ATTACHMENT 7: ADDITIONAL BUSINESS PROPOSAL
INSTRUCTIONS AND UNIFORM COST ASSUMPTIONS
CLINICAL CENTRAL LABORATORY SERVICES

In addition to the format requirements for the Business Proposal that are contained in Section L of the solicitation, the information presented in this section of the RFP is intended to provide uniform cost assumptions and business clarifications.
Offerors are advised to give careful consideration to the Statement of Work, all reference material provided as attachments, the Technical Evaluation Criteria, and, the RFP as a whole, in the development of your proposal. The information requested in these instructions should be used as a guide for the development and formatting of your Business Proposal. Offerors should consider and include the information requested here, as well as any other information which will benefit the proposal.
BUSINESS PROPOSAL – TABLE OF CONTENTS
SECTION 1 – PROPOSAL COVER SHEET (use form NIH 2043 identified in Section J)
SECTION 2 – COST OR PRICE SUPPORT
Section L of the RFP specifies the minimum documentation requirements for cost data and all cost related support. All related documentation must be included in the proposal in a clearly marked section.
SECTION 3 – UNIFORM COST ASSUMPTIONS
The following uniform cost assumptions shall be used by all offerors when preparing their cost proposal. The assumptions have been added to facilitate the development of the business proposal.
The government anticipates receiving a fixed-price schedule per unit cost.
A. Assume the Project Manager and Data Manager will attend one (1), one-hour, teleconference per Task Order to facilitate contract initiation and study set-up.
B. Assume the contractor will provide investigator training one (1) time, via webinar, per Task Order.
C. Assume a 30% screen failure rate.
D. Assume an additional 10% of samples to include unscheduled visits and repeat labs required for adverse event (AE) follow-up.
1) Scope of Activities
The list below represents the average number of specific tasks offerors should assume to perform each year under the contract. This list is provided to indicate the general scope of work of the contract. The NIAID anticipates a 7-year IDIQ contract ordering period.

Assume that the period of performance of Task Orders may range from less than one year to multiple years.
The maximum number of clinical sites to participate in these studies is 40. This number may change, up or down, depending on the requirements of each protocol.
The quantities identified in this solicitation are estimates only. The actual quantities may be higher or lower.

6) Government Furnished Equipment (GFE)
Government Furnished Equipment available to be transferred from incumbent contractor.
The purchase of Government Furnished Equipment will not be authorized as a direct charge under this contract.

Past Performance Data, Including References
Section L of the RFP specifies the minimum documentation requirements for providing past performance information. This information must be included in with the offeror’s proposal submission. All related documentation must be included in the offeror’s proposal in a clearly marked section.
