About this file
This document consists of attachment pages from a Minnesota state solicitation for EZ Lift products, specifically the S500PN-03 model (500 lb Smart Stand without scale, 3" Caster, Standard). The file contains responder declarations, pricing instructions, and additional information requirements that vendors must complete as part of their proposal submission. The solicitation requires responders to enter bid prices directly into the SWIFT system at the unit line level rather than using a separate price schedule document. Responders should direct technical questions about the lifts to Elissa Laumb as referenced in the Request for Bids, and those needing assistance with the SWIFT system can contact the Vendor Assistance Help Desk at (651) 201-8100.
Responders must certify that their submission information is accurate and reliable, that they have authority to bind their organization contractually, and that they have not engaged in collusion with other vendors. Vendors are required to disclose any organizational conflicts of interest and obtain necessary approvals for any copyrighted material included in their response. The pricing section permits alterations or erasures to bid prices only when clearly marked and authenticated by an authorized representative. Responders must also provide contractor contact information for both contract management and purchase order receipt, specify any prompt pay discounts offered (with options ranging from 1% to 5% at various payment terms or Net 30), and indicate their delivery timeframe after receipt of order.
