About this file
This document contains a past performance reference questionnaire template and instructions for a USAID solicitation. The questionnaire is to be completed by an offeror's references and contracting personnel to provide feedback on the offeror's past performance on relevant contracts. It requests information on contract details, scope of work, quality of service, schedule compliance, cost control, and management. Respondents provide ratings in these areas using exceptional, satisfactory, and unsatisfactory designations. It also collects narrative feedback and asks if the respondent would work with the offeror again. Once filled out, the questionnaires must be submitted directly to USAID by the specified due date before the closing time of solicitation number 72051923R00004 for the Central America Regional Support Services requirement.
