Attachment: Optional Past Performance Form
INSTRUCTIONS--CONTRACTOR PERFORMANCE ASSESSMENT REPORT
1) The solicitation requests that all past performance information submitted to be evaluated for RFQ 15JPSS22Q00000037 be entered into the CPAR system for the Contracting Officer to review in response to the solicitation.
2) Offerors can also provide an attachment of a CPAR record in final status for consideration of any past performance information. Final status refers to the individual report specific to draft status or final status. Interim reports for a contract or order with multiple ordering periods is acceptable.
3) The final option would be offerors to submit the completed form below with their response to the solicitation. The completed form must be signed by the contracting official for the past performance reference that is submitted. Offerors that submit this form to document past performance should not count this document towards the technical response page limit.
4) The form can be signed and submitted by the COR or the CO. The description must be complete. A rating must be provided with a narrative that supports the rating. The recommendation should also be completed by the COR or CO for the contract provided to support past performance. All ratings shall follow the Federal Acquisition Regulation guidance for contract performance assessment ratings or they may not be considered in the evaluation of past performance for the offeror.
