STATEMENT OF WORK

LAKE FORK ERFO ROAD REPAIR

C.1 Scope of Contract This project consists of repairing damage to Forest Road 50070, Lake Fork Road. The road was severely damaged during 2021 monsoon storms, hitting a recent burn area above the roadway. The work will consist of culvert replacement, gravel fill, drainage grading, rock riprap placement, general roadway repair, surface replacement and function of the roadway.

The quantities and items in the contract are in the schedule of items. The intent of the contract is to provide for the complete construction of the project described in the contract. Unless otherwise provided, the contractor is to furnish all labor, materials, equipment, tools, transportation, and supplies and perform all work required to complete the project in conformity with the drawings and specifications, and in accordance with the provisions of the contract. Payment for contract work will be made only for and under those pay items included in the Schedule of Items. All other work and materials will be considered as incidental to and included in the payment for the items shown in the Schedule.

C.2 Period of Performance and Project Location

Period of Performance: Estimated start date: June 15, 2024, with work being completed with 150 calendar days after start date/commencement of work. All work, including clean up, must be completed by September 15, 2025.

Location: Manti La Sal National Forest, Ferron-Price Ranger District in Price, Utah.

Forest Road 50070 – Lake Fork Rd - Utah County, Utah. The Lake Fork Canyon ERFO Road Repair project begins approximately 19 miles from Spanish Fork City. Forest road 50070 (Lake Fork Canyon) begins off Highway 89 near the junction of us Highway 6 at Thistle, UT.

From Spanish Fork, travel east on HWY 6 approximately 12.5 miles to the junction of US-89 to Mt. Pleasant/Ephraim/Manti, travel 1.4 miles and turn left onto Lake Fork Road, then travel approximately 3 miles to site. (Coordinates: 39.96313, -111.45649).

C.3 Magnitude of Construction – FAR 36.204 Magnitude of Construction Project Price Range - $1,000, 000 - $5,000,000 (Base Bid) Price Range - $1,000, 000 – $10,000,000 (Bid Option Items: 1 - 3)

C.4 Technical Requirements and Specifications Specific construction drawings and Supplemental Project Specifications are included in Section J. All items shown or noted on plans, which are not specifically bid items, are considered indirect payment items, per FP-14, Section 109.05(b). The cost of furnishing and installing all indirect payment items will not be paid for separately but shall be included in the unit price bid for other items unless noted otherwise.

The Forest Service, US Department of agriculture has adopted FP-14 - “Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects” from “U.S. Department of Transportation” Federal Highway Administration for construction of National Forest System Roads. C.5 Site Information and Limitations The following site conditions are considered incidental to the contract and the contractor will not be paid directly for any of the following items:

(a) Utilities are not available at the site for construction purposes.

(b) No fuel storage on site is permitted. Fuel trucks must be equipped with spill containment kits and portable berms that meet all state and federal regulations. A spill containment plan must be submitted and approved prior to construction.

(c) All construction equipment shall be pressure washed before entering the Site in compliance with the specifications. The removal of mud and debris from treads, tracks and undercarriage, with emphasis on axles, frame, cross-members, motor mounts, and underneath steps, running boards, and front bumper/brush guard assemblies will be required. The purpose is to reduce or eliminate the transportation of noxious weeds, which is required by Federal and State regulations. The contractor shall notify the Contracting Officer 48 hours in advance of mobilizing equipment to the site to arrange an inspection and meet the requirements of FSSS 171 Weed Prevention.

(d) The contractor will be able to utilize the Forest Service Roads; however, the contractor will be responsible for any damage to the roads due to construction activities. The contractor shall be responsible for obtaining permission to use all other roads and staging areas.

C.6 Traffic Control and Construction Signing

No work that endangers, interferes, or conflicts with traffic or access to work sites shall be performed until a plan for satisfactory warning and handling of traffic has been submitted by the contractor and approved by the COR (Contracting Officer Representative). Construction signing for traffic control shall conform to the Manual of Uniform Traffic Control Devices (MUTCD).

All traffic control signs will be placed in areas adequate for a truck pulling a fifth wheel trailer to be turned around. Contractor shall not be paid directly for this item, rather it will be considered incidental to other items of work listed in the Bid Schedule.

C.7 Work camps, Staging and Storage Areas

Areas for staging operations and storage of materials shall be approved by the Contracting Officer. The contractor must request in writing for approval from the Contracting Officer to stage trailers (work or housing) on site.

C.8 INSPECTION OF WORKSITE

The contractor acknowledges they have taken the necessary steps to ascertain the nature and location of work and have investigated and satisfied themselves as to the general and local conditions, including unclassified material for excavation, that can affect the work or its cost.

Any failure of the contractor to take the actions described and acknowledged in this paragraph will not relieve the contractor from the responsibility of estimating properly the difficulty and cost of successfully performing the work, or for proceeding to successfully perform the work without additional expenses to the government.

The USDA Forest Service anticipates issuance of a solicitation in early spring 2024 with all project related documents based on the responses to this inquiry. Interested contractors who respond to the said solicitation must be actively registered in SAM (System for Award Management, www.sam.gov). No response to this correspondence is necessary if your firm is not interested or lacks capacity to perform the work.
