Request or incident of repair with a total estimated cost (labor and direct materialcosts) exceeding $3,000.00 which is not coveredunder routine maintenance as identifiedby the MCP, is not covered by this contract. Work identified by the contractor as exceeding the limits of their responsibility shallbe stopped immediately andreported tothe John Martin Project Office POC and the Contracting Officer. An estimate for completion of workshallbe submittedto the John Martin Project Office POC andthe Contracting Officer within two working days except for emergency work, which shall be submitted at the earliest possible time but not later than the end of the following workday. No work shall be performed by the contractor exceeding the scope of this PWS without prior permission from the Contracting Officer and acknowledgement of John Martin Project Office POC. In case of an emergency work requirement, without the coordination and direction of the POC and the Contracting Officer. Before proceeding with work the Contractor shall establish costlimitation for the approved work with the POC and Contracting Officer. The Government reserves the right to determine how these repairs are to be handled. This exclusiondoes not apply if the repair is to correct damage caused by Contractor negligence.
Any work or maintenanceactions completed as a Non-Routine Major Service Requestwill be annotated in a detailed Work Completion Report Work.
5.5.7 Callback Service Request
When an elevator which was previously worked on by the contractor’s personnel, has a repeat malfunction within a 72-hour period, the Contractor shall be obligated to provide, at no extra cost, a return visit by a trained elevator mechanic to correct the problem, even if the problem is minor in nature. The elevator personnel shall respond to this callback withina three-hour timeperiodregardless of whattimethe POC made thecallback complaint, including the “after hours” time periods. Any work or maintenance actions completed as a Callback Service Request will be annotated in a detailed Work Completion Report Work. Callback service requests are not included in the 4-service-call limit.
5.6 Work Completion Report
At the completion of any work/inspection/test, and prior to leaving the site, the contractor shall provide written documentation of all work that was performed. The Work Completion Report shall include a description of what work was accomplished, and the date and time the job/work was completed and indicate if the work that was performed was maintenance or repair. The Contractor shall also notate the total number of hours worked on site.
This information willbe used as a quality checkof conformancewith the Contractor’s Maintenance Control Program (MCP). In addition, the contractor shall also list any parts, components and equipment that were repaired or replaced. In no case shall the Work Completion Report exceed cost limits established from the Contractor’s estimate. The Work Completion Report shall be reviewed with the John Martin Project Office POC, or their alternate. The POC, or alternate’s, shall sign and notate if they agree with the report or if there is a discrepancy;
however, signing of the Work Completion Report shall not be interpreted as acceptance of costs that exceed previously agreed to costs. The Contractor shall leave one copy of the Work Completion Report onsite and retain a copy for submissionwith the Payment Invoice.
5.7 Payment Invoice
The Contractor shall submit a payment invoice to the John Martin Project Office POC with the quarterly QC Report for the previous month’s contract operations. The Contractor will only be paid for tasks performed as specified in this PWS. Actualwork/tasks accomplished by the Out of Cycle, Non-Routine Major and After-Hours Emergency Service Requests will be invoiced to the John Martin Dam Project separately and not be included with Quarterly Normal Service Requests invoices. Anywork exceeding the limitations of this PWS will need to be approved by the Contracting Officer prior to execution of effort by the Contractor. The combination of labor, actual cost of parts, equipment, materials, procured services, administrative costs, profit and overhead will be used as the total payment for normal repairs and for work exceeding the Contractor’s limits of responsibility. The contractor shall furnish proof of repair part retail costs, or procured services costs, by submittal of original invoice, catalog price, or other documentation acceptable to the POC. The Contractor shall also furnish copies of all Work Completion Reports as documentation of work performed.
