Statement of Work (SOW)
STATEMENT OF WORK (SOW)
for
NATURAL GAS LEAK DETECTION
SURVEY
Malmstrom Air Force Base
MONTANA
2 December 2022
SECTION 1
DESCRIPTION OF SERVICE
1. SUMMARY OF EXPECTATIONS
1.1. The Contractor shall provide all labor, hardware, equipment, and incidentals necessary to perform and conduct a mobile natural gas line leak detection survey of all distribution mains, a walking gas leak survey of all commercial and residential gas services up to and including the meter sets, and above ground distribution piping and distribution equipment at Malmstrom AFB, MT. Motor vehicles must be equipped with a multiple intake system of adequate width to obtain samples of air at least eight feet in wide along the path of the survey. The Contractor shall check detection instruments which shall be capable of detecting gas concentrations of at least 50 ppm, for proper operation and calibrate each instrument with a known gas sample per the manufacturer’s specifications before using the instruments to conduct the surveys and provide proof of calibration to the Contracting Officer’s Representative. The contractor shall also include a project work plan, quality control plan, a kickoff meeting via phone, in brief meeting with site personnel, natural gas survey and leak pinpointing, out brief meeting, and a Leak Detection Survey Report with field logs, natural gas leak detection forms, a leak location map, a prioritized list, and budgetary repair cost estimates of the natural gas leaks that were identified during the field survey. The gas leak survey shall be conducted after May 15th and shall not be conducted on snow covered or frozen ground.
1.2. This Statement of Work (SOW) will cover service lines, up to 114,000 ft of gas mains and up to 5,000 testing locations. Each testing location may contain multiple survey points.
1.3 This survey will be conducted in accordance with the following regulatory requirements. 49
Code of Federal Regulations (CFR) 192.723 treating the base as a business district. Leaks will be located and classified using methods and techniques identified in Chapter 4 of Leak Detection of the United States Department of Transportation (USDOT) Pipeline and Hazardous Material Safety Administration (PHMSA) Guidance Manual for Operators of Small natural Gas Systems (current edition). United States Department of Transportation Pipeline Safety Regulations listed in the 49 CFR Parts 190 to 199. Montana Rule 38.5.2202 Incorporation by Referencing of Federal Pipeline Safety Regulations, located in USDOT Pipeline Safety Regulations, CFR, Title 49, chapter 1, subchapter D, parts 191, 192, and 193, including all revisions and amendments enacted by USDOT on or before November 14, 2022.
1.4 The Contractor shall be responsible for obtaining a EAL for Base Access, a Veted CEAL for personal and equipment to access to the Weapons Storage Area (if the WSA option is awarded), processing his own Work Clearance Request (AF Form 103), commonly known as a “Digging Permit,” and coordinating with Government inspectors as required. The Contractor is responsible for obtaining all coordination signatures for the permit. The approved permit procedures and guidelines shall be the responsibility of the Contractor.
1.5 The Contractor shall provide proof of instrument calibration prior to the start of work as well as a Final Report in the form of a CD within 10 days after the completion of work.

1.6 The location of the project areas is shown in the Figures at the end of this document.
2. HOURS OF OPERATION
2.1. Normal Hours. The contractor shall perform all the routine services required under this contract during the following hours: 0700 through 1600 Monday through Friday, with the exception of Federal Holidays, or when the base is officially closed by the base commander.
2.2. Recognized Holidays. The contractor is not required to provide routine service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. NOTE: Normally, if the holiday falls on Saturday, it is observed on Friday. If the holiday falls on Sunday, it is observed on Monday.
3. CONTRACTOR EMPLOYEES
3.1. Employee Certifications. Contractor shall have valid Contractor’s license with the State of Montana. General Contractors are required to have license or hire Sub-Contractors with valid Contractors license with the State of Montana Contractors Licensing Board.
4. ENVIRONMENTAL, SAFETY, AND HEALTH
4.1. The Contractor shall comply with all existing Occupational Safety and Health Act (OSHA) standards of safety at all times in the performance of this contract. Hazards to the safe use of the premises due to the Contractor's work and/or equipment shall be suitably marked at all times. Pedestrian and vehicle traffic ways shall be kept clear and unobstructed.
4.2. The Contractor shall immediately report any and all injuries and damage involving Contractor, his employees, Air Force civilian, or military personnel to the Quality Assurance Evaluator (QAE) and the Wing Safety Office.
4.3. The Contractor shall cooperate fully and assist Government personnel in any Government investigation of the mishap, until released by the mishap investigative authority through the Contracting Officer. Safety personnel shall have the right, at any time, to investigate and report all mishaps involving Air Force personnel and property.
The Contracting Officer and Contractor Safety Representative may accompany Air Force mishap investigation personnel but may not release information regarding the mishap without written approval from the Air Force Safety investigating official or senior installation commander.
4.4. The Contractor shall maintain records of Contractor employee’s occupational injuries, illnesses, and other related activities as required.
4.5. The Contractor shall comply with the highest degree of environmental protection or safety practices where any disagreements between requirements may exist.
4.6. The Contractor shall be liable and responsible for determining and maintaining compliance with all applicable Federal, State, and local environmental laws or regulations.
4.7. The Contractor shall be liable and responsible for the reporting and clean-up of any hazardous chemical discharge or spills resulting from Contractor negligence.
4.8. The Contractor shall be liable and responsible for all costs resulting from non- compliance, whether accidental or intended. This includes any fines or corrective actions assessed by any regulatory or governing agency due to the actions or omissions of the Contractor in carrying out his duties.
5. MISHAP NOTIFICATION AND REPORTING. The contractor shall immediately report any and all injuries and damage involving contractor, Air Force civilian or military personnel to the QAE and the Wing Safety Office.
5.1. Cooperate fully and assist government personnel in any government investigation of the mishap, until released by the mishap investigative authority through the CO. Safety personnel shall have the right, at any time, to investigate and report all mishaps involving Air Force personnel and property. The CO and contractor Safety representative may accompany Air Force mishap investigation personnel but may not release information regarding the mishap without written approval of the AF Safety investigating official or senior installation commander.
5.2. Maintain records of contractor employee’s occupational injuries, illnesses, and other related activities as required.
6. FIRE PREVENTION. Establish and implement procedures to ensure careful handling and storage of flammable and combustible products IAW National Fire Protection Association (NFPA) Standard 30 and National Fire Codes.
7. SECURITY REQUIREMENTS
7.1 Contractor will be required to comply with all security measures while on Malmstrom AFB. Contractor will submit a list of employees requiring access to the base at least seven days prior to work start.
7.2 Activities of the Contractor and Contractor's employees and sub-contractors and their employees while on base will be in accordance with base regulations, including those of the Fire Marshal as well as security directives. This includes, but is not limited to, giving way to all emergency vehicles that are in response, properly wearing seat belts at all times, and refraining from use of cell phones while driving except when using a hands-free device.
8. ANTI-TERRORISM (AT) AWARENESS TRAINING.
8.1 In accordance with DoDI O-2000.16v1_AFI 10-245-O, Antiterrorism (AT) Program Implementation, ensure Level I - AT Awareness Training is completed by all contractors within 30 days of requiring access, and annually thereafter. The training will be provided by the government. The objective of the training is to provide personnel with requisite knowledge necessary to remain vigilant and enable employment of AT tactics, techniques, and procedures.
9. FORCE PROTECTION.
9.1 Contractor personnel are required to abide by all real-world Force Protection Condition (FPCON) measures, random anti-terrorism measures (RAMs), shelter-in-place orders, evacuations and lock-down notices as directed. Examples include mandatory badge or ID checks, facility single points-of-entry and searches of hand carried items.
10. HOUSEKEEPING AND CUSTODIAL SERVICES. The contractor must maintain the contractor assigned areas in a clean, safe, and sanitary condition. Remove trash from all the work areas and store all flammables in an approved flammable storage container and ensure the area is left in an orderly manner at the end of the duty day.
10.1. Sanitary Facilities. The Contractor’s employees may use the nearest sanitary facilities. The Contractor shall not use facilities in controlled or restricted areas IAW security requirements.
10.2. Refuse Collection. Waste generated by the Contractor in performance of the services specified with this SOW shall be disposed of by contractor off base in compliance with State and
Local ordinances, including hazardous materials in which proper disposal instructions shall be complied with. All trash, debris, and refuse caused by the job shall be removed and disposed and shall be deposited in an approved receptacle or container furnished by the Contractor. The job site shall be cleaned at the end of each workday.
11. ADMINISTRATIVE RECORDS. All records, documents, and associated papers provided by the government and generated during the period of this contract become government property and will remain in place upon contract termination or completion. During the period of the contract, records, documents, and associated papers shall be available for review.
12. INSPECTION.
12.1 Quality Assurance Evaluator. The 341st Civil Engineering Squadron will provide a Quality Assurance Evaluator (QAE) to verify accomplishment of work. The QAE will be the spokesman for compliance with base standards. Controversies between the QAE and the Contractor will be resolved by the Contracting Officer. If, for some reason, a change in the primary or alternate QAE is required, the Contractor will be notified in writing by the Contracting Officer. The Contractor shall coordinate Contractor activities with the 341st Civil Engineering Squadron.
12.2. Final Inspection. The Contractor will notify the QAE of their estimated work completion date at least 3 days in advance, to verify accomplishment of work and receive contractor work report. Any deficiencies found shall be rectified by the Contractor and the Contractor shall inform the QAE upon completion.
