Instructions:
1. Labor Categories; Please fill in column B should your firms labor category title differ from Column A.
2. Labor Rates; In columns E-I, please provide fully burdened labor rates (includes all direct and indirect costs and profit) for each contract period.
3. Should you wish to propose additional labor categories, fill in all yellow highlighted areas in- rows 23 - 30.
3. Provide a material mark up rate not to exceed 10%

The labor and material mark-up rates provided herein shall be used when the Government elects to use the Time and Material Contract Line Items. (CLINS, 0002, 1003, 2003, 3003, and 4003)
