Construction work includes subgrade preparation with 12" Class 2 Aggregate Base compacted to 95% relative compaction, installation of 4" thick concrete reinforced with steel at 12" on center and dowels at 18" on center, hydrovac excavation for electrical trenches, installation of two 2" RGC above-ground and PVC Schedule 40 underground electrical conduits with pull wire, two 125 Amp breakers at the existing electrical source, a Christy box, UFER ground installation, and an 8' copper ground rod assembly with specified hardware. The contractor must provide all labor, materials, equipment, and administrative support while coordinating with the Contracting Officer's Representative (COR). The Period of Performance requires completion of all tasks within 180 calendar days after Notice to Proceed, with work hours between 7:00 AM and 4:00 PM, Monday through Friday. The contractor must submit a detailed Schedule of Values and project schedule within 14 calendar days of NTP, develop and execute a QA/QC plan with three inspection/control phases, submit project-specific plans (Construction Safety, Environmental Protection, Fire Safety, Quality Control, and Waste Management) within 10 Federal workdays, and conduct pre-final and final walkthroughs with punch list documentation. VA will provide project drawings and specifications, with a 15 Federal workday review period for submittals and 10 Federal workday review period for RFIs. The SOW includes extensive security and information protection requirements applicable to federal contracts involving VA facilities, including HIPAA compliance, data breach protocols with liquidated damages of $37.50 per affected individual, mandatory annual privacy training, and security control assessments, though this specific construction project does not require access to Protected Health Information or classified data.

That only the minimum necessary patient information is used to provide appropriate service to the correct patient.

That the undersigned understands that all parties are bound by the requirements of the “Health Insurance Portability and Accountability Act of 1996” which provide guidance on the protection of patient privacy and confidentiality.
